Enoxaparin and Aspirin Compared With Aspirin Alone to Prevent Placenta-Mediated Pregnancy Complications A Randomized Controlled Trial

OBJECTIVE: To evaluate whether daily enoxaparin, added to low-dose aspirin, started before 14 weeks of gestation reduces placenta-mediated complications in pregnant women with previous severe preeclampsia diagnosed before 34 weeks of gestation. METHODS: In this open-label multicenter randomized trial, we enrolled consenting pregnant women with previous severe preeclampsia diagnosed before 34 weeks of gestation, gestational age at randomization of 7-13 weeks, singleton pregnancy, and no plan for anticoagulation. Eligible patients were randomly assigned to a one-to-one ratio to receive daily either 4,000 international units enoxaparin plus 100 mg aspirin or 100 mg aspirin alone. Randomization was done by a web-based randomization system. The primary composite outcome comprised maternal death, perinatal death, preeclampsia, small for gestational age (less than the 10th percentile), and placental abruption. A sample size of 232 women equally divided into two groups was needed to detect a significant reduction in primary outcome from 55% in the aspirin group to 36.7% in the enoxaparin-aspirin group (alpha: 0.05, beta: 0.8, two-sided). RESULTS: Between November 14, 2009, and February 21, 2015, 257 participants were enrolled. Baseline demographic and clinical factors were similar between groups. Eight women were excluded after randomization (six in the enoxaparin-aspirin group and two in the aspirin group), leaving 124 participants assigned to enoxaparin-aspirin and 125 to aspirin. Five participants were lost to follow-up (two in the enoxaparin-aspirin group and three in the aspirin group). There was no significant difference between the groups in the primary outcome: enoxaparin-aspirin 42 of 122 (34.4%) compared with aspirin alone 50 of 122 (41%) (relative risk 0.84, 95% confidence interval 0.61-1.16, P=.29). The occurrence of complications did not differ between the two groups. CONCLUSION: Antepartum prophylactic enoxaparin does not significantly reduce placenta-mediated complications in women receiving low-dose aspirin for previous severe preeclampsia diagnosed before 34 weeks of gestation.

Summary: The prevalence of understated executive dysfunction (UED), assessed using the clock-drawing test (CDT) and the Frontal Assessment Battery (FAB), was investigated in middle-aged adults without cognitive complaints. The study found that more than one third of participants had UED, which was associated with obesity and metabolic syndrome. Screening tests targeting executive functions could be useful for early detection of UED and initiating interventions to improve cognitive performance.

The Presence of the Autophagic Markers LC3B and Sequestosome 1/p62 in the Hydatidiform Mole

Summary: Autophagy is involved in both normal and pathological pregnancies, but its role in hydatidiform moles (HM) is unclear. This study examined the expression of autophagy markers LC3B and p62 in 36 HM samples and compared them to 19 nonmolar pregnancies. LC3B was found in almost half of the villi and p62 was found in almost all villi. LC3B expression was significantly higher in complete HM compared to partial HM. There were also different expression patterns of LC3B in trophoblast layers. Overall, autophagic markers are present in HM and their expression differs between complete and partial moles.

Relevance of fetal brain MRI compared to ultrasound for detecting cerebral anomalies in fetuses with cleft lip and/or palate: a cohort study

Prognostic value of an automated thrombin generation assay in COVID-19 patients entering hospital: A multicentric, prospective observational study

Impact of Enhanced Recovery After Surgery (ERAS) program in gynecologic oncology and patient satisfaction.

Is intimate partner abuse underestimated as a precipitating factor for venous thromboembolism in women?

Summary: This article reports 20 cases of first venous thromboembolism (VTE) events in women of childbearing age after evidence of intimate partner violence. Most patients did not report the violence at their first consultation and doctors did not suspect it. This situation may be underdiagnosed and calls for systematic evaluation. Screening for intimate partner abuse could have significant consequences regarding the protection of affected women and the assessment of bleeding risk with anticoagulant treatment.
