Summary: The study confirms that lymphovascular invasion and histologic grade are the strongest predictors of neck lymph node metastasis in cN0 patients with pharynx and larynx cancer. Neck dissection is still necessary for cN0 patients to identify the risk of lymph node metastasis even in the modern era.

Summary: This study analyzed the role of Sentinel lymph node biopsy (SLNB) in assessing lymph node status and survival in high-risk squamous cell carcinoma patients. The results showed that although SLNB is suitable for determining lymph node status, it did not provide any benefit regarding further metastasis or tumor-specific survival.

Summary: The potential of sentinel lymph node biopsy (SLNB) in patients with recurrent or secondary oral squamous cell carcinoma (OSCC) without clinical signs of lymph node metastases (cN0) was evaluated in this retrospective study. Results showed a detection rate of 88.2% for sentinel lymph nodes, with two cases showing positive SLN. The SLNB had a negative predictive value of 86.7% and a sensitivity of 50.0% as a neck staging procedure.

Summary: Management of neck in early-stage, clinically node-negative oral squamous cell carcinoma (OSCC) patients remains controversial. This study demonstrated that SLNB using Tc-99m-tilmanocept in a two-day protocol is a reliable and safe staging method for such patients, with no local or nodal recurrences observed in the follow-up period.

Summary: In recent years, there has been a trend towards less aggressive surgical management of axillary lymph nodes in breast cancer. This study evaluated the effectiveness of evaluating axillary sentinel lymph nodes and found that serial sectioning at 2-mm intervals reduced false negatives due to macrometastatic disease. Discordant cases were mostly false negatives detected on the first permanent section level due to sampling errors.

Summary: This study assessed the accuracy of intraoperative pathologic examination in vulva cancer patients and evaluated patient outcomes. The results showed that intraoperative assessment of sentinel lymph nodes can accurately determine the need for lymphadenectomy and does not compromise patient outcomes.

Summary: This study investigated the lymphatic progression patterns in head and neck squamous cell carcinomas (HNSCC) and analyzed the risk of metastases in different lymph node levels (LNL) based on the involvement of upstream LNLs, T-category, HPV status, and other risk factors. The findings provide valuable insights for the personalization of CTV-N definition in the future.

Summary: This retrospective cohort study aimed to determine if positive and close surgical margins are associated with worse prognosis in patients who underwent transoral robotic surgery after neoadjuvant chemotherapy. The results showed that positive margins were not independently predictive of tumor control and survival. A threshold of 1.25 mm was identified as the most appropriate to define close margins, but no difference was measured between close and wide negative margins.

Summary: Numerous studies have shown that arthroscopic partial meniscectomy (APM) is not (cost-) effective in patients with symptoms attributed to a degenerative meniscus tear. The study found that not performing APM to treat degenerative meniscus tears in the Netherlands can significantly reduce healthcare expenditures by €54 million. Therefore, it is recommended to further reduce the use of APM for this condition.

Summary: The objective of this study was to identify subgroups of patients with MRI-confirmed degenerative meniscus tears who may benefit from APM compared to non-surgical or sham treatment. The results showed a small improvement in knee pain for the APM group compared to the non-surgical or sham group at 24 months follow-up. However, there were no significant differences in overall knee function and health-related quality of life between the two groups. No relevant subgroup of patients who benefitted from APM was identified.

Summary: Neutrophil-to-lymphocyte ratio (NLR) has been associated with prognosis in various cancers, including head and neck cancer. However, its prognostic value in oropharyngeal carcinomas (OPSCC), especially in relation to HPV status, is limited. This meta-analysis shows that an elevated pretreatment NLR is associated with worse outcomes in OPSCC, specifically in HPV-positive cases. NLR could serve as an affordable prognostic marker in HPV-positive OPSCC.

Summary: This study investigated changes in sexuality and sexual dysfunction in head and neck cancer patients in the first two years after treatment, specifically looking at the impact of different treatments. Results showed that patients treated with chemoradiation experienced a deterioration in sexuality and sexual dysfunction, with differences observed between men and women. Men reported changes in erectile function, orgasm, satisfaction with intercourse, and overall satisfaction, while women reported changes in desire, arousal, and orgasm. These findings provide important information for improving sexual care for head and neck cancer patients.

Summary: This study aimed to identify factors related to swallowing function up to 5 years after oral cancer treatment. It was found that patients who are older at diagnosis, women, and patients who regularly consume alcohol before their treatment may have poorer swallow functioning after oral cancer treatment. Patients that fit these criteria should have their swallowing evaluated during clinical follow-ups and sent to swallowing therapy when needed. Attention to optimizing tongue function is crucial during therapy to maintain an optimal swallowing function.

Summary: The management of cT3 laryngeal cancers remains controversial, and there is a lack of studies showing which treatment has better survival outcomes. This study compared total laryngectomy, concurrent chemoradiation, and radiation therapy alone in T3 laryngeal cancers and found that TL and CRT had similar overall survival outcomes, which were better than RT alone.

Summary: This retrospective cohort study aimed to assess the effectiveness of different treatment strategies for anastomotic leak after oesophagectomy. The study found that less invasive primary treatment resulted in fewer complications.
