The effect of vitamin D on lower respiratory tract infections in children

Aim: Lower respiratory tract infections including mainly pneumonia represent an important public health problem leading to high mortality and mobidity rates in children aged below five years in developing countries including our country. Vitamin D deficiency has been associated with increased risk of rickets/osteomalacia, various cancers, autoimmune diseases, hyperproliferative skin diseases, cardiovascular system diseases and infectious diseases. Vitamin D has an important role in cellular and humoral immunity and pulmonary functions. Vitamin D deficiency and lower respiratory tract infection are common health problems in children in our country and no clinical study investigating the relationship between these problems has been conducted so far. In this case-control study, we aimed to assess the association between vitamin D level and lower respiratory tract infection in children. Material and Methods: Sixty-three children aged between six months and five years with lower respiratory infections and 59 age-matched children who had no history of respiratory symptoms in the last month and no accompanying chronic disease were compared in terms of vitamin D levels. The children in the patient group were also evaluated by the clinical picture. Results: No significant correlation was found between vitamin D levels and lower respiratory tract infection in terms of disease and its severity. However, it was found that vitamin D deficiency/insufficiency was observed with a high rate in all children included in the study. Conclusions: Although no correlation was found between vitamin D level and lower respiratory tract infection, it is recommended that vitamin D level should be measured in children with lower respiratory tract infection and vitamin D supplementation should be given to all children especially in winter months based on the fact that the level of vitamin D was lower than normal in approximately half of the children included in the study and considering the effects of vitamin D on infections, pulmonary functions and immunity.

Summary: This study evaluated the impact of a newly established CF registry system in Turkey on the follow-up and care of CF patients. The results showed that the registry system led to higher z-scores of height and BMI, increased usage of medications for lung function improvement, and lower incidence of PBS. This suggests that the registry system has positively influenced the care of CF patients.

Summary: This study aims to evaluate the clinical features of children with tracheostomy and the predictors of psychological status of their primary caregivers, and determine the associations between the children's clinical course with their caregivers' psychological status. The results showed that mothers of children with tracheostomy may experience psychological conditions such as depression, burnout, burden, and low self-esteem.

A homozygous missense variant in the WRN gene segregating in a family with progressive pulmonary failure with recurrent spontaneous pneumothorax and interstitial lung disease

Summary: This report presents a case of a family with five patients with ILD. The patients were diagnosed with ILD through clinical and radiological evaluations. Molecular genetic analysis of the family revealed two homozygous rare variants that were closely associated with the phenotype. Network analysis suggested that the variant in the WRN gene could be the main factor leading to the ILD phenotype, potentially through the altered DNA repair and telomere maintenance pathways.

Aerobic exercise capacity, cough strength, posture, and depression in primary ciliary dyskinesia

Summary: This study compared exercise capacity, cough strength, thoracoabdominal mobility, posture, and depression in children with primary ciliary dyskinesia (PCD) and healthy peers. The results showed that PCD patients had impaired aerobic exercise capacity, cough strength, and thoracoabdominal mobility, deteriorated posture, and elevated perception of depression. Additionally, cough strength was correlated with maximal expiratory strength and total number of steps in these patients.

Summary: The study conducted face-to-face interviews with healthcare workers in 19 pandemic hospitals across 13 provinces in Turkey between September and November 2021. It found that 33.5% of the 1,556 participants perceived a need for mental health services, but only 8.5% actually received these services. Factors such as age, gender, smoking, chronic diseases, mental disorders, recent contact with COVID-19, vaccination history, occupation, and COVID-19 exposure at work were significantly associated with the perceived need for mental health services. Higher scores in mental health assessments and lower scores in quality of life assessments were related to the perceived need for mental health services. The findings highlight the significant need for mental health support among Turkish healthcare workers during the pandemic and identify high-priority groups for intervention.

Summary: This study aimed to investigate the neutrophil, lymphocyte ratio (NLR), mean platelet volume (MPV), and immunoreactive trypsinogen (IRT) as possible inflammatory biomarkers for cystic fibrosis (CF) in infants. The results showed that higher MPV and IRT levels during newborn screening (NBS) period were associated with worse nutritional outcomes, which may reflect chronic inflammation. Therefore, children with higher MPV and IRT should be closely followed up in terms of chronic inflammation and nutritional status.

Evaluation of specificity and sensitivity of IRT/IRT protocol in the cystic fibrosis newborn screening program: 6-year experience of three tertiary centers

Summary: This cross-sectional study aimed to evaluate the cutoff values of immunoreactive trypsinogen (IRT) and investigate the relationship between IRT values and clinical characteristics in children with cystic fibrosis (CF). The study included data from children with positive newborn screening (NBS) from 2015 to 2021. Sensitivity and specificity of IRT cutoff values were assessed, and the positive predictive value (PPV) was determined. The study found that while the sensitivity of CF NBS was high, the PPV was significantly lower than expected. Revising the NBS strategy may help overcome false-positive results.
