A Review of SHV Extended-Spectrum β-Lactamases: Neglected Yet Ubiquitous

beta-lactamases are the primary cause of resistance to beta-lactams among members of the family Enterobacteriaceae. SHV enzymes have emerged in Enterobacteriaceae causing infections in health care in the last decades of the Twentieth century, and they are now observed in isolates in different epidemiological settings both in human, animal and the environment. Likely originated from a chromosomal penicillinase of Kiebsiella pneumoniae, SHV beta-lactamases currently encompass a large number of allelic variants including extended-spectrum beta-lactamases (ESBL), non-ESBL and several not classified variants. SHV enzymes have evolved from a narrow- to an extended-spectrum of hydrolyzing activity, including monobactams and carbapenems, as a result of amino acid changes that altered the configuration around the active site of the beta-lactamases. SHV-ESBLs are usually encoded by self-transmissible plasmids that frequently carry resistance genes to other drug classes and have become widespread throughout the world in several Enterobacteriaceae, emphasizing their clinical significance.

Summary: Collateral sensitivity (CS) is a phenomenon where resistance to one antibiotic increases sensitivity to other antibiotics, providing treatment opportunities to combat antibiotic resistance. This study investigates the generality of CS effects for different resistance mutations in Streptococcus pneumoniae, a gram-positive pathogen. By measuring the collateral and fitness effects of relevant resistance alleles, the researchers identify common CS effects, but also unique effects for each allele or allelic pair. Mathematical modeling demonstrates that both collateral and fitness effects impact the population dynamics of resistance evolution. The findings highlight the importance of considering allelic identity and interactions in CS-based therapies.

Identification of antibiotic collateral sensitivity and resistance interactions in population surveillance data

Summary: This study developed a methodology to quantify collateral effect directionality and effect size from large-scale antimicrobial resistance population surveillance data, which was evaluated using MIC data for 419 Escherichia coli strains. The proposed approach identified several antibiotic combinations showing symmetrical or non-symmetrical CR and CS, providing guidance for future combination therapy and prescribing.
