Summary: The differential diagnosis of acute non-traumatic tetraparesis can be systematized based on the site of defect, including upper motor neuron, peripheral nerve, neuromuscular junction, or muscle. A thorough assessment of present illness history, along with neurological examination and ancillary studies, is crucial for accurate diagnosis and timely treatment of potentially life-threatening conditions presenting with acute tetraparesis.

Summary: The spectrum of immune-mediated neuropathies is broad and the different subtypes are still being researched. Diagnosis and treatment of these disorders are challenging due to the numerous subtypes involved. This literature review focuses on CIDP, GBS, and MMN, analyzing their molecular, electrophysiological, and ultrasound features. Immune dysfunction can cause damage to the peripheral nervous system. Conduction blocks and ultrasound are important factors in diagnosing these disorders, and immunotherapy is a common treatment approach.

Summary: In patients with carpal tunnel syndrome (CTS) whose symptoms are more severe in the nondominant hand, ultrasound can be used to detect structural abnormalities or anatomical variations that may be causative or influence treatment approach. Structural findings were significantly associated with symptoms mainly in the nondominant hand and a body mass index (BMI) <30 kg/m(2) in multivariable analysis. Consideration of neuromuscular ultrasound in addition to electrodiagnostic studies is recommended for this subgroup of CTS patients.

Summary: The study found that within 6 months of GBS hospital admission, the mortality rate was 6.6 times higher compared to the general population of the same age. The mortality rate continued to increase for approximately 4 years following GBS, and then leveled off to a similar long-term mortality rate.

Summary: This study focused on phenotype features and the correlation with molecular diagnosis of congenital myasthenic syndromes (CMS) patients, showing a higher chance of confirmed molecular diagnosis in group A and a lower chance in group C. It was found that some individual clinical features, alterations on biopsy, and electrophysiology can enhance the specificity for CMS diagnosis.

Summary: This study demonstrated the importance of high-resolution ultrasound (US) in the early diagnosis of leprosy neuropathy, as it identified neural thickening in leprosy household contacts (HC). The combination of positive anti-PGL-I serology and absence of a BCG scar can help identify individuals with a greater likelihood of developing leprosy neuropathy.

Summary: This comprehensive case series study of 19 patients with cardiac sarcoidosis emphasizes the importance of using cardiac magnetic resonance imaging (CMR) and cardiac positron emission tomography using 18F-Fluorodeoxyglucose (PET-FDG) as diagnostic criteria, as cardiac sarcoidosis can be the initial manifestation of systemic sarcoidosis, requiring prompt diagnosis and treatment to prevent complications and mortality.

Summary: Distal sensory polyneuropathy (DSP) is characterized by length-dependent, sensory-predominant symptoms and signs, including chronic pain, tingling, and poor balance. It can be challenging to diagnose and manage. Recent advances in understanding the causes and potential therapies for DSP are discussed in this review.
