Summary: Early initiation of effective antibiotic therapy is essential for critically ill patients with sepsis or septic shock, but current guidelines are lacking. Renal function is a key determinant for achieving the required antibiotic exposure. Model-informed approaches are suggested to individualize treatment regimens and adjust them based on monitoring antibiotic concentrations and other clinical data during treatment.

Summary: In this study, metoprolol treatment in patients with COVID-19-associated ARDS was shown to be safe and effective in reducing lung inflammation and improving oxygenation, resulting in fewer days on invasive mechanical ventilation. Repurposing metoprolol for COVID-19-associated ARDS could be a cost-effective strategy to alleviate the burden of the pandemic.

Summary: This study aimed to determine the prevalence of augmented renal clearance (ARC) in trauma patients receiving nutrition therapy. Results showed that 50% of patients exhibited ARC, with factors such as male gender, traumatic brain injury, higher injury severity score, younger age, lower serum creatinine concentration, and protein intake being associated with ARC. Patients with multiple risk factors for ARC should be closely monitored for renal dosing of electrolytes, nutrients, and medications.

Summary: Alcaligenes faecalis, a rare gram-negative pathogenic bacterium, is primarily found in newborns and immunosuppressed hosts. The optimal treatment for this bacterium remains uncertain. We report a case of an immunocompetent patient who developed meningitis due to A. faecalis after a dental procedure, highlighting the potential emergence of this pathogen in immunocompetent adults.

Summary: Diabetic kidney disease (DKD) is a common complication of diabetes mellitus (DM) and the leading cause of end-stage kidney disease (ESKD) worldwide. Its development involves hemodynamic, metabolic, and inflammatory factors. Persistent albuminuria along with a decline in glomerular filtration rate (GFR) defines this disease, but novel biomarkers originating from its pathogenesis need to be discussed to aid in diagnosis, follow-up, treatment response, and prognosis.

Summary: This case report describes a patient from Africa who presented with acute febrile illness and thrombocytopenia in Colombia and was eventually diagnosed with concurrent infection of Plasmodium falciparum malaria and dengue. Coinfection of dengue and malaria is rarely reported, and should be suspected in patients living or returning from endemic areas for both diseases or during dengue outbreaks. This case serves as a reminder of the importance of early diagnosis and treatment for this significant condition, which can result in high morbidity and mortality if not promptly addressed.

This case report describes a patient who developed liver dysfunction after using injectable contraceptives, and achieved full recovery with drug suspension and supportive measures.
