Combined resection and RFA in colorectal liver metastases: stratification of long-term outcomes

Background: Combined hepatic resection and radiofrequency ablation (resection-RFA) is a widely accepted multidisciplinary treatment for unresectable colorectal cancer liver metastases. Worse prognosis after resection-RFA is correlated to tumor morphology, although unfavorable morphology is inherent to this patient cohort. This study aimed to select patients who may or may not benefit from resection-RFA with the aid of tumor biology. Method: Data from 485 patients who underwent curative hepatectomy with or without concurrent RFA were retrospectively collected and analyzed. Clinicopathologic characteristics, predictors of overall survival (OS), and OS of patients stratified by tumor biology in resection-RFA were analyzed. Results: Combined resection-RFA was performed in 86 patients (17.7%) and a standalone resection in 399 patients. Baseline patients' characteristics of the resection-RFA group were significantly different in terms of median number of tumors (5 versus 2) and bilobar distribution (84.9% versus 29.1%) from those of the resection-only group. Multivariate analysis identified four independent predictors of decreased OS in the resection-RFA group. Three were related to tumor biology: primary tumor nodal metastases (hazard ratio [ HR], 2.32; 95% confidence interval (95% CI), 1.16-4.64], Kirsten rat sarcoma viral oncogene homologmutation (HR, 2.64; 95% CI, 1.36-5.14), and preoperative high carcinoembryonic antigen (HR, 2.33; 95% CI, 1.13-4.81), and one related to tumor morphology-ablated lesions >= 3 (HR, 2.05; 95% CI, 1.41-3.80; P = 0.023). To examine the prognostic influence of tumor biology, the resection-RFA group was stratified into two groups by number of predictors related to tumor biology (lowrisk: 0-1 risk factors; n = 56 and high risk: 2-3 risk factors; n = 30). Median OS of the low risk, high risk, and resection-alone groups were 61.8, 20.7, and 75.3 mo, respectively. The 5-y OS rate was similar between the lowrisk and resection-alone group (52.7% versus 58.7%, respectively; P = 0.323). Conclusions: Patients with low-risk tumors undergoing a combined resection-RFA approach had roughly comparable OS to those who only underwent resection, irrespective of advanced tumor morphology. Summary: This study validated the applicability of the 7th and 8th editions of the AJCC staging system for IPMN-associated PDAC. The 8th edition introduced T1a sub-staging and a new distinction between N1 and N2, both of which showed mixed results. The findings support the use of the AJCC 8th edition in the IPMN-associated PDAC population, but suggest that certain cutoffs may need to be revisited.

Summary: This study aimed to describe the rate of occult carcinoma deposits in total hepatectomy specimens from patients treated with liver transplant for colorectal liver metastases. The results showed that a complete radiographic response does not reliably predict a complete pathologic response, and residual invasive cancer can be found in disappearing tumors after resection. Liver transplant is considered a promising treatment option for patients with unresectable colorectal liver metastases.

Summary: The study found that gp130 is expressed in the epithelium of most human PDAC, but is rarely expressed in the stroma. The small molecule gp130 inhibitor SC144 potently inhibits PDAC progression in vitro and may block IL-6 or OSM/gp130/STAT3 signaling. These results suggest gp130 as a potential new drug target for pancreatic cancer therapy.

Travel distance and social vulnerability index: Impact on liver-related mortality among patients with end-stage liver disease

Summary: The reasons for geographic disparities in liver-related mortality across the US are explored in this study. It is found that travel distance to liver transplantation programs and social vulnerability have an impact on county differences in liver-related mortality. Counties with higher social vulnerability are associated with higher liver-related mortality, while longer travel distances to transplant centers are associated with higher mortality.

Summary: Introduction: Prescription opioids, including those prescribed after surgery, have greatly contributed to the US opioid epidemic. Educating opioid prescribers is a crucial component of ensuring the safe use of opioids among surgical patients. This study implemented an annual education curriculum for new surgical prescribers, resulting in significant improvements in knowledge and comfort levels. However, there remains a persistent knowledge and comfort gap among these prescribers.

Assessing a Structured Mental Fitness Program for Academic Acute Care Surgeons: A Pilot Study

Humanized Anti-Carcinoembryonic Antigen Antibodies Brightly Target and Label Gastric Cancer in Orthotopic Mouse Models

Summary: This study demonstrates the potential of using humanized anti-carcinoembryonic antigen (CEA) antibodies conjugated with near-infrared dyes to specifically label gastric cancers in mouse models. Orthotopic models showed bright and specific labeling with more than ten times higher tumor-to-background ratios compared to the control. This tumor-specific fluorescent antibody has promising potential as a clinical tool for improving visualization of gastric cancer margins during surgical resection.

Use of Broadband Personality Tests in Bariatric Surgery: A Scoping Review

Summary: This scoping review analyzed the literature on the use of broadband personality tests in the bariatric surgical population to optimize weight loss outcomes. The study found significant associations between personality scales and weight loss, but inconsistent reporting of outcome measures made it challenging to draw concrete conclusions. The American Society for Metabolic and Bariatric Surgery recommends standardization of outcome reporting to improve the reliability of predicting weight loss outcomes.
