Summary: TAVI is the standard treatment for aortic stenosis in older patients in Germany with a well-established supply density. Political regulations and healthcare reforms may impact TAVI capacities in the future. The need for TAVI procedures is projected to increase in the future based on demographic trends, with potential changes in access due to minimum volume regulations.

Summary: In a large series of ViV-TAVR procedures, the incidence of periprocedural PPI was relatively low and decreased with the use of new-generation THV systems. Older age, larger THV size, and previous right bundle branch block were associated with an increased risk of PPI. Although there were no significant differences in 30-day mortality between the PPI and no-PPI groups, PPI patients exhibited a trend toward higher mortality risk at follow-up.

Summary: The study aimed to assess the disease activity in native aortic valves and durability of bioprosthetic valves in patients who underwent TAVI, comparing them with those who had bioprosthetic SAVR. The findings suggest that TAVI patients show ongoing disease activity in their native aortic valves, but the durability of bioprosthetic valves in TAVI is similar to that in bioprosthetic SAVR in the midterm.

Summary: Cerebral embolization is a known complication of transcatheter aortic valve implantation (TAVI), but the effect of the procedure on ocular perfusion is unclear. This study found that TAVI resulted in reduced choroidal perfusion, areas of retinal ischemia, and embolization in a considerable proportion of patients. These complications should be monitored and included in consent procedures for TAVI.

Summary: This tutorial review provides systematic and detailed techniques for the retrograde valve-crossing of a stenotic aortic valve in transcatheter aortic valve implantation. It depicts the main challenges in retrograde valve-crossing, provides step-by-step guidance on the technique, and offers an in-depth description of the three-dimensional anatomy under a two-dimensional fluoroscopy view. Modified techniques for different anatomies are also described.

Summary: Transcatheter aortic valve implantation (TAVI) has become the standard of care for elderly high-risk patients with symptomatic severe aortic stenosis. However, diagnosing bioprosthetic valve dysfunction after TAVI is challenging and there are limited evidence-based criteria to guide therapy. This review focuses on the role of imaging modalities in monitoring the integrity of transcatheter heart valves.

Summary: The study aimed to evaluate the mid-term outcomes of patients with diverse aortic diseases undergoing transapical TAVI. Results showed that transapical TAVI is a safe and effective method, with reasonable median survival time and freedom from death and cardio-cerebral adverse events.

Summary: The study focused on the efficacy of redo-TAVI in reducing para-valvular regurgitation after transcatheter aortic valve implantation. Results showed that redo-TAVI was effective in reducing regurgitation and improving procedural success rates for PVR patients. Further investigation is needed for patient selection in PVR therapies.

Summary: This study aimed to investigate sex-specific differences in aortic valve calcification (AVC) and its correlation with paravalvular leakage (PVL) post-transcatheter aortic valve intervention (TAVI). The results showed that although male patients had a higher burden of AVC, there was no difference in the incidence of moderate PVL compared to female patients.

Summary: The use of valve-in-valve transcatheter aortic valve implantation (ViV-TAVI) is increasing, and this study evaluated clinical outcomes in patients with ViV-TAVI compared to patients with native valve TAVI (NV-TAVI). It was found that ViV-TAVI is a safe and effective treatment for failing bioprosthetic valves with mortality and stroke rates comparable to NV-TAVI for both valve types. The study also compared outcomes in patients with ViV-TAVI treated with self-expandable versus balloon-expandable valves.

Summary: This study highlights the increasing trend of valve-in-valve procedures for patients with aortic stenosis over the last decade. The results show that these interventions are feasible and result in lasting improvements in mean aortic valve gradients. There were no differences in technical and device successes or event-free survival between TAVR-in-SAVR and TAVR-in-TAVR.

Summary: Bicuspid aortic valve disease is the most common congenital heart disease, associated with unique morphological features and lack of consensus in sizing measurement. Multi-slice computed tomography plays a crucial role in TAVI procedure planning, and advancements in devices and procedure planning have improved clinical outcomes.

Summary: This study aimed to compare the clinical and procedural outcomes of patients with rheumatic aortic stenosis (AS) undergoing transcatheter aortic valve implantation (TAVI) with patients undergoing TAVI for degenerative AS. The results showed that the technical success rate and 1-year cardiovascular mortality rate were comparable between patients with rheumatic AS and degenerative AS. However, patients with rheumatic AS had lower rates of 30-day and 1-year cardiovascular mortality compared with patients with degenerative AS.
