Clinicopathologic significance of mismatch repair protein expression in endometrioid endometrial cancer

This study evaluated the association between mismatch repair protein expression and clinicopathologic outcomes in patients with endometrioid endometrial cancer. The results showed an association between mismatch repair deficiency and poor prognostic factors, suggesting its potential as a prognostic indicator for endometrioid endometrial cancer.
Objective: To evaluate the association between mismatch repair (MMR) protein expression and clinicopathologic outcomes in patients with endometrioid endometrial cancer (EC). Materials and methods: A retrospective review of the clinico-pathologic outcomes was performed on patients who were diagnosed with EC and had results of MMR protein immunohistochemistry. MMRdeficient (MMR-d) was defined as absence of expression in any of the 4 MMR proteins (MLH1, MSH2, MSH6, and PMS2). Demographics, pathologic variables, and survival outcomes were compared according to the MMR status. Results: A total of 193 EC patients with available MMR expression data were included, of whom 163 patients had endometrioid type EC. Overall, 44 patients (27.0%) were classified as MMR-d. Compared with MMR-proficient (MMR-p) group, MMR-d group was associated with more frequent lymphovascular space invasion (LVSI, p = 0.001). MMR-d was also related with higher risk of lymph node (LN) metastasis in endometrioid type EC (p = 0.008), especially para-aortic LN metastasis. During the median follow-up period of 19.1 months (1-44.5), MMR-d group, especially MLH1/PMS2 subgroup, showed a tendency of reduced PFS (p = 0.036 and p = 0.008, respectively). On Cox regression analysis, however, LN metastasis remained as the only independent risk factor for PFS (p = 0.004) in endometrioid EC, and MLH1/PMS2 loss showed a marginally significant association (p = 0.054). Conclusion: Our findings of the associations between MMR deficiency and poor prognostic factors, such as LVSI and LN metastasis, may suggest the prognostic value of MMR status in EC and need further prospective validation studies. Multiple Primary Malignancies in Patients with Gynecologic Cancer

Summary: This study investigated the prevalence and oncologic outcomes of gynecologic cancer patients with multiple primary malignant tumors (MPMT). The results showed that the prevalence of MPMT in patients was 8.6% and the average diagnostic period between primary and secondary cancer was 60 months. The most common coexisting cancer in the MPMT group was endometrial-ovarian cancer, while breast cancer was the most common non-gynecologic cancer in the coexisting cancer group. The incidence of synchronous cancer was significantly higher in the MPMT group compared to the coexisting cancer group, and early-stage ovarian cancer was more highly diagnosed in the MPMT group.

Summary: The study found that although there were no significant differences in residual disease and CIN recurrence between the GI-negative and GI-positive groups, GI-positive patients were more likely to receive additional surgical treatments. Careful consideration should be given to repeat surgery based on GI positivity to avoid overtreatment and surgical complications.

Role of systematic lymphadenectomy in patients with intermediate to high-risk early stage endometrial cancer

Summary: The objective of this study was to determine the clinical significance of systematic lymph node dissection (LND) and better define its extent in intermediate-to high-risk early stage endometrial cancer (EC). The rates of lymph node metastasis (LNM) were assessed according to risk groups and anatomic sites. LNM served as an independent risk factor for recurrence, and nodal assessment should be performed up to the infra-renal level in high-risk EC.

Effect of Loop Electrosurgical Excision Procedure on Sexual Dysfunction in Korean Women

Summary: This study aimed to investigate the impact of LEEP on sexual dysfunction in Korean women. The results showed that the prevalence of female sexual dysfunction before and after LEEP was 62.5% and 66.7%, respectively. However, the changes in total FSFI and FSDS scores after LEEP were not significant. The subdomains of sexual function and sexual distress were also not significantly altered by LEEP.
