How the microbiota shapes rheumatic diseases

The human gut harbours a tremendously diverse and abundant microbial community that correlates with, and even modulates, many health-related processes. The mucosal interfaces are particularly active sites of microorganism-host interplay. Growing insight into the characteristic composition and functionality of the mucosal microbiota has revealed that the microbiota is involved in mucosal barrier integrity and immune function. This involvement affects proinflammatory and anti-inflammatory processes not only at the epithelial level, but also at remote sites such as the joints. Here, we review the role of the gut microbiota in shaping local and systemic immune responses and how disturbances in the host-microorganism interplay can potentially affect the development and progression of rheumatic diseases. Increasing our understanding of how to promote host-microorganism homeostasis could therefore reveal novel strategies for the prevention or alleviation of rheumatic disease.

These findings support the use of bimekizumab as a potential treatment option for axSpA.

Summary: This study comprehensively examines the effects of biodegradation on the properties of poly(3-hydroxybutyrate) (PHB) scaffolds supplemented with reduced graphene oxide (rGO). The addition of 1.0 wt% rGO does not affect the enzymatic biodegradation rate of PHB scaffolds for 30 days. Biodegradation leads to depolymerization of the amorphous phase and an increase in crystallinity. The PHB-0.7rGO fibers exhibit the strongest piezoresponse, making them promising for tissue-engineering applications.

Summary: This study evaluated the effectiveness, durability, and tolerability of BIC/FTC/TAF in treating HIV patients in a real-world setting in Belgium. The results showed that the overall virological suppression rate was 93.5% after 48 weeks, indicating excellent effectiveness and tolerability with rare occurrence of drug resistance.

Summary: This study reports breakthrough COVID-19 infections in hematologic malignancies patients who received prophylactic tixagevimab/cilgavimab. Most patients had lymphoproliferative disorders and were infected with the omicron variant. The use of tixagevimab/cilgavimab appears to reduce the severity of COVID-19 in these patients, but further studies are needed to determine the optimal drug administration strategies for immunocompromised patients.

Summary: Through a genome-wide rare variant burden association analysis, it was found that there is an association between at-risk variants in the TLR7 gene and rare loss-of-function variants in TLR3-dependent type I interferon immunity genes. These findings suggest that rare variants in TLR3- and TLR7-dependent type I interferon immunity genes may underlie life-threatening COVID-19 in patients under 60 years old.

Summary: This multicenter retrospective study analyzed the outcome and risk factors for mortality in 326 adult HSCT patients who developed COVID-19. The results showed that age, comorbidities, active hematologic disease, timing from transplant, and severity of the infection were associated with a higher risk of mortality. The overall mortality rate was 21%, with COVID-19 being the main or secondary cause of death in 16% of the patients.

Summary: Patients with previous CD19-directed CAR T-cell therapy have a higher susceptibility to viral infections, including COVID-19. However, this study finds that COVID-19 mortality rates in these patients have improved over time, especially with the Omicron variant having a significantly lower death risk. Prior vaccination and monoclonal antibody treatment also contribute to reducing mortality and mitigating the severity of the disease.

Summary: Nirmatrelvir/ritonavir treatment is associated with lower mortality in hematological malignancy patients. The treatment is related to the presence of extra-pulmonary symptoms and vaccination status for COVID-19, but not chronic pulmonary disease or obesity.

Summary: This study investigates the association between different definitions of axial involvement and syndesmophytes development in patients with Psoriatic Arthritis (PsA). The results show that the probability of developing syndesmophytes is much higher when axial involvement is determined radiographically rather than clinically, particularly in the context of high CRP.

Summary: This sub-analysis of the Candida III study investigated the demographical and clinical characteristics of patients with candidemia who required prolonged hospitalization for completion of intravenous antifungal treatment. The study found that the use of initial echinocandin treatment was associated with prolonged hospital stay, while factors such as neutropenia, intensive care unit admission, catheter related candidemia, total parenteral nutrition, and C. parapsilosis infection were associated with shorter hospitalization.

Summary: This study aimed to evaluate the feasibility and diagnostic accuracy of a deep learning network for detection of structural lesions of sacroiliitis on multicenter pelvic CT scans. The results showed that the optimized deep learning pipeline, including an explainability analysis, can detect structural lesions of sacroiliitis on pelvic CT scans with excellent statistical performance on a slice-by-slice and patient level.

Summary: Regulatory T cells (Tregs) and invariant natural killer T (iNKT) cells play a crucial role in controlling autoimmune diseases. The study reveals that Treg depletion enhances iNKT cell responses and leads to severe liver and gut pathology in preclinical models of autoimmunity.
