Critically Ill Patients with Renal Hyperfiltration: Optimizing Antibiotic Dose

Renal hyperfiltration (RHF) is a common condition in critically ill patients, characterized by enhanced renal clearance and increased elimination of renally eliminated medications. Multiple risk factors and potential mechanisms contribute to the occurrence of RHF. RHF is associated with suboptimal exposure to antibiotics, leading to increased risk of treatment failure and unfavorable patient outcomes. This review provides an overview of the evidence related to RHF, including definition, epidemiology, risk factors, pathophysiology, pharmacokinetic variability, and considerations for optimizing antibiotic dosage in critically ill patients.
Renal hyperfiltration (RHF) is a prevalent phenomenon in critically ill patients characterized by augmented renal clearance (ARC) and increased of elimination of renally eliminated medications. Multiple risk factors had been described and potential mechanisms may contribute to the occurrence of this condition. RHF and ARC are associated with the risk of suboptimal exposure to antibiotics increasing the risk of treatment failure and unfavorable patient outcomes. The current review discusses the available evidence related to the RHF phenomenon, including definition, epidemiology, risk factors, pathophysiology, pharmacokinetic variability, and considerations for optimizing the dosage of antibiotics in critically ill patients.

We report a case of an immunocompetent patient who developed meningitis due to A. faecalis after a dental procedure, highlighting the potential emergence of this pathogen in immunocompetent adults.

Persistent albuminuria along with a decline in glomerular filtration rate (GFR) defines this disease, but novel biomarkers originating from its pathogenesis need to be discussed to aid in diagnosis, follow-up, treatment response, and prognosis.

Concurrent Dengue-Malaria Infection: The Importance of Acute Febrile Illness in Endemic Zones

Summary: This case report describes a patient from Africa who presented with acute febrile illness and thrombocytopenia in Colombia and was eventually diagnosed with concurrent infection of Plasmodium falciparum malaria and dengue. Coinfection of dengue and malaria is rarely reported, and should be suspected in patients living or returning from endemic areas for both diseases or during dengue outbreaks. This case serves as a reminder of the importance of early diagnosis and treatment for this significant condition, which can result in high morbidity and mortality if not promptly addressed.
