Summary: This study compared clinical outcomes in ABO-compatible and incompatible hematopoietic stem cell transplantation (HSCT) recipients, and found that ABO-incompatible patients with high recipient-derived isoagglutinin (RDI) levels had unfavorable outcomes in terms of survival, acute GVHD grade IV, and red blood cell (RBC) transfusion. The high RDI group was independently associated with increased risk of death, RBC transfusion, and poor platelet (PLT) engraftment. Therefore, high RDI levels can serve as a marker for predicting unfavorable outcomes in ABO-incompatible HSCT.

Summary: This study aimed to investigate the impact of ABO incompatibility on transplantation outcomes in antithymocyte globulin (ATG)-based haploidentical stem cell transplantation (haplo-SCT) with peripheral blood stem cell grafts. The results showed no significant association between ABO matching and patient outcomes, including overall survival, disease-free survival, graft-versus-host disease, relapse, and engraftment kinetics. Therefore, ABO incompatibility does not have a major impact on the prognosis of patients undergoing this type of transplantation.

Summary: In this study, selective ABO immunoadsorption was found to be a feasible, safe, and effective method to reduce isohemagglutinins before allogeneic hematopoietic stem cell transplantation in major ABO incompatibility cases. There was no observed difference in transfusion rate or incidence of post-transplant pure red cell aplasia compared to other strategies used to reduce isohemagglutinins.
