Summary: Occult micrometastases, which routine pathological analysis may miss, can be detected through mapping of the pulmonary lymphatic system by near-infrared fluorescence imaging. Identifying the sentinel lymph node (SLN) through this technique and analyzing it using immunohistochemistry may improve micrometastasis detection and staging in non-small cell lung cancer (NSCLC) patients undergoing thoracic surgery. This study assessed the feasibility and safety of identifying SLNs using NIR fluorescence imaging in NSCLC patients.

Summary: This study reports three novel IR820-fatty acid conjugates for precise lymphatic metastasis illumination and primary tumor diagnosis. The conjugates are able to bind to albumin in vivo and display significantly enhanced fluorescence quantum yield and imaging resolution after incubation with bovine serum albumin in vitro. Among them, IR-PA shows distinct advantage in terms of fluorescence quantum yield, photostability, and imaging resolution.

The conjugates can bind to albumin in vivo, enhancing fluorescence quantum yield, and show a large Stokes shift for improved imaging resolution. Among them, IR-PA demonstrates better fluorescence quantum yield, photostability, and imaging resolution than other conjugates. These findings provide a self-adaptive albumin-binding near-infrared probe conjugate for accurate diagnosis of primary tumors and lymphatic metastases.

The results showed that the technique was safe, feasible, and able to detect lymph nodes as small as 1 mm. All lymph node metastases would have been detected if SLN biopsy had been performed.
