Induction of Myeloid-Derived Suppressor Cells in Cryopyrin-Associated Periodic Syndromes

Cryopyrin-associated periodic syndromes (CAPS) are caused by mutations in the NLRP3 gene leading to overproduction of IL-1 beta and other NLRP3 inflammasome products. Myeloid derived suppressor cells (MDSCs) represent a novel innate immune cell subset capable of suppressing T-cell responses. As inflammasome products were previously found to induce MDSCs, we hypothesized that NLRP3 inflammasome-dependent factors induce the generation of MDSCs in CAPS. We studied neutrophilic MDSCs, their clinical relevance, and MDSC-inducing factors in a unique cohort of CAPS patients under anti-IL-1 therapy. Despite anti-IL-1 therapy and low clinical disease activity, CAPS patients showed significantly elevated MDSCs compared to healthy controls. MDSCs were functionally competent, as they suppressed polyclonal T-cell proliferation, as well as Th1 and Th17 responses. In addition, MDSCs decreased monocytic IL-1 beta secretion. Multiplex assays revealed a distinct pattern of MDSC-inducing cytokines, chemokines, and growth factors. Experimental analyses demonstrated that IL-1 cytokine family members and auto inflammation-associated alarmins differentially induced human MDSCs. Increased MDSCs might represent a novel autologous anti-inflammatory mechanism in autoinflammatory conditions and may serve as a future therapeutic target. (C) 2016 S. Karger AG, Basel

Summary: This study aims to validate the ICH and ICH-GS scores in patients with cerebellar hemorrhage and compare the outcomes of surgical management with conservative treatment. The results showed that both ICH and ICH-GS scores have good prognostic accuracy for overall mortality and functional outcomes. Surgical management was beneficial for patients with severe cerebellar hemorrhages, indicated by high ICH scores, while conservative treatment was reasonable for patients with lower ICH scores. Therefore, ICH and ICH-GS scores are useful tools for predicting survival and outcome in cerebellar ICH patients.

Angled-tip vs. straight-tip guidewire in ERCP: a randomized, multicenter study

The Conventional Weil Osteotomy Does Not Require Screw Fixation

Summary: The Weil osteotomy is commonly performed for chronic metatarsalgia, and screw fixation is historically used for metatarsal head refixation. However, this study suggests that screw fixation is not always necessary. Retrospective analysis of 155 patients with 278 Weil osteotomies demonstrated a lower revision rate in patients without screw fixation. Therefore, regular screw fixation after a Weil osteotomy may not be justified based on the available data.

The APSANTICO Study: A Prospective Observational Study to Evaluate Antiphospholipid Antibody Profiles in Patients with Thromboembolic Antiphospholipid Syndrome (APS) after COVID-19 Infection and/or Vaccination

Summary: This prospective non-interventional trial included 82 patients with thromboembolic antiphospholipid syndrome (APS) and assessed the effect of COVID-19 infections and/or COVID-19 vaccinations on antiphospholipid antibodies (aPL). The study found no increase in aPL in the total study population, with slight decreases observed for certain antibodies and slight increases observed only in patients with COVID-19 infection and vaccination. The low recurrence rate of thrombotic events in these patients may be attributed to high vaccination rates and effective anticoagulation.

Sex difference in early mortality after abdominal aortic aneurysm repair

Both maximum aneurysm diameter and aortic size index (ASI) are important indicators of the risk of AAA rupture, among which ASI has been shown capable of equalizing sex-related anatomical differences.

Preservation of the Subtalar Joint Determines Outcomes in a 10-Year Evaluation of Ankle Arthrodesis

Summary: Posttraumatic osteoarthritis may require surgical fusion of the ankle joint if non-surgical therapy is unsuccessful. The decision to perform fusion is based on the patient's pain and disability, radiographic imaging, and surgeon experience, without strict guidelines. This study compared outcomes of tibiotalocalcaneal arthrodesis (TTCA) and tibiotalar arthrodesis (TTA) to highlight the importance of the subtalar joint. The results showed significantly worse clinical scores in the TTCA group compared to the TTA group, indicating the need for careful consideration of generous subtalar joint fusion with planned ankle arthrodesis.

Septic History Limits the Outcome of Tibiotalocalcaneal Arthrodesis

Summary: Joint destruction necessitates tibiotalocalcaneal arthrodesis (TTCA) in cases of clinical deficits that cannot be controlled conservatively, possibly leading to sepsis. We aimed to compare the underlying etiology of posttraumatic joint destruction and the outcomes after TTCA in patients with a septic or aseptic history. The significantly worse results of S-TTCA compared to A-TTCA show the long and stressful ordeal that patients with a septic history suffer. Further attention must be paid to infection prophylaxis and, if necessary, early infection revision.

A 23-Year Observational Follow-Up Clinical Evaluation of Direct Posterior Composite Restorations

Summary: The purpose of this study was to observe the quality of posterior composite restorations after more than 23 years of service. A total of 22 patients with 42 restorations underwent two follow-up examinations. Results showed significantly worse scores for six out of seven criteria at the second follow-up evaluation. The study suggests the need for further research with extended follow-up time and regular assessments.
