The Fragmented QRS Complex in Lead V1: Time for an Update of the Athlete's ECG?

Differentiating between ECG patterns related to athletes' heart remodeling and pathological findings is challenging. This study found a high prevalence of fragmented QRS in lead V1 among young athletes, which is associated with heart adaptations but not exercise-induced ventricular arrhythmias.
Differentiating between ECG patterns related to athletes' heart remodeling and pathological findings is a challenge in sports cardiology. As the significance of fragmented complex in athletes remains uncertain, this study aimed to assess the presence of fragmented QRS in lead V1 (fQRSV1) among young athletes and its association with heart adaptations and arrhythmias. Young athletes referred for annual pre-participation screening receiving a maximal exercise testing and transthoracic echocardiography from January 2015 to March 2021 were included. The study included 684 young athletes. The prevalence of fQRSV1 was 33%. Subjects with fQRSV1 had higher exercise capacity and indexes of right ventricular function and remodeling. Among highly trained athletes, the fQRSV1 group demonstrated also increased left ventricular wall thickness. No significant association existed between fQRSV1 and exercise-induced arrhythmias, even in highly trained athletes. The high prevalence of fQRSV1 in young athletes is associated with training-induced heart adaptations but not exercise-induced ventricular arrhythmias.Graphical AbstractThe fragmented QRS complex in lead V1 in young athletes. Can exercise test intensity and modality affect the prevalence of arrhythmic events in young athletes?

Summary: This study compared the effects of submaximal Harvard Step Test (HST) with incremental Maximal Exercise Test (MET) on inducing and detecting arrhythmias in younger athletes. It found that MET induced more arrhythmias than HST, regardless of the test phase. Higher test intensity and longer exercise duration in MET might contribute to its higher arrhythmogenicity.

The tool is valuable for assessing female athletes and identifying and managing PFD at an early stage. Despite an increase in awareness, barriers to identifying PFD still exist.

Summary: Obesity is associated with a higher risk of cardiac arrhythmias. Sleeve gastrectomy (SG) may increase the risk of exercise-induced arrhythmias in the early post-surgical phase, but it does not seem to increase the occurrence of arrhythmias in the long-term.

The Effect of Exercise Training on Irisin Secretion in Patients with Type 2 Diabetes: A Systematic Review

Summary: This study investigated the effect of different exercise training modalities and intensities on irisin concentration in patients with type 2 diabetes. The results showed that high-intensity exercise training was more effective in increasing irisin secretion. Based on limited evidence, high-intensity training is suggested as a valid exercise training to increase circulating irisin in patients with type 2 diabetes.

Reproducibility and Predictive Value of White-Coat Hypertension in Young to Middle-Age Subjects

Summary: The aim of this study was to investigate the reproducibility of white-coat hypertension (WCH) and its predictive capacity for hypertension needing antihypertensive treatment (HT) in young to middle-age subjects. The results showed that WCH has limited reproducibility, and the risk of future hypertension increased when diagnosed with two sets of blood pressure measurements instead of one.

Summary: Recently, eSports has emerged as a popular form of new media, thanks to the widespread availability of games and online technologies. Despite ongoing discussions about its definition, eSports athletes undergo rigorous training, compete in tournaments, and must follow rules set by governing bodies, just like traditional athletes. However, like any competitive discipline, eSports players are susceptible to injuries related to their sedentary lifestyle, improper posture, prolonged screen time, and repetitive movements. This paper highlights the importance of conducting pre-participation evaluations and implementing effective injury prevention strategies for professional eSports athletes.

Both Moderate and Heavy Alcohol Use Amplify the Adverse Cardiovascular Effects of Smoking in Young Patients with Hypertension

Summary: The aim of this study was to evaluate the association between alcohol and smoking and cardiovascular and renal events, and to investigate the impact of moderate and heavy alcohol consumption on this association. The findings showed that smoking had a detrimental effect on cardiovascular and renal events in alcohol drinkers, but not in abstainers. There was a synergistic effect between smoking and alcohol use, and this was seen in both moderate and heavy alcohol consumers.

Overshoot of the Respiratory Exchange Ratio during Recovery from Maximal Exercise Testing in Young Patients with Congenital Heart Disease

Summary: This study investigates the overshoot of respiratory exchange ratio (RER) after exercise in young patients with congenital heart disease (CHD) and compares it with healthy controls. The results show that CHD patients have a lower magnitude of RER overshoot, indicating reduced cardiorespiratory fitness. Therefore, RER overshoot should be included as part of routine exercise testing in CHD patients to improve prognostic risk stratification accuracy.

Down-staging of obesity one year after bariatric surgery: a new proposal of Edmonton obesity staging system

Summary: The aim of this study was to assess the modifications in EOSS and EOSS-CRF before and after bariatric surgery (BS). The study found that after BS, there was a significant reduction in the number of patients classified as severely obese. Using EOSS, the percentage of patients without functional impairment increased from 61% before surgery to 69% after BS. Using EOSS-CRF, the percentage of patients considered without functional impairment increased from 9.7% before BS to 50% after BS. This study suggests that objective measurements of CRF can provide additional value in assessing the severity of obesity.
