Summary: Insulin resistance (IR) is associated with poorer clinical outcomes and inadequate treatment response in patients with bipolar disorder (BD). BD patients with IR exhibit impairments in verbal memory and executive function, as well as smaller hippocampal volumes and prefrontal neurochemical alterations. They are also more likely to have chronic and rapid cycling courses and lower Global Assessment of Functioning scores. BD patients with impaired glucose metabolism also have a higher rate of poor response to mood stabilizers.

The results indicate that QoL is negatively impacted by depressive episodes but remains stable throughout the course of the disorder. Higher QoL scores may be associated with increased manic episodes, while lower scores may predict higher occurrence of depressive episodes. Comorbidities such as Obsessive Compulsive Disorder, Post-Traumatic Stress Disorder, and substance abuse were found to be important predictors of QoL in BD patients.

The negative effect of childhood trauma on resilience is partly mediated by attachment-related anxiety and avoidance.
