Intracerebral hemorrhage (ICH) is a subtype of stroke with high mortality and morbidity. When a diseased artery within the brain bursts, expansion and absorption of the resulting hematoma trigger a series of reactions that cause primary and secondary brain injury. Microglia are extremely important for removing the hematoma and clearing debris, but they are also a source of ongoing inflammation. This article discusses the role of microglial activation/polarization and related inflammatory mediators, such as Toll-like receptor 4, matrix metalloproteinases, high-mobility group protein box-1, nuclear factor erythroid 2-related factor 2, heme oxygenase, and iron, in secondary injury after ICH and highlights the potential targets for ICH treatment.

Summary: This study investigated the acceptable level of uncertainty for experienced stroke clinicians in deciding whether to administer intravenous thrombolysis (IVT) before endovascular treatment (EVT). The results showed that clinicians would generally feel comfortable skipping IVT if there was reasonable comparability between IVT+EVT and EVT alone, with no more than 3% of patients being unable to achieve functional independence at 90 days due to skipping IVT.

Metabolomics profiling to characterize cerebral ischemia-reperfusion injury in mice

Summary: This study investigated the metabolic alterations during ischemia-reperfusion injury using metabolomic analysis. A total of 82 differentially expressed metabolites were identified, with some significantly elevated in the reperfusion group. Metabolomics pathway analysis revealed the importance of glycerophospholipid metabolism, linoleic acid metabolism, pyrimidine metabolism, and galactose metabolism in the response to cerebral ischemia-reperfusion. This study provides important insights into the metabolic changes induced by cerebral ischemia-reperfusion.

The results showed that pretreatment with SBB could significantly reduce brain tissue autofluorescence without affecting specific fluorescence signals, improving the signal-to-noise ratio of fluorescence imaging.

First-pass effect in patients with acute basilar artery occlusions undergoing stent retriever thrombectomy

Summary: This study aimed to evaluate the impact of FPE on efficacy and safety for patients with acute basilar artery occlusion (BAO). The results showed that achieving FPE was strongly associated with favorable clinical outcomes, lower mortality rate, and shorter time to recanalization. Compared with final successful reperfusion, FPE also independently contributed to favorable outcomes.

Effect of endovascular treatment within 6 hours for acute basilar artery occlusion

Summary: The study aimed to investigate the efficacy and safety of combining endovascular treatment (EVT) with standard medical treatment (SMT) in Chinese patients with acute basilar artery occlusion (BAO) within 6 hours of occlusion. Patients were divided into SMT-alone group or EVT+SMT group. Results showed that EVT was associated with a higher rate of favorable functional outcome and a lower proportion of deaths. Predictors of favorable outcome in the EVT+SMT group included younger age, lower baseline National Institutes of Health Stroke Scale score, higher baseline posterior circulation Alberta Stroke Program Early CT Score, absence of diabetes mellitus, and modified Thrombolysis in Cerebral Infarction scores 2b-3.

Summary: This study revealed the impact of CXCR4 and P-BTK on NLRP3-mediated pyroptosis and lipid peroxidation in SAH, both in vivo and in vitro. Inhibition of the CXCR4-BTK axis can attenuate NLRP3-mediated pyroptosis and lipid peroxidation by regulating NF-kappa B activation in EBI after SAH.
