Summary: This study found that psychological alterations are common in patients with irritable bowel syndrome (IBS) and there is often overlap among various psychological factors. Five psychological factors (physical fatigue, GI-specific anxiety, perceived stress, pain catastrophizing, and trait anxiety) were significantly associated with the severity of GI symptoms. An increasing number of these psychological alterations also led to a gradual increase in the overall severity of GI symptoms.

Summary: This study aimed to assess the postprandial response, including gut peptide secretion and gastric myoelectric activity, in patients with constipation-predominant irritable bowel syndrome (IBS-C). The results showed that IBS-C patients had significantly elevated preprandial levels of gastrin and insulin, while VIP and ghrelin levels were decreased. Moreover, the gastric motility was impaired in IBS-C patients. These findings suggest that disturbances in the postprandial concentration of gut peptides may contribute to abnormal gastric function and intestinal motility, leading to exacerbation of clinical symptoms in IBS patients.

Summary: This narrative review summarizes the epidemiology, pathophysiology, diagnosis, management, and prognosis of irritable bowel syndrome (IBS), which has a high prevalence in the United States, especially among women and young people. The diagnosis is traditionally based on the positive identification of symptoms related to various syndromes, while treatment methods should be customized based on specific symptoms.

Summary: From the perspective of insurers, it is cost-effective to implement routine and algorithmic prescription drug coverage restrictions that require patients to first fail low-cost behavioral, dietary, and off-label treatments. These findings suggest the importance of addressing insurance coverage and drug pricing to provide optimal care for patients with IBS.

Summary: Machine learning analysis of a large prospective cohort of patients with chronic constipation revealed that irritable bowel syndrome with constipation (IBS-C) and functional constipation (FC) only differ significantly in the presence of abdominal pain, suggesting that they may not be distinct syndromes but rather a single syndrome varying along a single clinical dimension. This has implications for patient recruitment into clinical trials, future disease classifications, and management guidelines.

Summary: Clear nutrition metrics are necessary for advancing sustainable diets. Current assessment methods mainly focus on food composition, nutrient requirements, and dietary intake. However, most studies fail to consider the bioavailability of nutrients in mixed human diets, leading to misleading conclusions regarding the environmental impact of animal-source foods compared to plant-source foods.

Summary: This study aimed to investigate pain processing pathways in patients with inflammatory bowel disease (IBD) using somatosensory testing and assess associations with multiple patient factors. The results showed that the absence of a stoma and female sex were associated with greater pressure sensitivity in remote body regions, suggesting widespread hyperalgesia. In addition, worse abdominal pain severity and biologic therapy use were associated with decreased pain inhibition, while biologic therapy use was also associated with increased pain facilitation. These findings suggest altered pain processing and central sensitization mechanisms in patients with IBD.

Summary: This study investigated the relationship between habitual FODMAP intake and post-prandial bowel symptoms in adults with IBS, FD, or FC, as well as in healthy adults. The results showed that IBS participants experienced more post-prandial bowel symptoms compared to participants with other functional bowel disorders or controls. Different symptoms were associated with different types of FODMAP intake.

Summary: The relationship between microbiota profile and stress exposure is still unclear. A rat model of unpredictable chronic mild stress (UCMS) was used to investigate this relationship. Female Sprague Dawley rats showed atypical behaviors following UCMS, but no depressive-like behaviors were observed. The relative abundance of taxa from the Clostridiales order and Desulfovibrionaceae family correlated with anxiety-like behaviors and plasma cytokine concentrations, suggesting the brain-to-gut influence of mild anxiety with a microbiota profile that may involve inflammatory pathways.

Summary: Australasia, including Australia, New Zealand, and Papua New Guinea, has a high prevalence of inflammatory bowel disease (IBD), and the practice of IBD medicine varies within this region. This review discusses barriers to IBD care across Australasia from the perspective of local healthcare professionals, including access to multidisciplinary teams, nutritional therapies, mental health disorders associated with IBD, medication availability, endoscopy access, rural barriers to care, Indigenous IBD care, and pediatric issues. The aim is to identify areas for improvement in IBD care and address research needs in Australasia.

Summary: This study examined the impact of heating and drying on milk protease activities in cow, sheep, and goat milk, compared to raw milk. The results showed that plasmin activity decreased in goat and sheep milk, while cathepsin D activity increased in spray-dried cow milk after heating. In vitro digestion with pancreatin increased both plasmin and cathepsin D activities. Milk proteases were found to contribute to protein hydrolysis in the stomach and small intestine.

Summary: Crohn's disease (CD) is a chronic, incurable and relapsing disease that affects the gastrointestinal tract. Exclusive enteral nutrition (EEN) is the first-line therapy for CD, but little is known about the patient experience of EEN. This study aimed to assess children's experiences of EEN and understand their mindset. The findings show that limited formula flavors and the need for support are important challenges for children on EEN. Psychological support strategies should be developed to ensure successful implementation of EEN in children.
