White Matter Diffusion Abnormalities in Carotid Artery Disease: A Systematic Review and Meta-Analysis

BACKGROUND & PURPOSEAbnormalities in apparent diffusion coefficient (ADC), fractional anisotropy (FA), and mean diffusivity (MD) values can be used to assess microstructural damage to white matter tracts and could represent a quantitative marker of chronic ischemia and thereby potentially serve as a stroke risk factor or a measure of existing subclinical ischemic disease burden. We performed a systematic review and 3 separate meta-analyses to evaluate the association between unilateral carotid steno-occlusion and ipsilateral ADC, FA, or MD abnormality. MATERIALS & METHODSA comprehensive literature search evaluating the association of carotid disease and quantitative white matter diffusion imaging was performed. The included studies examined patients for ADC, FA, and MD values ipsilateral and contralateral to the site of carotid artery disease. Three meta-analyses using standardized mean differences with assessment of study heterogeneity were performed. RESULTSOf the 2,920 manuscripts screened, 6 met eligibility for meta-analysis. Of the included manuscripts, 2 studied ADC values, 6 studied FA values, and 2 studied MD values. Our 3 meta-analyses showed standardized mean difference for ADC, FA, and MD values between cerebral hemispheres ipsilateral and contralateral to carotid artery disease site as 1.13 (95% CI: .79-1.47, P < .001), -.42 (95% CI: -.62 to -.21, P < .001), and .23 (95% CI: -.32 to -.77, P = .41), respectively. Measures of heterogeneity showed mild heterogeneity in the 3 meta-analyses. CONCLUSIONCarotid artery disease is associated with significant ADC and FA value changes, suggesting that carotid disease is associated with quantifiable white matter microstructural damage.

Optimal Carotid Plaque Features on Computed Tomography Angiography Associated With Ischemic Stroke

Summary: Recent evidence suggests that specific plaque features detected on imaging may be more highly associated with ischemic stroke than stenosis. The study found that intraluminal thrombus, maximum soft plaque thickness, and the rim sign are important markers for discriminating ipsilateral carotid sources of stroke, providing better diagnostic information than carotid stenosis measurement alone. These alternative markers may better diagnose vulnerable carotid plaque and guide treatment decisions.

Summary: International guidelines for the management of asymptomatic carotid stenosis (ACS) vary, and a multispecialty position statement aims to reconcile conflicting views. Several high-risk features associated with an increased risk of long-term ipsilateral ischemic stroke in ACS patients have been identified. Some guidelines recommend prophylactic carotid intervention in high-risk patients. The optimal management of ACS patients remains controversial.

Summary: This study aimed to characterize strategies developed by seven regional cooperatives participating in the EvidenceNOW initiative to generate and report EHR-based eCQMs for QI in small-to-medium size practices. The study found that cooperatives ultimately used hybrid strategies to generate and report eCQMs, as neither EHRs alone nor centralized sources alone could effectively operationalize eCQMs for QI.

A survey of computable biomedical knowledge repositories

Association between high-risk extracranial carotid plaque and covert brain infarctions and cerebral microbleeds

Summary: Covert brain infarctions (CBIs) and cerebral microbleeds (CMBs) are subclinical sequelae of ischemic and hemorrhagic cerebral small vessel disease, respectively. This study aimed to determine the association of specific vulnerable carotid plaque features to CBIs and CMBs. The results showed that intraplaque hemorrhage (IPH) and plaque ulceration were associated with CBIs but not CMBs, suggesting that advanced atherosclerosis contributes predominantly to ischemic markers of subclinical vascular injury.

The results showed significant improvements in various cognitive aspects for patients without intraplaque hemorrhage, while patients with intraplaque hemorrhage did not show significant improvements and even experienced a decline in attention index.

Association Between Spotty Calcification in Nonstenosing Extracranial Carotid Artery Plaque and Ipsilateral Ischemic Stroke

Summary: Through studying 117 patients who met the criteria for cryptogenic stroke, it was found that spotty calcifications near the carotid bifurcations were associated with ipsilateral ischemic stroke, suggesting that spotty calcifications may serve as imaging markers for high-risk plaques.

Playing in the clinical decision support sandbox: tools and training for all

Summary: This article presents the CDS-Sandbox, a cloud-based virtual machine designed to assist Clinical Decision Support (CDS) developers and implementers in learning and utilizing FHIR- and CQL-based open-source tools and technologies. Successful demonstrations of the tools for authoring and testing CDS artifacts were conducted using the CDS-Sandbox at the 2020 and 2021 AMIA Annual Symposia. Positive feedback and demonstrated understanding of the workshop materials were received from participants. The CDS-Sandbox and tutorial materials enable learning and implementation of CDS concepts and practices for the clinical informatics community.

Extracranial Carotid Plaque Calcification and Cerebrovascular Ischemia: A Systematic Review and Meta-Analysis
