Early High-Dose Erythropoietin Therapy After Out-of-Hospital Cardiac Arrest A Multicenter, Randomized Controlled Trial

BACKGROUND Preliminary data suggested a clinical benefit in treating out-of-hospital cardiac arrest (OHCA) patients with a high dose of erythropoietin (Epo) analogs. OBJECTIVES The authors aimed to evaluate the efficacy of epoetin alfa treatment on the outcome of OHCA patients in a phase 3 trial. METHODS The authors performed a multicenter, single-blind, randomized controlled trial. Patients still comatose after a witnessed OHCA of presumed cardiac origin were eligible. In the intervention group, patients received 5 intravenous injections spaced 12 h apart during the first 48 h (40,000 units each, resulting in a maximal dose of 200,000 total units), started as soon as possible after resuscitation. In the control group, patients received standard care without Epo. The main endpoint was the proportion of patients in each group reaching level 1 on the Cerebral Performance Category (CPC) scale (survival with no or minor neurological sequelae) at day 60. Secondary endpoints included all-cause mortality rate, distribution of patients in CPC levels at different time points, and side effects. RESULTS In total, 476 patients were included in the primary analysis. Baseline characteristics were similar in the 2 groups. At day 60, 32.4% of patients (76 of 234) in the intervention group reached a CPC 1 level, as compared with 32.1% of patients (78 of 242) in the control group (odds ratio: 1.01; 95% confidence interval: 0.68 to 1.48). The mortality rate and proportion of patients in each CPC level did not differ at any time points. Serious adverse events were more frequent in Epo-treated patients as compared with controls (22.6% vs. 14.9%; p = 0.03), particularly thrombotic complications (12.4% vs. 5.8%; p = 0.01). CONCLUSIONS In patients resuscitated from an OHCA of presumed cardiac cause, early administration of erythropoietin plus standard therapy did not confer a benefit, and was associated with a higher complication rate. Association between type II diabetes mellitus and 90-day mortality in a large multicenter prospectively collected cohort. A FROG ICU post-hoc study

Summary: The study aimed to determine the impact of pre-existing type II diabetes mellitus (T2DM) on 90-day mortality after admission to the intensive care unit (ICU). The results showed that in the unmatched cohort, patients with T2DM had a higher risk of death within 90 days compared to patients without diabetes. However, after propensity score matching, there was no significant difference in the 90-day mortality risk between T2DM patients and non-T2DM patients. These findings were consistent with the analysis performed on imputed datasets.

Summary: The STRONG-HF study found that rapid up-titration of GRMT in a high intensity care strategy can improve outcomes in patients with acute HF. This study aimed to evaluate the role of NT-proBNP in early treatment. The results showed that regardless of baseline NT-proBNP levels, HIC was associated with a reduced risk of 180-day HF readmission or death compared to usual care.

Summary: This study demonstrates that a strategy of rapid uptitration of oral treatment for heart failure and close follow-up, compared with usual care, can significantly improve the prognosis of patients with acute heart failure, regardless of left ventricular ejection fraction. This approach reduces the risk of HF rehospitalization or all-cause death within 180 days after AHF hospitalization, independently of LVEF.

Summary: This study aims to report the prevalence and outcomes of acute encephalopathy in patients with severe COVID-19, as well as identify the determinants of 90-day outcomes. The results showed that more than half of COVID-19 patients with acute encephalopathy had poor outcomes, which were associated with factors such as age, comorbidities, impairment of consciousness, and etiology of acute encephalopathy. This highlights the importance of recognizing and managing encephalopathy in COVID-19 patients.

Summary: Through a genome-wide rare variant burden association analysis, it was found that there is an association between at-risk variants in the TLR7 gene and rare loss-of-function variants in TLR3-dependent type I interferon immunity genes. These findings suggest that rare variants in TLR3- and TLR7-dependent type I interferon immunity genes may underlie life-threatening COVID-19 in patients under 60 years old.

Data from 163 ICUs in France, Belgium, and Switzerland were analyzed, revealing an association between ICU load, age, frailty, and respiratory severity with LST limitations.

Summary: This study aimed to investigate the occurrence, characteristics, and outcomes of fungal mediastinitis after cardiac surgery. Among 73,688 cardiac surgery procedures, 40 patients developed fungal mediastinitis. Candida spp. was the most common fungal strain. Survivors were younger, had lower body mass index, and lower severity scores.
