Lymph vessels: the forgotten second circulation in health and disease

The lymphatic circulation is still a somewhat forgotten part of the circulatory system. Despite this, novel insights in lymph angiogenesis in health and disease, application of immune markers for lymphatic growth and differentiation and also the introduction of new imaging techniques to visualize the lymphatic circulation have improved our understanding of lymphatic function in both health and disease, especially in the last decade. These achievements yield better understanding of the various manifestations of lymph oedemas and malformations, and also the patterns of lymphovascular spread of cancers. Immune markers that recognize lymphatic endothelium antigens, such as podoplanin, LYVE-1 and Prox-1, can be successfully applied in diagnostic pathology and have revealed (at least partial) lymphatic differentiation in many types of vascular lesions.

Summary: For accurate assessment of cardiac hypertrophy at autopsy, uniform terminology and methodology are required, as well as a systematic approach to measuring heart weight, size, and thickness with age, gender, and body weight and height considerations. Recommendations have been made by the Association for European Cardiovascular Pathology for this purpose. In cases where expertise in cardiovascular pathology is lacking, seeking a second opinion from a specialist pathologist is essential, particularly through systematic histologic analysis. Genetic testing postmortem should be considered in the management of sudden cardiac death.

Summary: The study found that tissue adhesion is common during leadless pacemaker retrieval, mainly consisting of fibrosis and thrombus which may affect the safety of retrieval. Leadless pacemakers may adhere to the heart's valves or subvalvular apparatus, suggesting risk stratification for excessive fibrotic encapsulation and adhesions should be considered in end-of-life strategy optimization.

Higher spatial resolution improves the interpretation of the extent of ventricular trabeculation

Summary: This study investigated the impact of higher spatial resolution on the detection of trabeculation in human hearts. The results showed that measurements using high resolution MRI provide greater accuracy in evaluating cardiac wall morphology, with the number of detected trabeculations and T/C ratio positively correlated with higher spatial resolution. Additionally, the study could not replicate the previous reported correlation between trabeculations of the left and right ventricle using histological validation.

Epicardial and endothelial cell activation concurs with extracellular matrix remodeling in atrial fibrillation

Summary: The study found that in atrial fibrillation (AF) patients, there was a decrease in epicardial cell gene expression, while an upregulation of epithelial-to-mesenchymal transition (EMT) and mesenchymal cell gene expression. Immunohistochemistry analysis showed thickening of the epicardium and an increase in (myo)fibroblast-like cells in the myocardium in AF patients. Additionally, endothelial cell proliferation, angiogenesis, and endothelial signaling were found to be upregulated in AF patients.

The histopathological spectrum of myocardial inflammation in relation to circumstance of death: a retrospective cohort study in clinical and forensic autopsies

Summary: Determining the significance of myocardial inflammation for the cause of death can be challenging. Our study reviewed the histological spectrum of myocardial inflammation between three groups of autopsy cases, defined by their reported cause of death. Only a diffuse increase of inflammatory cells could reliably discriminate between an incidental presence of inflammation or a potentially significant one. Determining the cause of death in case of myocardial inflammation requires a comprehensive approach.
