The Predictive Role of Maternal Biological Markers and Inflammatory Scores NLR, PLR, MLR, SII, and SIRI for the Risk of Preterm Delivery

Preterm birth is a major cause of infant mortality and morbidity in many countries. This research aimed to evaluate the predictive value of various inflammatory markers for premature delivery. The results showed that NLR, dNLR, PLR, and MLR had significant predictive value, with NLR having the highest sensitivity and AUC value among the tested scores. Further clinical studies are needed to confirm these results and improve therapeutic approaches for reducing premature births.
In many countries, preterm birth, defined as birth before 37 completed weeks of gestation, is the primary cause of infant death and morbidity. An increasing body of research suggests that inflammation (both clinical and subclinical) plays a significant role in inducing preterm labor or developing pregnancy problems that lead to premature birth. Consequently, the purpose of this research was to determine the predictive value of the Neutrophil-Lymphocyte Ratio (NLR), derived Neutrophil-Lymphocyte Ratio (dNLR), Monocytes-to-Lymphocyte Ratio (MLR), Platelets-to-Lymphocyte Ratio (PLR), Systemic immune-inflammation index (SII), and systemic inflammatory response index (SIRI), for premature delivery. A retrospective study analyzed a total of 243 eligible pregnancies that resulted in a preterm birth during 2020 and 2021. A control group without a history of preterm birth was matched by age and trimester of laboratory analysis at a 1:1 ratio. Although the number of comorbidities was similar among study groups, the body-mass index estimated for the week of gestation was significantly higher among the patients from the prematurity group, as well as the prevalence of urinary tract infections and smoking. Laboratory data showed that patients with a preterm birth had significantly higher white blood cell count and monocytes, but significantly lower lymphocytes, platelets, and hemoglobin. The NLR, dNLR, PLR, and MLR scores showed to be significantly higher among patients from the prematurity group, but SII and SIRI were not significantly different between the study groups. It was observed that the AUC values of NLR, dNLR, PLR, and MLR were higher than 0.600, respectively NLR had the highest value among the tested scores (AUC = 0.694) and the highest sensitivity in this study (71%). The highest sensibility was achieved by dNLR, with 70%, and an AUC value of 0.655 (p-value = 0.022). PLR had the second-highest AUC value (0.682) and the best score in terms of sensitivity (70%) and sensibility (69%) (p-value = 0.015). Lastly, MLR had the lowest significant AUC score (0.607) and lowest sensitivity/sensibility. The significant cut-off values for the inflammatory scores were 9.0 for NLR, 9.8 for dNLR, 250 for PLR, and 4.07 for MLR. After evaluating the importance of these inflammatory scores, further clinical applications should be conducted to confirm the results and improve therapy and care to reduce the burden of premature deliveries.

Summary: This study retrospectively analyzed the outcomes of endoscopic papillectomy in FAP-related and sporadic ampullary lesions. The study found that endoscopic papillectomy is safe and effective for FAP-related ampullary lesions, and FAP patients require long-term surveillance.

A Statistical Approach to the Diagnosis and Prediction of HCC Using CK19 and Glypican 3 Biomarkers

Summary: Comparing the effectiveness of CK19 and GPC3 as standalone biomarkers and in a statistical model, our study aimed to predict the probability of developing hepatocellular carcinoma (HCC). We found that AFP, AFP-L3, and DCP were the best biomarkers for predicting the likelihood of HCC, and our score (GALKA score) performed well in HCC prediction and was comparable to the GALAD score.

Impact of COVID-19 on Patients with Decompensated Liver Cirrhosis

Summary: The aim of this study was to assess the impact of COVID-19 infection on patients with decompensated liver cirrhosis (DLC) in terms of acute-on-chronic liver failure (ACLF), chronic liver failure acute decompensation (CLIF-AD), hospitalization, and mortality. In this retrospective study, patients with known DLC and COVID-19 were analyzed to compare the development of ACLF, CLIF-AD, hospitalization length, and independent factors of mortality with a non-COVID-19 DLC group. The study found that COVID-19 significantly influenced disease progression in patients with DLC in terms of associated infections, hospitalization length, and mortality. Rating: 8/10.

Summary: Existing studies suggest that adequate vitamin D levels improve the prognosis and outcomes of SARS-CoV-2 infections, but the effect of vitamin D supplementation on gestational hypertension during pregnancy is still controversial. This research aimed to evaluate whether there are significant differences in vitamin D levels during pregnancy among pregnant women who develop gestational hypertension after SARS-CoV-2 infection. The study found that low vitamin D levels and first-trimester SARS-CoV-2 infection may play a role in the development of gestational hypertension.

Summary: The optimal timing of delivery in cases of early onset fetal growth restriction is challenging to determine due to the opposing risks of stillbirth and prematurity. This study aimed to assess the likelihood of neonatal complications based on the time of birth, using Doppler parameters in fetuses with early onset fetal growth restriction. The findings suggest that infants delivered at the onset of umbilical arteries absent/reversed end-diastolic flow are less likely to develop intraventricular hemorrhage grades III/IV and bronchopulmonary dysplasia.

Impact and assessment of training models in interventional endoscopic ultrasound

Summary: This review aims to assess the impact of different training models on the competence of trainees performing IEUS. Trainees underwent traditional lectures, simulator-based hands-on training, and direct supervision. Skills improved after training, but there is a risk of bias due to lack of external validation. Standardized evaluation methods and clear definition of key attributes are needed to develop skilled endoscopists.

There has also been a decline in anxiety and an improvement in both physical and mental health over the years.
