Relevant domains, core outcome sets and measurements for implant dentistry clinical trials: The Implant Dentistry Core Outcome Set and Measurement (ID-COSM) international consensus report

This study aimed to develop a core outcome set and measurements for implant dentistry clinical trials in order to improve evidence-based implant dentistry and quality of care. The study involved steps such as systematic reviews, patient focus groups, a Delphi project, and expert group discussions. The results identified 22 core outcomes in four domains, including pathophysiology, implant/prosthesis lifespan, life impact, and access to care. This study is of significant importance in improving implant dentistry and quality of care.
Aim: Lack of consistently reported outcomes limits progress in evidence-based implant dentistry and quality of care. The objective of this initiative was to develop a core outcome set (COS) and measurements for implant dentistry clinical trials (ID-COSM). Materials and Methods: This Core Outcome Measures in Effectiveness Trials (COMET)-registered international initiative comprised six steps over 24 months: (i) systematic reviews of outcomes reported in the last 10 years; (ii) international patient focus groups; (iii) a Delphi project with a broad range of stakeholders (care providers, clinical researchers, methodologists, patients and industry representatives); (iv) expert group discussions organizing the outcomes in domains using a theoretical framework and identifying the COSs; (v) identification of valid measurement systems to capture the different domains and (vi) final consensus and formal approval involving experts and patients. The methods were modified from the best practice approach following the Outcome Measures in Rheumatoid Arthritis Clinical Trial and COMET manuals. Results: The systematic reviews and patient focus groups identified 754 (665 + 89, respectively) relevant outcome measures. After elimination of redundancies and duplicates, 111 were formally assessed in the Delphi project. By applying prespecified filters, the Delphi process identified 22 essential outcomes. These were reduced to 13 after aggregating alternative assessments of the same features. The expert committee organized them into four core outcome areas: (i) pathophysiology, (ii) implant/prosthesis lifespan, (iii) life impact and (iv) access to care. In each area, core outcomes were identified to capture both the benefits and harms of therapy. Mandatory outcome domains included assessment of surgical morbidity and complications, peri-implant tissue health status, intervention-related adverse events, complication-free survival and overall patient satisfaction and comfort. Outcomes deemed mandatory in specific circumstances comprised function (mastication, speech, aesthetics and denture retention), quality of life, effort for treatment and maintenance and cost effectiveness. Specialized COSs were identified for bone and soft-tissue augmentation procedures. The validity of measurement instruments ranged from international consensus (peri-implant tissue health status) to early identification of important outcomes (patient-reported outcomes identified by the focus groups). Conclusions: The ID-COSM initiative reached a consensus on a core set of mandatory outcomes for clinical trials in implant dentistry and/or soft tissue/bone augmentation. Adoption in future protocols and reporting on the respective domain areas by currently ongoing trials will contribute to improving evidence-informed implant dentistry and quality of care.

Prevention and management of peri-implant mucositis and peri-implantitis: A systematic review of outcome measures used in clinical studies in the last 10 years

Early bone healing to implants with different surface characteristics. A pre-clinical in vivo study

Summary: This study aimed to investigate early bone healing around implants with non-modified and modified surfaces. Four different surface characteristics were tested in the mandibles of 6 dogs. The results showed that implants with surface modifications had higher bone-to-implant contact and more newly formed bone within the defect area compared to implants with non-modified surfaces. Acid-etching in oxalic and hydrofluoric acid was found to promote early formation of bone-to-implant contact.

Neuroinflammation related to the blood-brain barrier and sphingosine-1-phosphate in a pre-clinical model of periodontal diseases and depression in rats

Summary: This study investigated the potential mechanisms underlying the association between periodontitis and depression. Using a rat model, the study found that neuroinflammation (microglia, blood-brain barrier permeability, and sphingosine-1-phosphate pathways) may play a role in this association. The findings showed that the combined effects of periodontitis and chronic mild stress increased neuroinflammation and disrupted the blood-brain barrier. The study also identified differential regulation of enzymes and receptors in the sphingosine-1-phosphate pathway. These findings contribute to our understanding of the relationship between periodontitis and depression.

Furcation involvement and tooth loss: A registry-based retrospective cohort study

Summary: This registry-based retrospective cohort study found that furcation status has a significant impact on the risk for molar loss. Molars with furcation involvement of degree 2 and 3 have higher risk compared to those without furcation involvement. Additionally, endodontic status and probing depth are also relevant risk factors for molar loss.

Comparative histological evaluation of intra- and extraorally de-epithelialized connective tissue graft samples harvested from the posterior palate region

Summary: This study evaluated the effectiveness of two different methods for de-epithelialization of autologous connective tissue graft samples. The results showed that both methods were effective in removing most of the oral epithelium, but none of them could completely predictably remove all oral epithelium.

Modified-free gingival graft technique for treatment of gingival recession defects at mandibular incisors: A randomized clinical trial

Summary: This study evaluated the clinical and patient-reported outcomes of using conventional free gingival graft (FGG) or modified FGG (ModFGG) for treating RT1 gingival recession defects at mandibular incisors. The results showed that ModFGG had a higher root coverage rate at 1 year compared to FGG (91.8% vs. 60.7%) and significantly improved the height of keratinized tissue (4.2mm vs. 2.2mm) with less shrinkage. ModFGG also had lower post-surgical morbidity at 2 weeks and higher patient satisfaction at 12 months.

Errors and complications in clinical periodontal practice due to methodologic bias and bad interpretation of the evidence

Summary: Different types of errors and complications can occur during and after periodontal or implant-related procedures. Methodological biases and misinterpretation of evidence are often overlooked causes of these issues. Proper evaluation of the literature requires solid clinical knowledge and a systematic approach to recognize and avoid common biases and interpretive errors for the effective application of available information. This review focuses on methodological biases, interpretive errors, and the choice of relevant endpoints to accurately assess the real impact and potential consequences of therapeutic approaches.

Clinical, microbiological and biochemical impact of a supportive care protocol with an air-polishing device, after surgical treatment of peri-implantitis: Randomized clinical trial

Summary: The present study aimed to evaluate the effectiveness of a supportive peri-implant care (SPIC) protocol after surgical therapy of peri-implantitis, including ultrasonic mechanical debridement and glycine powder air polishing. After 12 months of SPIC, there was a significant reduction in probing depths (PD), with the test group showing a larger reduction compared to the control group. The SPIC protocol can maintain or even improve the results obtained after surgical treatment of peri-implantitis.

Efficacy of adjunctive measures in the non-surgical treatment of peri-implantitis: A systematic review

The adjunctive effect of a resorbable membrane to a xenogeneic bone replacement graft in the reconstructive surgical therapy of peri-implantitis: A randomized clinical trial

Summary: This study aimed to evaluate the potential adjunctive effect of a resorbable collagen membrane in the reconstructive surgical therapy of peri-implantitis. Clinical outcomes and radiographic bone levels were assessed at baseline and 12 months after surgery. The study failed to demonstrate any additional clinical or radiographic benefits of using a resorbable membrane in the treatment of peri-implantitis.
