Enhanced thermic effect of food, postprandial NEFA suppression and raised adiponectin in obese women who eat slowly

ObjectiveMeal duration may influence cardiometabolic health. The aim of this study was to explore postprandial effects of meal duration on human metabolism and appetite. DesignPostprandial comparisons following a standard meal eaten slowly over 40min (D40') and the same meal eaten quickly over 10min (D10') on a different day. Each participant therefore acted as their own control, thereby limiting confounding factors. PatientsObese premenopausal Caucasian women (n=10) with confirmed normoglycaemia were recruited from an obesity clinic at UHCW, Coventry UK. Subjects underwent whole-body calorimetry (8-h) on two separate days. MeasurementsFollowing standard lunch (D40 vs D10), 4-h postprandial analysis included thermic effect of food (TEF) and bloods taken at predefined times (including baseline fasting). Analytes included lipid profile, adiponectin, insulin, glucose, ghrelin, leptin, endotoxin, gut and pancreatic hormones. Appetite was measured using visual-analogue scales and ad libitum food intake at subsequent meal. Paired sample t-tests [including area under the curve (AUC)] were used to compare D40 and D10 trials. ResultsPostprandial TEF (over 240-min) was significantly greater for D40 than D10 [mean (SEM): 809kcal (38) vs 299 kcal (34); 106% vs 39%, respectively, P=0006; AUC 717 kcal.h vs 224kcal.h, respectively, P=002]. Postprandial plasma NEFA was significantly lower, and adiponectin levels were significantly higher for D40 than D10 [AUC (SEM): NEFA 627 mol.h/l (56) vs 769mol.h/l (60), respectively, P=002; adiponectin 334 g.h/ml (39) vs 273g.h/ml (38), respectively, P=004]. Other postprandial analytes and appetite measures were equivalent. ConclusionsIn obese women, eating slowly associates with enhanced TEF, elevated serum adiponectin and suppressed NEFA.

Two-dimensional shear wave elastography (2D-SWE) shows promise as a novel and non-invasive method for assessing liver fibrosis in patients with obesity and NAFLD.

Severe obesity in a specialist type 2 diabetes outpatient clinic: an Australian retrospective cohort study

Summary: The study revealed that many patients with type 2 diabetes mellitus who meet the criteria for bariatric surgery are not receiving adequate medical or surgical management for their obesity. There is a need to improve referrals from T2DM services to weight management/bariatric services and to increase the use of GLP1 agonists and SGLT2 inhibitors when appropriate. These findings support the prioritization of obesity management in the treatment of type 2 diabetes.

Factors associated with long intensive care unit (ICU) admission among inpatients with and without diabetes in South Western Sydney public hospitals using the New South Wales admission patient data collection (2014-2017)

Clinical Use of the Edmonton Obesity Staging System for the Assessment of Weight Management Outcomes in People with Class 3 Obesity

Summary: This study aimed to evaluate the effects of an intensive non-surgical weight management program on weight loss and metabolic outcomes in patients with varying degrees of weight-related complications. The results showed that after 12 months of treatment, there was significant weight loss and improvement in metabolic parameters in both groups, regardless of the severity of complications. This suggests that intensive non-surgical weight management programs can be effective in achieving clinically meaningful weight loss in patients with severe obesity.

Obesity was associated with older age, consumption of fried food, and physical inactivity.
