Outcomes of an enhanced recovery after surgery programme for pancreaticoduodenectomy

Background: Enhanced recovery after surgery (ERAS) has demonstrated in colorectal surgery a reduction in morbidity and length of stay without compromising security. Experience with ERAS programs in pancreatoduodenectomy (PD) is still limited. The aims of this study were first to evaluate the applicability of an ERAS program for PD patients in our hospital, and second to analyze the postoperative results. Methods: A retrospective study using a prospectively maintained database identified 41 consecutive PD included in an ERAS program. Key elements studied were early removal of tubes and drainages, early oral feeding and early mobilization. Variables studied were mortality, morbidity, perioperative data, length of stay, re-interventions and inpatient readmission. This group of patients was compared with an historic control group of 44 PD patients with a standard postoperative management. Results: A total of 85 pancreatoduodenectomies were analyzed (41 patients in the ERAS group, and 44 patients in the control group. General mortality was 2.4% (2 patients) belonging to the control group. There were no statistical differences in mortality, length of stay in intensive care, reoperationss, and readmissions. ERAS group had a lower morbidity rate than the control group (32 vs. 48%; P=. 072), as well as a lower length of stay (14.2 vs. 18.7 days). All the key ERAS proposed elements were achieved. Conclusions: ERAS programs may be implemented safely in pancreaticoduodenectomy. They may reduce the length of stay, unifying perioperative care and diminishing clinical variability and hospital costs. (C) 2015 AEC. Published by Elsevier Espana, S.L.U. All rights reserved.

Cephalic pancreaticoduodenectomy for ductal adenocarcinoma in the elderly. Can we do it safely?

Summary: This retrospective study focused on patients aged 75 and above who underwent PDC for pancreatic cancer. The study found that elderly patients had higher postoperative mortality and more non-surgical complications. However, there were no significant differences in overall survival and disease-free survival between the elderly group and the younger group. Therefore, age itself should not be considered a contraindication for PDC, as long as patients are carefully selected.

Long-term comparative study on open versus laparoscopic biliopancreatic diversion

Summary: Pollution in the seas and oceans, especially microplastics, has a significant impact on marine organisms. This study evaluated the effects of human activity on Holothuria tubulosa by analyzing biomarkers. The results showed higher microplastics presence in sediments and digestive tracts from highly urbanized areas. The study also revealed an antioxidant response in H. tubulosa in the most impacted areas.

Oxidative stress and behavioral responses of moorish geckos (Tarentola mauritanica) submitted to the presence of an introduced potential predator (Hemorrhois hippocrepis)

Oxidative Stress Response in the Seaweed Padina pavonica Associated with the Invasive Halimeda incrassata and Penicillus capitatus

Summary: This study evaluated the potential competition between the invasive Penicillus capitatus and Halimeda incrassata and the native Padina pavonica by using biomarkers. The presence of the invasive algae significantly decreased the densities of native algae and resulted in an antioxidant response in P. pavonica, indicating a context of stress. However, there was no evidence of oxidative damage. Therefore, the presence of the invasive H. incrassata and P. capitatus can be considered a source of competitive stress for P. pavonica.

MRI is the gold standard for diagnosis, but lacks high specificity. This study found that using 3D reconstruction models from MRI can provide a more visual representation of perianal disease and assist with preoperative planning.

Black spot disease related to a trematode ectoparasite causes oxidative stress in Xyrichtys novacula

Summary: This study aimed to identify the parasite affecting Xyricthys novacula, determine its abundance in different areas, and evaluate the immune and antioxidant responses in the fish. The parasite was identified as Scaphanocephalus sp. and had a higher abundance in the non-protected area compared to the marine protected area. The presence of the parasite resulted in increased immune and antioxidant responses in both the mucus and liver of X. novacula.

Simultaneous Surgical Approach with Hyperthermic Intraperitoneal Chemotherapy (HIPEC) in Patients with Concurrent Peritoneal and Liver Metastases of Colon Cancer Origin

Summary: This study analyzed the postoperative outcomes and survival of patients with advanced metastatic colon cancer who underwent simultaneous liver resection and peritoneal cytoreduction with HIPEC. The results showed that simultaneous peritoneal and liver resection was associated with increased postoperative morbidity and hospital stay, but with similar postoperative mortality and overall and disease free survival. These findings reflect the evolution of these patients and support the incorporation of this surgical strategy in highly selected patients within a multimodal therapeutic plan.

Analysis of potential risk factors in the survival of patients with primary retroperitoneal liposarcoma

Summary: This observational study examines overall survival and disease-free survival in patients with primary liposarcoma. The study found that compartment surgery does not provide benefits in terms of overall survival and disease-free survival. The ASA classification (>= 3) predicts a poor prognosis in both overall survival and disease-free survival. Resection with free margins, defined as R0 in this study, shows better disease-free survival.

Summary: This study aimed to demonstrate that near-infrared fluorescence cholangiography (NIRF-C) with a very low dose of indocyanine green (ICG) provides good visualization of the biliary tract while avoiding hepatic hyperluminescence. The results showed that NIRF-C with 0.25mg ICG during laparoscopic cholecystectomy (LC) allows real-time identification of biliary ducts and avoids hepatic hyperluminescence, even in obese patients or those with gallbladder inflammation.
