Small cell lung cancer (SCLC) is characterized by prevalent circulating tumour cells (CTCs), early metastasis and poor prognosis. We show that SCLC patients (37/38) have rare CTC subpopulations co-expressing vascular endothelial-cadherin (VE-cadherin) and cytokeratins consistent with vasculogenic mimicry (VM), a process whereby tumour cells form 'endothelial-like' vessels. Single-cell genomic analysis reveals characteristic SCLC genomic changes in both VE-cadherin-positive and-negative CTCs. Higher levels of VM are associated with worse overall survival in 41 limited-stage patients' biopsies (P<0.025). VM vessels are also observed in 9/10 CTC patient-derived explants (CDX), where molecular analysis of fractionated VE-cadherin-positive cells uncovered copy-number alterations and mutated TP53, confirming human tumour origin. VE-cadherin is required for VM in NCI-H446 SCLC xenografts, where VM decreases tumour latency and, despite increased cisplatin intra-tumour delivery, decreases cisplatin efficacy. The functional significance of VM in SCLC suggests VM regulation may provide new targets for therapeutic intervention.

Treatment patterns and outcomes among patients with small-cell lung cancer (SCLC) in Europe: a retrospective cohort study

Fiona Blackhall, Nicolas Girard, Alain Livartowski, Laura McDonald, Montserrat Roset, Nuria Lara, Ariadna Juarez Garcia

Summary: This retrospective study analyzed the characteristics, treatment patterns, and clinical outcomes of patients with small-cell lung cancer (SCLC). The results showed that the prognosis of SCLC is still unsatisfactory, especially for patients with extensive-stage disease (ED-SCLC), indicating a high unmet need.

Support and information needs for patients with non-small cell lung cancer receiving concurrent chemo-radiotherapy treatment with curative intent: Findings from a qualitative study

Grant Punnett, Jackie Fenemore, Fiona Blackhall, Janelle Yorke

Summary: The purpose of this study was to identify the support and information needs of patients and their informal caregivers along the CCRT pathway. Semi-structured interviews were conducted to identify physical, psychological, and practical support needs, as well as information needs at different time points. The results showed that the need for disease and treatment information and support remains consistent throughout CCRT and beyond, and additional support for regular activities may also be desired.

Emerging therapies targeting the delta-like ligand 3 (DLL3) in small cell lung cancer

Charles M. M. Rudin, Martin Reck, Melissa L. L. Johnson, Fiona Blackhall, Christine L. L. Hann, James Chih-Hsin Yang, Julie M. M. Bailis, Gwyn Bebb, Amanda Goldrick, John Umejiego, Luis Paz-Ares

Summary: Small cell lung cancer (SCLC) is a highly aggressive neuroendocrine carcinoma with limited treatment options. DLL3, a Notch inhibitory ligand, is overexpressed on SCLC cells, making it an attractive therapeutic target. This article discusses the clinical experience with a DLL3-targeting antibody-drug conjugate called Rova-T, as well as other DLL3-targeting agents currently in development, including T-cell engager molecules and CAR T-cell therapy. The challenges and opportunities for DLL3-targeting therapies, such as using DLL3 as a biomarker and exploring combinatorial approaches, are also discussed.

Summary: Through longitudinal evolutionary analysis of 126 non-small cell lung cancer tumors, it was found that 25% of cases showed early metastatic divergence before the last clonal sweep in the primary tumor, which was enriched in patients who were smokers at the time of initial diagnosis. Polyclonal dissemination was associated with tumor recurrence, while primary lymph node disease played a limited role in metastatic relapse. Our study highlights the importance of selection in metastatic clone evolution within untreated primary tumors.

Summary: This study analyzed 1,644 tumor regions and found various gene mutations and evolutionary processes in lung adenocarcinoma. It was discovered that clonal expansion and whole genome doubling play important roles in the development and relapse of non-small cell lung cancer. The findings also revealed similarities in etiology and pathogenesis between lung adenocarcinoma in smokers and never-smokers.

Summary: Cancer-associated cachexia (CAC) is a significant factor in the morbidity and mortality of non-small cell lung cancer patients. The distribution of skeletal muscle and adipose tissue at the time of diagnosis is associated with survival rates. Individuals who develop CAC exhibit distinct tumor genomic and transcriptomic profiles, including enrichment of inflammatory signaling and epithelial-mesenchymal transitional pathways. The protein GDF15 is identified as a potential therapeutic target for managing CAC.

Summary: The TRACERx study reveals the relationship between tissue morphology, the genomic landscape, and relapse risk of lung adenocarcinomas. Higher-grade tumors have increased chromosomal complexity and tend to evolve towards higher-grade patterns. The presence of micropapillary pattern and 'tumor spread through air spaces' are associated with intrathoracic recurrence.

Summary: This study investigates the experiences of patients and clinicians using the ePROMs service at Christie NHS Foundation Trust in the UK. The results show that the use of ePROMs improves communication and increases patient involvement in their care, as reported by both patients and clinicians. This study demonstrates the acceptability and positive impact of incorporating ePROMs into routine cancer care.

Summary: This study investigates the phenotype and molecular mechanisms of vasculogenic mimicry (VM) in small cell lung cancer (SCLC) using circulating tumor cell-derived explant (CDX) models and genetically engineered mouse models (GEMMs). The results show that VM vessels are present in CDX models, GEMMs, and SCLC patient biopsies, and perfused VM vessels support tumor growth. Only NOTCH-active non-NE cells are VM-competent and exhibit characteristics related to blood vessel development and extracellular matrix organization. On Matrigel, VM-primed non-NE cells remodel the extracellular matrix into hollow tubules through an integrin b1-dependent process.

Summary: Combining AKT inhibitor and SERD improved PFS in a clinical trial for HR+ breast cancer patients. However, CDK4/6 inhibitor treatment may reduce response to subsequent endocrine treatment.
