Surgical approaches to vestibular schwannomas (VS) are widely known and extensively recorded. For the first time, an exclusive endoscopic approach to the internal acoustic canal (IAC) was described and used to safely remove a cochlear schwannoma involving IAC in March 2012. The aim of this article was to summarize indications and technique to treat intracanalicular VS by transcanal/transpromontorial endoscopic approach. Because management of intracanalicular VSs is complex and strongly debated, this kind of therapeutic option in the appropriate and selected cases could modify classic concepts of the management of this pathology.

Subcochlear canaliculus patterns in the pediatric and adult population: radiological findings and surgical implications

Daniele Marchioni, Luca Gazzini, Nicola Bisi, Marco Barillari, Alessia Rubini

Summary: A retrospective study was conducted on patients who underwent temporal bone CT scans between 2014 and 2019, with pediatric and adult patients divided into different age subgroups. The pneumatization of the subcochlear canaliculus was found to increase constantly from 0 to 19 years and then gradually decrease.

Decompression of the geniculate ganglion and labyrinthine segments of the facial nerve through a middle cranial fossa approach using an ultrasonic surgical system: an anatomic study

Summary: This study evaluated a novel method for bone tissue management in facial nerve decompression using Piezosurgery technology via a middle cranial fossa approach. Results showed safe dissection of the facial nerve without damage to surrounding structures, suggesting this technique is feasible for surgical decompression. More cadaveric training and further case studies are recommended for intraoperative use.

Management of parapharyngeal space tumors with transparotid-transcervical approach: analysis of prognostic factors related with disease-control and functional outcomes

Summary: The transparotid-transcervical approach (TTa) is a safe and effective surgical option for parapharyngeal space (PPS) tumors, achieving satisfactory local disease control. Malignant tumors have a higher rate of positive margins due to their infiltrative nature, potentially requiring more complex treatment approaches. Post-operative cranial nerve function remains satisfactory regardless of tumor characteristics, with higher X cranial nerve deficit rates observed in retrostyloid and neurogenic lesions.

Summary: This study reveals that the morphological appearance of the cochlear nerve is useful for predicting the hearing outcome of cochlear implantation surgery, and pre-surgery serviceable hearing is closely related to satisfactory hearing results.

Multifocal Giant Cell Tumor of the Nasal Fossa: Case Report and Literature Review

Edoardo Serafini, Margherita Basso, Claudio Melchiorri, Gianluca Di Massa, Massimo Lupi, Matteo Alicandri-Ciufelli, Daniele Marchioni

Summary: Giant cell tumors of bone (GCT) are rare soft tissue tumors, and their occurrence in the nasal cavity is even rarer. This article reviewed 15 cases of GCT in the nasal cavity and paranasal sinuses, and reported a case of multifocal GCT in the nasal cavity. GCT should be considered as a possibility when evaluating tumors of the head and neck. Surgical removal is the preferred treatment, with minimally invasive trans-nasal approach being recommended to reduce post-operative complications.

Summary: Endocrine surgery has made significant advances in recent decades, expanding surgical and nonsurgical techniques. The use of newer generation robotics allows for more flexibility and better tactile feedback. Autofluorescence technology shows promise in identifying parathyroid glands and reducing the risk of hypoparathyroidism. Future research will incorporate artificial intelligence to improve preoperative assessment and intraoperative decision-making.
