Toxicity of targeted anticancer treatments on the liver in myeloproliferative neoplasms

This article provides a brief review of drug-induced liver injury (DILI) caused by targeted anticancer agents in the management of classical myeloproliferative neoplasms.
The liver has a central role in metabolism, therefore, it is susceptible to harmful effects of ingested medications (drugs, herbs, and nutritional supplements). Drug-induced liver injury (DILI) comprises a range of unexpected reactions that occur after exposure to various classes of medication. Even though most cases consist of mild, temporary elevations in liver enzyme markers, DILI can also manifest as acute liver failure in some patients and can be associated with mortality. Herein, we briefly review available data on DILI induced by targeted anticancer agents in managing classical myeloproliferative neoplasms: Chronic myeloid leukemia, polycythemia vera, essential thrombocythemia, and myelofibrosis.

Summary: This article presents a case report of a 17-year-old female with acute pancreatitis who developed acute onset bilateral painless vision loss due to bilateral infarction of the lateral geniculate bodies (LGB), and a systematic review of relevant literature. The review findings indicate that bilateral LGB infarction is a rare cause of vision loss, predominantly affecting women and often associated with systemic diseases.

Summary: Neurotropic viruses pose a threat to human populations, causing various neurological diseases such as parkinsonism, encephalitis, flaccid myelitis, and Guillain-Barre syndrome. Neuroinvasion can occur through transneural transmission, blood brain barrier dysfunction, or infected immune cell trafficking into the central nervous system (CNS). The ability of the virus to evade direct detection in cerebrospinal fluid (CSF) does not exclude CNS involvement, as neuroevasive mechanisms may still be present.

Budd-Chiari syndrome in myeloproliferative neoplasms: A review of literature

Summary: Myeloproliferative neoplasms (MPNs) are clonal disorders characterized by excessive production of mature myeloid cells. Classical MPNs, such as polycythemia vera, essential thrombocythemia, and primary myelofibrosis, have a tendency to develop thrombotic complications at unusual sites. The pathogenesis of thrombotic events in MPNs is complex and involves endothelial injury, leukocyte adhesion, genetic mutations, and other factors. In this review, we discuss the epidemiology, pathogenesis, histopathology, risk factors, classification, clinical presentation, diagnosis, and management of Budd-Chiari syndrome in Philadelphia-negative MPNs.

COVID-19 in the intensive care unit: Unmasking the critical factors impacting patient survival

Summary: Amid the COVID-19 pandemic, a retrospective analysis of ICU patients revealed prognostic factors associated with mortality. Age 60 or older, impaired consciousness, lung disease, elevated INR and BUN levels were identified as contributing factors to increased mortality. Surprisingly, symptoms such as dizziness/lightheadedness, myalgia, and headache were associated with higher survival rates.

The temporal relationship of paraneoplastic aquaporin-4-IgG seropositive neuromyelitis optica spectrum disorder (NMOSD) and breast cancer: a systematic review and meta-analysis

Summary: A systematic review and meta-analysis revealed that breast cancer is often associated with neuromyelitis optica spectrum disorder (NMOSD), with the diagnosis of breast cancer occurring more often after the onset of NMOSD symptoms. Older women with a new diagnosis of NMOSD should be considered for frequent breast cancer screening.

Craniocervical junction intradural extramedullary meningioma with cord compression

Summary: Craniocervical spine meningiomas are rare with non-specific motor or sensory symptoms. Severe symptoms like respiratory distress and quadriparesis are possible due to the critical location. We described the clinical presentation, neuroimaging findings, diagnostic challenges, and management of a craniocervical junction meningioma in a woman in her 30s. A total of 24 similar cases were found in a literature review.

Breast cancer-associated paraneoplastic neuromyelitis optica with cervical cord compression and spondylosis requiring laminectomy: A case report

Summary: This is a rare case report of a 71-year-old woman with neuromyelitis optica, an autoimmune inflammatory disorder affecting the central nervous system, in a paraneoplastic context. The patient had a history of triple-negative infiltrating ductal breast carcinoma and presented with respiratory failure and underwent a cervical laminectomy. She experienced symptoms such as breast pain, lower extremity weakness, and neck pain.

Summary: Brain calcifications, also known as Fahr's disease, is a rare neurological disorder with diverse clinical symptoms and unclear pathophysiology. This case series highlights the different etiologies and clinical presentations of brain calcifications, including genetic and endocrine causes. Future research should focus on identifying genetic mutations, understanding molecular pathways, and improving diagnostic techniques to enhance diagnosis and management of this intricate neurological disorder.

Polyarteritis Nodosa Following mRNA-1273 COVID-19 Vaccination: Case Study and Review of Immunological Mechanisms

Summary: A 59-year-old female developed diffuse abdominal pain two days after receiving the mRNA-1273 COVID-19 vaccine. A CT angiogram revealed the presence of vascular irregularities consistent with polyarteritis nodosa (PAN). The patient was treated with intravenous methylprednisolone followed by oral methotrexate for immunosuppressive maintenance therapy.
