Summary: A trial of vaginal breech delivery at extremely preterm and very preterm gestations does not appear to increase the risk of adverse neurodevelopmental outcomes at the age of four. However, in moderate to late preterm births, a trial of vaginal breech delivery was associated with an increased risk of visual impairment and autism spectrum disorders compared to children born in cephalic presentation. Special consideration and careful patient selection are needed for a trial of vaginal preterm breech delivery.

Summary: This research aimed to analyze the trends in preterm labor and birth-related hospitalizations in England. The overall admission rates decreased by 26.2%, with spontaneous labor with preterm delivery, spontaneous labor without delivery, and preterm delivery without spontaneous labor being the common causes. Women aged 25-29 years accounted for the majority of hospital admissions.

Summary: The study found that an increased maternal heart rate in the postoperative period is associated with a higher risk of preterm labor, while nulliparous patients and those with lower intraoperative diastolic pressure are more likely to have preterm delivery. Maintaining stable perioperative hemodynamics is important for patients undergoing in utero surgery.

Summary: The study found that manual rotation of fetuses in occiput posterior positions at full dilation did not increase the rate of spontaneous vaginal delivery, nor reduce the rates of operative vaginal delivery or cesarean delivery. Maternal and neonatal morbidity were also similar between the two groups.

Summary: Survivors of preterm birth face various disabilities, and addressing the rates of low birth weight, neonatal and maternal morbidities and mortalities is crucial. Research has provided theories on risk factors and clinical manifestations, but there is a lack of biomarkers for early diagnosis. To overcome current limitations, an integrated approach studying the network interaction between different tissues and organs is needed.

Summary: We examined the impact of lockdown measures in Lombardy on obstetric and perinatal outcomes. Using healthcare databases, we compared births during the two lockdowns with births from corresponding periods in previous years. The proportion of low birth weight and preterm births decreased during the lockdowns, while induction of labor increased. There was no difference in stillbirth rate, but cesarean section was less frequent during the second lockdown.

Summary: Replacing chest X-ray with ultra-low-dose chest CT (ULDCT) in the diagnostic work-up of patients suspected of non-traumatic pulmonary disease at the emergency department has similar effects on short-term functional health, hospital admissions, and length of stay. However, more incidental findings were found in the ULDCT group.

Summary: This article provides a systematic review of the available evidence on the diagnostic accuracy of biparametric (bp)-MRI and multiparametric (mp)-MRI for solid renal masses in differentiating malignant from benign, aggressive from indolent, and clear cell renal cell carcinoma (ccRCC) from other histology. The review reveals deficits in the available evidence, with limited studies and high risk of bias, as well as heterogeneity in definitions, imaging techniques, diagnostic criteria, and outcome measures for solid masses.

Summary: The EU In-Vitro Diagnostic Device Regulation (IVDR) aims to ensure transparent validation of diagnostic devices, traceability of results, and post-market surveillance. Laboratory-developed tests (LDTs) may be used in the absence of suitable commercial devices. The implementation of IVDR may lead to the need for urgent replacement of commercial tests with LDTs, and the workload will depend on various factors.

Summary: This study examined the impact of labor induction at each week of gestation from 37 to 42 weeks on offspring's school performance at age 12. The results showed that induction of labor was associated with decreased school performance scores and a lower proportion of children reaching higher secondary school level. These long-term effects should be taken into account in counseling and decision making.

Summary: This study applied covariate-specific receiver operating characteristic (ROC) curve analysis to individual patient data on D-dimer testing for excluding pulmonary embolism. The results showed significant differences in D-dimer concentration and performance among age groups, while the distribution of results was similar for sex. Summary: Imaging tests are commonly used in healthcare, but concerns about their overuse have overshadowed their benefits in clinical decision-making. This study demonstrates the importance of considering patient-centered outcomes (PCOs) when evaluating the value of imaging tests. The PROD framework categorizes PCOs into four main domains and highlights the interplay between them and the factors that can modify them.
