Acidic tumor microenvironment abrogates the efficacy of mTORC1 inhibitors

Background: Blocking the mechanistic target of rapamycin complex-1 (mTORC1) with chemical inhibitors such as rapamycin has shown limited clinical efficacy in cancer. The tumor microenvironment is characterized by an acidic pH which interferes with cancer therapies. The consequences of acidity on the anti-cancer efficacy of mTORC1 inhibitors have not been characterized and are thus the focus of our study. Methods: Cancer cell lines were treated with rapamycin in acidic or physiological conditions and cell proliferation was investigated. The effect of acidity on mTORC1 activity was determined by Western blot. The anticancer efficacy of rapamycin in combination with sodium bicarbonate to increase the intratumoral pH was tested in two different mouse models and compared to rapamycin treatment alone. Histological analysis was performed on tumor samples to evaluate proliferation, apoptosis and necrosis. Results: Exposing cancer cells to acidic pH in vitro significantly reduced the anti-proliferative effect of rapamycin. At the molecular level, acidity significantly decreased mTORC1 activity, suggesting that cancer cell proliferation is independent of mTORC1 in acidic conditions. In contrast, the activation of mitogen-activated protein kinase (MAPK) or AKT were not affected by acidity, and blocking MAPK or AKT with a chemical inhibitor maintained an anti-proliferative effect at low pH. In tumor mouse models, the use of sodium bicarbonate increased mTORC1 activity in cancer cells and potentiated the anti-cancer efficacy of rapamycin. Combining sodium bicarbonate with rapamycin resulted in increased tumor necrosis, increased cancer cell apoptosis and decreased cancer cell proliferation as compared to single treatment. Conclusions: Taken together, these results emphasize the inefficacy of mTORC1 inhibitors in acidic conditions. They further highlight the potential of combining sodium bicarbonate with mTORC1 inhibitors to improve their anti-tumoral efficacy.

The results showed that preoperative sarcopenia, based on CT assessment alone, had no impact on postoperative clinical outcomes or overall survival. Future research should consider comprehensive assessment of muscle strength and physical performance for preoperative risk stratification.

Summary: The present large scale study aimed to assess the prevalence and consequences of malnutrition in surgical IBD patients in the US. The study found that the true prevalence of malnutrition was greatly underestimated when only using clinical assessment based on BMI and weight loss. Patients with malnutrition had higher rates of complications, longer hospital stays, and higher mortality rates.

Impact of positive microscopic resection margins (R1) after gastrectomy in diffuse-type gastric cancer

Summary: This study aimed to assess the prognostic value of microscopically positive margins (R1) after gastrectomy on survival and tumor recurrence in diffuse-type gastric cancer (DTGC) patients. It showed that patients with R1 had poorer overall survival and disease-free survival, as well as higher early tumor recurrence rates. Therefore, achieving R0 resection should be pursued whenever possible.

Mechanosensitive mTORC1 signaling maintains lymphatic valves

Summary: In this study, the dynamics and molecular mechanisms of lymphatic endothelial cell (LEC) renewal in adult mesenteric lymphatic vessels were investigated. It was found that adult LEC turnover and proliferation were limited to the valve regions of collecting vessels, with valve cells having the shortest lifespan. Proliferating valve sinus LECs were identified as the main source for maintenance and repair of lymphatic valves. Mechanistic target of rapamycin complex 1 (mTORC1) was identified as a mechanoresponsive pathway activated by fluid shear stress in LECs, driving valve cell division or dictating their mechanic resilience through increased protein synthesis. Overactivation of lymphatic mTORC1 in vivo promoted supernumerary valve formation.

Simultaneous pancreas and kidney transplant is often recommended for patients with end-stage kidney disease, but careful selection is necessary. New strategies, including dynamic organ perfusion technologies and stem cell therapies, aim to improve outcomes and reduce immune reactions. This article reviews the indications, surgical techniques, outcomes, and future directions of these transplantations.

In addition, clinical practice should be based on the latest evidence-based medicine and challenge conventional surgical practices. Surgeons should also lead and support multidisciplinary and collaborative teamwork especially with anesthetists and nursing staff.
