Gene Therapy for Leber Hereditary Optic Neuropathy Initial Results

Purpose: Leber hereditary optic neuropathy (LHON) is a disorder characterized by severe and rapidly progressive visual loss when caused by a mutation in the mitochondrial gene encoding NADH: ubiquinone oxidoreductase subunit 4 (ND4). We have initiated a gene therapy trial to determine the safety and tolerability of escalated doses of an adeno-associated virus vector (AAV) expressing a normal ND4 complementary DNA in patients with a G to A mutation at nucleotide 11778 of the mitochondrial genome. Design: In this prospective open-label trial (NCT02161380), the study drug (self-complementary AAV [scAAV] 2(Y444,500,730F)-P1ND4v2) was intravitreally injected unilaterally into the eyes of 5 blind participants with G11778A LHON. Four participants with visual loss for more than 12 months were treated. The fifth participant had visual loss for less than 12 months. The first 3 participants were treated at the low dose of vector (5 x 10(9) vg), and the fourth participant was treated at the medium dose (2.46 x 10(10) vg). The fifth participant with visual loss for less than 12 months received the low dose. Treated participants were followed for 90 to 180 days and underwent ocular and systemic safety assessments along with visual structure and function examinations. Participants: Five legally blind patients with G11778A LHON. Main Outcome Measures: Loss of visual acuity. Results: Visual acuity as measured by the Early Treatment Diabetic Retinopathy Study (ETDRS) eye chart remained unchanged from baseline to 3 months in the first 3 participants. For 2 participants with 90-day follow-up, acuity increased from hand movements to 7 letters in 1 and by 15 letters in 1, representing an improvement equivalent to 3 lines. No one lost vision, and no serious adverse events were observed. Minor adverse events included a transient increase of intraocular pressure (IOP), exposure keratitis, subconjunctival hemorrhage, a sore throat, and a transient increase in neutralizing antibodies (NAbs) against AAV2 in 1 participant. All blood samples were negative for vector DNA. Conclusions: No serious safety problems were observed in the first 5 participants enrolled in this phase I trial of virus-based gene transfer in this mitochondrial disorder. Additional study follow-up of these and additional participants planned for the next 4 years is needed to confirm these preliminary observations. (C) 2016 by the American Academy of Ophthalmology.

Leber Hereditary Optic Neuropathy Gene Therapy: Adverse Events and Visual Acuity Results of All Patient Groups

Summary: This study evaluated the safety of gene therapy for G11778A Leber hereditary optic neuropathy (LHON) and found that the only vector-related adverse event was uveitis, which did not result in significant vision sequelae. Improvements in visual acuity of >15 letters were observed in some treated eyes and fellow eyes, as well as in some surrogate study eyes and fellow eyes of natural history subjects. However, all study eyes in group 3 (BCVA > 20/40) lost > 15 letters of visual acuity within the first year despite treatment.

Summary: This study investigated the effectiveness and safety of PHBC in reducing the number and intensity of HLA-B27-related AAU relapses. The results showed that compared to the placebo group, the PHBC group had fewer relapses and a higher proportion of responders, with comparable severity of attacks. Therefore, PHBC treatment may be beneficial.

Natural History of Glaucoma Progression in the DBA/2J Model: Early Contribution of Muller Cell Gliosis

Summary: Glaucoma is a chronic optic neuropathy characterized by progressive degeneration of retinal ganglion cells (RGCs). The main causes of RGC death are traditionally attributed to elevated intraocular pressure (IOP) and resulting mechanical stress. However, glaucoma can occur independent of IOP, suggesting a multifactorial nature of the disease. The present study aimed to comprehensively evaluate the neuro-glial-vascular changes associated with glaucoma progression.

Visual Acuity, Retinal Morphology, and Patients' Perceptions after Voretigene Neparovec-rzyl Therapy for RPE65-Associated Retinal Disease

Summary: This study aimed to investigate the impact of patients' age, baseline visual acuity, and intraoperative foveal detachment on the outcomes of subretinal voretigene neparvovec-rzyl therapy, as well as the patients' perceptions of the treatment effect. The results showed that there were no significant vision changes for adult patients, while there was a trend of improvement for pediatric patients. The decrease in foveal thickness was observed, but its long-term significance remains to be determined.

A Case of Bilateral Diffuse Uveal Melanocytic Proliferation in the Setting of Urothelial Carcinoma of the Ureter: A Failed Response to Plasmapheresis

Summary: Bilateral diffuse uveal melanocytic proliferation (BDUMP) is a rare paraneoplastic syndrome that presents with gradual vision loss. This case report describes a 73-year-old man with metastatic urothelial carcinoma who developed visual symptoms. Ophthalmic examination revealed fluid accumulation, subretinal deposits, and irregular hyperpigmentation, consistent with the diagnosis of BDUMP. Despite undergoing multiple plasmapheresis sessions, the patient did not experience sustained clinical improvement.

A Novel Method to Detect and Monitor Retinal Vasculitis Using Swept-Source OCT Angiography

Summary: This study introduces a novel method for assessing retinal vasculitis using swept-source OCT angiography (SS-OCTA), which detects structural retinal thickening secondary to inflammatory retinal vascular leakage. Further studies are needed to confirm whether SS-OCTA may serve as a semi-quantitative alternative to fluorescein angiography for diagnosing and monitoring treatment response in patients with retinal vasculitis.
