Summary: This study aimed to quantify the impact of individual complications on mortality, organ failure, hospital stay, and readmission after pancreatoduodenectomy. The results showed that postoperative pancreatic fistula and postpancreatectomy hemorrhage had the greatest independent impact on mortality and organ failure, while delayed gastric emptying had the greatest independent impact on prolonged hospital stay. Interventions focusing on these complications may have the greatest impact on patient outcomes.

Summary: This study investigated the impact of remnant pancreas and other perioperative factors on postoperative pancreatic fistula (POPF) occurring after robot-assisted distal pancreatectomy (RDP) for nonmalignant pancreatic neoplasms. The results showed that a transection plan involving the tail of the pancreas and spleen preservation independently increased the incidence of POPF. The length of the remnant pancreas was also found to be a risk factor for POPF.

Summary: After pancreaticoduodenectomy, the risk of postoperative pancreatic fistula and acute pancreatitis is associated with the acinar content at the pancreatic resection margin, with an increased risk when the acinar content is over 60%.

Summary: This study aimed to provide an evidence-based overview on the best perioperative drug treatment in pancreatic surgery. The analysis showed that somatostatin analogues and glucocorticoids can reduce the incidence of postoperative pancreatic fistula, while there is limited evidence for other drug treatments.

Summary: This study validates the use of prolonged length of stay as a substitute for traditional coding of complications in colorectal cancer surgery to calculate failure-to-rescue rates. The results show that ICD-10 codes poorly identified complications, and prolonged length of stay could be a valid replacement when measuring failure-to-rescue in colorectal surgical patients.

Summary: This study aimed to describe the recurrence pattern, treatments received, and oncological outcomes of pancreatic neuroendocrine tumors (PanNETs) after curative surgery. The study found that recurrence following surgery is infrequent but reduces survival, with most recurrences being distant and managed with multiple therapies. Prospective studies are needed to establish surveillance strategies and treatment sequences to control the disease and prolong survival.

Summary: Pancreatic tumors in children are rare and often difficult to diagnose due to their non-specific clinical presentation and overlapping imaging characteristics. These tumors can be categorized as epithelial or non-epithelial, with epithelial tumors further classified as tumors of the exocrine or endocrine pancreas. The most prevalent solid pancreatic tumor in children is the solid pseudopapillary neoplasm, while pancreatoblastoma is the most common malignant tumor.

Summary: The study found that while the implementation of ERP led to a decrease in superficial surgical site infections, there was an increase in urinary retention. ERP can reduce PLOS but may not significantly impact the total postoperative time spent in the hospital within 90 days (i.e. CLOS).

Summary: Ex vivo organoid cultures have gained attention as a model for studying regenerative medicine and diseases in the pancreas. Pancreatic organoids can be generated for diabetes research and studying pancreatic ductal adenocarcinoma, serving as a translational bridge between in vitro and in vivo models.

Summary: The emergence of the Omicron variant has led to increased infectiousness but decreased virulence compared to ancestral COVID-19 variants. Previous studies have recommended delaying non-emergency surgeries for up to 4-8 weeks after COVID-19 infection. However, postponing surgery has been linked to tumor progression and worse survival outcomes for colorectal cancer patients. In this study, we examined the risk and optimal timing for surgery in colorectal cancer patients with perioperative COVID-19 infection.

Summary: This study investigated the risk factors and outcomes of postoperative hemorrhage (POH) after pancreatic injury. The results showed that intra-abdominal abscess, intestinal fistula, shock on admission, absence of abdominal suction drainage, and higher body mass index were associated with POH. Additionally, POH was associated with a poor prognosis.

Summary: Very few surveys have been conducted on oncosurgical decisions for pancreatic cancer patients and the differences in therapeutic approaches between low/medium and high-volume centers. This study aimed to investigate the management strategies and potential differences in surgical decisions among Spanish centers affiliated to the Spanish Group of Pancreatic Surgery (GECP). Consensus was reached on a limited number of questions regarding pre-, intra-, and post-operative management, with higher agreement observed in high-volume centers for certain intra-operative procedures. A further prospective study is necessary to evaluate the reasons why high-volume centers achieve better outcomes.

Summary: This study compared pancreaticoduodenectomy (PD) and total pancreatectomy (TP) with islet autotransplantation (IAT) in high-risk patients for postoperative pancreatic fistula (POPF). The results showed that although PD had a higher incidence of complications, TP-IAT had a shorter postoperative stay and patients had good metabolic control and sustained C-peptide production.

Summary: A regret-based decision model was applied to evaluate attitudes toward neoadjuvant therapy versus upfront surgery for potentially resectable pancreatic adenocarcinoma. Results showed that regret of omission and commission influenced clinicians' therapeutic decisions, and surgeons or specialists working in surgical centers were less likely to recommend neoadjuvant therapy.

Summary: The risk prediction for tumor recurrence after surgery for non-functional pancreatic neuroendocrine tumors is a major unmet clinical need. Multiple prediction models have been developed, but none are currently incorporated into international guidelines. This systematic review found 13 original models, of which 3 were validated outside the patient group in which they were developed. The lack of external validation hinders the progress toward clinical use.

Summary: This study highlights the significant disparities in access to pancreatic surgery in Italy. The distribution of adequate facilities for pancreatic surgery is uneven across the country, with patients from the south and central regions having higher rates of mobility to the north for surgery. The study also found that mortality rates were higher for non-migrating patients in the south and central regions, indicating the need for improved facilities and equal access to care.

Summary: Objective of this study was to evaluate the reliability, construct validity, and responsiveness of the PROMIS-29 profile questionnaire as a measure of recovery after pancreatic surgery. The questionnaire demonstrated good reliability and construct validity, successfully discriminated between different recovery trajectories, and showed responsiveness up to 90 days after surgery.

Summary: The aim of this study was to explore the correlation between incidence and mortality of pancreatic cancer and healthcare indicators in 36 European countries. The results showed that both incidence and mortality were increasing, and they were strongly positively correlated. Higher health expenditure, availability of beds and medical technology, as well as the number of practicing doctors were related to higher incidence. On the other hand, lower mortality was associated with indicators such as outpatient curative care expenditure, the number of pet scanners, and radiation therapy equipment. The study highlights the impact of healthcare environment on pancreatic cancer incidence and mortality.

Summary: This is a non-inferiority trial comparing open distal pancreatectomy (ODP) with minimally invasive distal pancreatectomy (MIDP) in patients with resectable pancreatic cancer. The results show that MIDP is not inferior to ODP in terms of radical resection rates, and the postoperative outcomes and survival rates are comparable as well.
