Quality Reporting of Multivariable Regression Models in Observational Studies Review of a Representative Sample of Articles Published in Biomedical Journals

Controlling for confounders is a crucial step in analytical observational studies, and multivariable models are widely used as statistical adjustment techniques. However, the validation of the assumptions of the multivariable regression models (MRMs) should be made clear in scientific reporting. The objective of this study is to review the quality of statistical reporting of the most commonly used MRMs (logistic, linear, and Cox regression) that were applied in analytical observational studies published between 2003 and 2014 by journals indexed in MEDLINE. Review of a representative sample of articles indexed in MEDLINE (n = 428) with observational design and use of MRMs (logistic, linear, and Cox regression). We assessed the quality of reporting about: model assumptions and goodness-of-fit, interactions, sensitivity analysis, crude and adjusted effect estimate, and specification of more than 1 adjusted model. The tests of underlying assumptions or goodness-of-fit of the MRMs used were described in 26.2% (95% CI: 22.0-30.3) of the articles and 18.5% (95% CI: 14.8-22.1) reported the interaction analysis. Reporting of all items assessed was higher in articles published in journals with a higher impact factor. A low percentage of articles indexed in MEDLINE that used multivariable techniques provided information demonstrating rigorous application of the model selected as an adjustment method. Given the importance of these methods to the final results and conclusions of observational studies, greater rigor is required in reporting the use of MRMs in the scientific literature.

Summary: This study assessed the trends in cardiovascular risk factor control and drug therapy in patients with type 2 diabetes mellitus from 2007 to 2018. The study found improvements in blood pressure and lipid control, but an insufficient achievement of glycemic targets. The use of antidiabetic and cardiovascular drugs increased, but the proportion of patients achieving all three targets remained low and plateaued after 2013.

Validation of an electronic frailty index with electronic health records: eFRAGICAP index

Summary: The study in Catalunya developed an electronic frailty index eFRAGICAP and assessed its predictive validity over a two-year period, as well as conducted concurrent validity compared to the Clinical Frailty Scale (CFS) and Risk Instrument for Screening in the Community (RISC). The eFRAGICAP showed good discriminative capacity in identifying frail subjects and predicting outcomes.

Measuring cigarette dependence: A comparison of two scales in two different groups of smokers

Summary: In this study, the Fagerstrom Test for Cigarette Dependence (FTCD) and the Cigarette Dependence Scale (CDS) were compared within different groups of smokers. The scores obtained with both scales were highly correlated, indicating good agreement in assessing cigarette dependence. However, there were differences in scores between the clinical and population-based groups.

Feasibility and Acceptability of Personalized Breast Cancer Screening (DECIDO Study): A Single-Arm Proof-of-Concept Trial

Summary: The aim of this study was to assess the acceptability and feasibility of offering risk-based breast cancer screening and its integration into regular clinical practice. The study found that women had a positive attitude towards adjusting screening frequency based on individual risk, with higher-risk women being more willing to participate. However, there was lower acceptance of low-intensity screening for lower-risk women. Participants also had limited knowledge about the benefits and harms of breast screening.

Summary: This study aimed to demonstrate that omalizumab is a corticosteroid-sparing therapy in patients with corticosteroid-dependent asthma, able to inhibit airway remodeling and reduce disease burden. The results showed that omalizumab significantly reduced the use of corticosteroids, improved lung function, reduced exacerbations, and facilitated bronchial epithelial repair.
