Daily consumption of two gold kiwifruit is effective in treating constipation in adults and is equally effective as fiber-matched psyllium.
Chronic constipation is highly prevalent worldwide and may be managed with two green or three gold kiwifruit daily. It is unknown whether a smaller standard serve of gold kiwifruit (two daily) is as effective in constipation management. The study aimed to improve chronic constipation with two gold kiwifruit and psyllium in lieu of a placebo daily over four weeks. Adult participants (18-65 years) with functional constipation (FC, n = 11), constipation-predominant irritable bowel syndrome (IBS-C, n = 13), and healthy controls (n = 32) were block-randomized to the treatment order: gold kiwifruit (2/day) or psyllium (fiber-matched, 7.5 g/day) for four weeks, followed by four weeks washout before crossover. Outcomes included alterations of Gastrointestinal Symptom Rating Scale (GSRS) domains and weekly complete spontaneous bowel movements (CSBM) as part of a larger study. Both interventions reduced GSRS constipation domain scores in all subjects compared to baseline values (p = 0.004). All participants reported significantly more weekly CSBM (p = 0.014). Two gold kiwifruit decreased straining (p = 0.021). Two gold kiwifruit daily are as effective as fiber-matched psyllium in treating constipation in adults and should be considered as a treatment option.

The role of holistic nutritional properties of diets in the assessment of food system and dietary sustainability

Lakshmi A. Dave, Suzanne M. Hodgkinson, Nicole C. Roy, Nick W. Smith, Warren C. McNabb

Summary: Clear nutrition metrics are necessary for advancing sustainable diets. Current assessment methods mainly focus on food composition, nutrient requirements, and dietary intake. However, most studies fail to consider the bioavailability of nutrients in mixed human diets, leading to misleading conclusions regarding the environmental impact of animal-source foods compared to plant-source foods.

Somatosensory assessments in patients with inflammatory bowel disease: a cross-sectional study examining pain processing pathways and the role of multiple patient factors

Carrie L. Falling, Simon Stebbings, G. David Baxter, Corey A. Siegel, Richard B. Gearry, Ramakrishnan Mani

Summary: This study aimed to investigate pain processing pathways in patients with inflammatory bowel disease (IBD) using somatosensory testing and assess associations with multiple patient factors. The results showed that the absence of a stoma and female sex were associated with greater pressure sensitivity in remote body regions, suggesting widespread hyperalgesia. In addition, worse abdominal pain severity and biologic therapy use were associated with decreased pain inhibition, while biologic therapy use was also associated with increased pain facilitation. These findings suggest altered pain processing and central sensitization mechanisms in patients with IBD.

Summary: This study identified genetic loci associated with medically refractory ulcerative colitis (UC) through a genome wide association analysis. The findings suggest different genetic risk factors for medically refractory UC compared to non-medically refractory UC. Further research may uncover additional loci related to disease severity.

Summary: This study aimed to develop and validate an IBD-specific nutrition self-screening tool (IBD-NST) that identifies patients at nutrition risk. The final validated IBD-NST includes body mass index (BMI), weight loss and IBD-specific nutrition-focussed questions which were acceptable to patients.

Summary: This study investigated the relationship between habitual FODMAP intake and post-prandial bowel symptoms in adults with IBS, FD, or FC, as well as in healthy adults. The results showed that IBS participants experienced more post-prandial bowel symptoms compared to participants with other functional bowel disorders or controls. Different symptoms were associated with different types of FODMAP intake.

Heat treatment and homogenization of bovine milk loosened gastric curd structure and increased gastric emptying in growing pigs

Summary: This study investigates the impact of heat treatment and homogenization on gastric curd formation and gastric emptying of milk proteins and lipids. The results show that both heat treatment and homogenization influence the in vivo gastric curd structure formed from bovine milk, and the extent of processing affects the gastric emptying rates of dry matter, proteins, and lipids.

Summary: This study aimed to compare the circulating concentrations of amino acids and neurotransmitters between healthy participants and those with functional gastrointestinal disorders (FGIDs). The results showed significant differences in branched-chain amino acids, ornithine, and alpha-aminobutyric acid between healthy controls and FGID groups. No other significant differences were observed for the neurotransmitters or any other amino acids analyzed.

Anxiety-like Behavior in Female Sprague Dawley Rats Associated with Cecal Clostridiales

Summary: The relationship between microbiota profile and stress exposure is still unclear. A rat model of unpredictable chronic mild stress (UCMS) was used to investigate this relationship. Female Sprague Dawley rats showed atypical behaviors following UCMS, but no depressive-like behaviors were observed. The relative abundance of taxa from the Clostridiales order and Desulfovibrionaceae family correlated with anxiety-like behaviors and plasma cytokine concentrations, suggesting the brain-to-gut influence of mild anxiety with a microbiota profile that may involve inflammatory pathways.

Summary: This study investigated how heat treatments affect protein hydrolysis in the milk of ruminants and found that the effects vary depending on the species and heat treatment applied. Self-digestion increased the potential antithrombin peptides in bovine milk. These findings suggest the potential application of enhancing bioactive peptides in the industry.

Summary: Australasia, including Australia, New Zealand, and Papua New Guinea, has a high prevalence of inflammatory bowel disease (IBD), and the practice of IBD medicine varies within this region. This review discusses barriers to IBD care across Australasia from the perspective of local healthcare professionals, including access to multidisciplinary teams, nutritional therapies, mental health disorders associated with IBD, medication availability, endoscopy access, rural barriers to care, Indigenous IBD care, and pediatric issues. The aim is to identify areas for improvement in IBD care and address research needs in Australasia.

Summary: This study examined the impact of heating and drying on milk protease activities in cow, sheep, and goat milk, compared to raw milk. The results showed that plasmin activity decreased in goat and sheep milk, while cathepsin D activity increased in spray-dried cow milk after heating. In vitro digestion with pancreatin increased both plasmin and cathepsin D activities. Milk proteases were found to contribute to protein hydrolysis in the stomach and small intestine.

Exclusive Enteral Nutrition for the Treatment of Pediatric Crohn's Disease: The Patient Perspective

Stephanie Christine Brown, Catherine L. Wall, Richard B. Gearry, Andrew S. Day

Summary: Crohn's disease (CD) is a chronic, incurable and relapsing disease that affects the gastrointestinal tract. Exclusive enteral nutrition (EEN) is the first-line therapy for CD, but little is known about the patient experience of EEN. This study aimed to assess children's experiences of EEN and understand their mindset. The findings show that limited formula flavors and the need for support are important challenges for children on EEN. Psychological support strategies should be developed to ensure successful implementation of EEN in children.
