Summary: This case highlights a new phenotype of a known mutation, showing that the same mutation in one family can lead to profoundly different manifestations. It also underscores the fact that genetic causes of hyperinsulinaemic hypoglycaemia may present late in life, emphasizing the importance of identifying such cases for establishing the correct treatment.

Summary: This study developed a game-theory model to explore the interactions between online reviews and free version strategies. The results showed that firms can adopt four different strategies to respond to the availability of online reviews, depending on the relative strength of network effect, cannibalization effect, and dispersed effect. Furthermore, the study found that even when offering a free version is the best strategy, online reviews and the free version strategy can be both complementary and substitutive.

Summary: This commentary explores the possible links between the observed effects and language-related misunderstanding, defined as unintentional errors in comprehension due to language form used by senders. It highlights the importance of addressing such misunderstandings in hazardous work environments and emphasizes the shared responsibility of employees and organizations. The findings underscore the need for redundancy in workplace communication to overcome information barriers and ensure consistent messaging across diverse media channels, especially in culturally and linguistically diverse workplaces.

Summary: This study aimed to clarify the factor structure of the BACE v3 scale, assessing barriers to accessing mental health care among Japanese individuals with mental disorders, and to examine its reliability and validity. Results showed a two-factor structure (treatment stigma and non-stigma) similar to the original, with acceptable internal consistency and reliability. The assessment demonstrated that the BACE v3 has valid and reliable measures for evaluating treatment stigma barriers in mental health care access among individuals in Japan.

Summary: Recent guidelines suggest actively avoiding fever to improve outcomes in comatose patients post-cardiac arrest, but the use of targeted temperature management (TTM) between 32°C and 36°C remains controversial. This study aimed to assess the association between TTM and outcome based on severity at ICU admission using the mCAHP score. The results showed that TTM32-36 was associated with improved neurologic outcome in patients with low and high severity scores. Rating: 9/10

Summary: The study aimed to determine the impact of pre-existing type II diabetes mellitus (T2DM) on 90-day mortality after admission to the intensive care unit (ICU). The results showed that in the unmatched cohort, patients with T2DM had a higher risk of death within 90 days compared to patients without diabetes. However, after propensity score matching, there was no significant difference in the 90-day mortality risk between T2DM patients and non-T2DM patients. These findings were consistent with the analysis performed on imputed datasets.

Summary: The STRONG-HF study found that rapid up-titration of GRMT in a high intensity care strategy can improve outcomes in patients with acute HF. This study aimed to evaluate the role of NT-proBNP in early treatment. The results showed that regardless of baseline NT-proBNP levels, HIC was associated with a reduced risk of 180-day HF readmission or death compared to usual care.

Summary: This study demonstrates that a strategy of rapid uptitration of oral treatment for heart failure and close follow-up, compared with usual care, can significantly improve the prognosis of patients with acute heart failure, regardless of left ventricular ejection fraction. This approach reduces the risk of HF rehospitalization or all-cause death within 180 days after AHF hospitalization, independently of LVEF.

Summary: This study aims to report the prevalence and outcomes of acute encephalopathy in patients with severe COVID-19, as well as identify the determinants of 90-day outcomes. The results showed that more than half of COVID-19 patients with acute encephalopathy had poor outcomes, which were associated with factors such as age, comorbidities, impairment of consciousness, and etiology of acute encephalopathy. This highlights the importance of recognizing and managing encephalopathy in COVID-19 patients.

Summary: Through a genome-wide rare variant burden association analysis, it was found that there is an association between at-risk variants in the TLR7 gene and rare loss-of-function variants in TLR3-dependent type I interferon immunity genes. These findings suggest that rare variants in TLR3- and TLR7-dependent type I interferon immunity genes may underlie life-threatening COVID-19 in patients under 60 years old.

This study aimed to investigate the prevalence, timing, and factors associated with LST decisions in critically ill COVID-19 patients. Data from 163 ICUs in France, Belgium, and Switzerland were analyzed, revealing an association between ICU load, age, frailty, and respiratory severity with LST limitations.

Summary: This study aimed to investigate the occurrence, characteristics, and outcomes of fungal mediastinitis after cardiac surgery. Among 73,688 cardiac surgery procedures, 40 patients developed fungal mediastinitis. Candida spp. was the most common fungal strain. Survivors were younger, had lower body mass index, and lower severity scores.
