Summary: Chronic pain and sleep disorders are common problems that seriously affect patients' quality of life, but there is still a lack of comprehensive understanding of this comorbidity. This review article summarizes the prevalence of comorbid sleep disorders in chronic pain patients, sleep detection methods, sleep characterization in chronic pain, the impact of sleep disorders on chronic pain, and current therapies. The neurochemical mechanisms underlying the comorbidity of chronic pain with sleep disorders are still limited.

Summary: Radiation therapy is increasingly used in the treatment of solid cancers, but the chronic effects of these treatments are poorly understood. Understanding radiation-specific chronic pain syndromes is crucial for preventing long-term functional impairments, optimizing quality of life, and continuing radiotherapy for patients. It is important to maintain a low threshold of suspicion for diagnosing these conditions early, as symptoms may arise at different times after radiation treatment. Key epidemiological data and evidence for managing each specific condition in vulnerable cancer populations are provided.

Summary: The prevalence and impact of sleep-disordered breathing (SDB) in chronic respiratory infections, such as cystic fibrosis (CF), bronchiectasis, and mycobacterial infections, are reported in this narrative review. The common pathophysiological mechanisms underlying SDB onset in these infections include inflammation, nocturnal cough and pain, mucous plugs, ventilatory impairment, upper airway involvement, and comorbidities. SDB may affect approximately 50% of bronchiectasis patients and significantly impact the quality of life in both children and adults with CF. Routine assessment of SDB should be incorporated into the clinical evaluation of patients with CF from the early stages of the disease to avoid late diagnosis.

Summary: Management of chronic pelvic pain remains a huge challenge, with unimodal therapeutic options often unsuccessful. Individualised multimodal management appears to be the most promising approach, leading to improvement for a large proportion of patients. The European Association of Urology Chronic Pelvic Pain Guideline Group provides an overview of important concepts for successfully diagnosing and treating this challenging disease.

Summary: This article aims to increase awareness of the association between chronic functional abdominal pain and restrictive eating disorders in adolescents. The review describes current diagnostic practices and proposes future research efforts to improve the assessment and treatment of comorbid CFAP and restrictive EDs.

Summary: This study investigated the association between common mental disorders (CMD) and chronic low back pain (CLBP) in women, and found that poor sleep quality acted as an effect modifier in this relationship. Women with CMD and poor sleep quality were more vulnerable to CLBP.

Summary: The relationship between sleep disturbances and headaches in children is bidirectional, potentially sharing common molecular mechanisms. Recognizing and evaluating sleep disturbances in pediatric headache patients is crucial for guiding treatment. First line therapies for sleep disturbances typically involve behavioral approaches, with surgical and pharmacologic strategies reserved for specific circumstances.

Summary: The study examined 130 adolescents with chronic pain, with 98 (mean age 14.49, 66% female) included in the final sample. Results showed that the PBI-Y had good internal reliability and validity, and was positively correlated with functional disability, pain catastrophizing, pain frequency days, pain intensity, and worst pain intensity.

Summary: This study evaluated the association between benign joint hypermobility syndrome (BJHS) and juvenile fibromyalgia syndrome (JFMS) with functional gastrointestinal (GI) disorders. The findings showed that 64% of children with functional GI disorders had JFMS, and 32% had BJHS. JFMS patients commonly experienced retrosternal chest pain, dysphagia, early satiation, nausea, vomiting, and regurgitation. Gastrointestinal symptoms did not differ significantly in BJHS patients. These results suggest that emotional distress plays a role in the complex relationship between functional GI disorders, JFMS, and BJHS, and a multidisciplinary approach is necessary for diagnosis and treatment.

Summary: Despite the lack of knowledge about the pathophysiology of gastrointestinal disorders (GIDs) in infants and children, recent advances in probiotics have opened up the potential for their use as a therapeutic and preventive strategy against these disorders. However, there is controversy and a lack of consensus regarding the use of probiotics, as well as limited comparative studies on their efficacy. This review aims to evaluate current studies on the use of probiotics for the prevention and treatment of common GIDs in the pediatric population, as well as discussing major action pathways and safety recommendations proposed by pediatric health agencies.

Summary: The study found that both IBS and FD, along with most of their major subtypes, are associated with a range of sleep disturbances. These sleep problems are unlikely to be explained by psychological factors and may play an independent role in the pathophysiology.

Summary: This review study found a link between sleep disorders and gastroenterological disorders with depression and chronic orofacial pain. Sleep disorders and gastroenterological disorders negatively affect the psyche, increase inflammation, disrupt tryptophan metabolism, and cause excessive microglial activation, leading to increased susceptibility to pain sensation and depression.

Summary: Individuals with chronic pain often experience co-existing sleep problems and depression-related states. Chronic pain, sleep problems, and depression interrelate, and have been shown to exacerbate one another, which negatively impacts quality of life. The study found that sleep quality may moderate the relationship between pain interference and depression among individuals with chronic pain.

Summary: This review aims to examine the relationship between sleep disturbance and chronic pain, and explore factors such as neurobiochemistry, anatomy, and systemic mediators that contribute to this relationship. Recent studies show an associative, rather than causative relationship between sleep disturbance and chronic pain.

Summary: The study demonstrated that Toxoplasma gondii infection can lead to chronic alterations in sleep-wake states in mice, characterized by increased time spent awake, increased cortical EEG theta power density, and decreased slow-wave sleep. These effects can be alleviated by anti-inflammatory treatment with corticosteroid dexamethasone, shedding light on the neurodegenerative and neuropsychiatric disorders reported in infected patients.
