Summary: Although the incidence and mortality rate of gastric cancer seem to be decreasing, it remains a significant global health issue. Early gastric cancer can be effectively treated with endoscopic resection, which has high rates of success and fewer adverse effects compared to surgical resection. However, proper patient selection and surveillance are crucial for ensuring optimal outcomes and detecting potential metachronous lesions. This review summarizes the current evidence on the endoscopic treatment of early gastric cancer, including short- and long-term outcomes and post-resection management.

Summary: The current standard of care for colon cancer surveillance, white light endoscopy, often misses dysplastic lesions that are not visible to the naked eye. Dye-based chromoendoscopy shows promise but lacks accuracy in differentiating tumor tissues from healthy tissues. This study aimed to improve tumor tissue visualization using various phthalocyanine (PC) dye-loaded micelles administered intravenously. Zinc PC (tetra-tert-butyl)-loaded micelles were identified as the optimal formulation, turning tumors dark blue for easy identification. These micelles were also effective in turning colorectal adenomas in mice a dark blue color, aiding in their detection and removal.

Summary: Fluorescently labeled probes show great potential for clinical application in highlighting gastrointestinal lesions, providing fluorescence-guided surveillance and enhancing histopathological analysis by highlighting areas of dysplasia as small as 400 microns, even in tissues with severe inflammation and ulceration.

Summary: Deep learning-based computer-aided diagnosis techniques have shown promising performance in endoscopic lesion identification and detection. However, the interpretability of the model-based results has not been adequately addressed. To solve the bias in feature localization, the researchers proposed a solution that optimizes the feature representations using data augmentation and an auxiliary loss function based on clinical priors. The improvement in the model's precision and visualization results were significant.

Summary: This review provides an overview of the inflammatory microenvironment in gastric precancerous lesions (GPL) and its implications for disease progression. Helicobacter pylori is identified as a key mediator for the inflammatory microenvironment and a driver for the progression of GPL to gastric cancer. Immune cells and inflammatory molecules are found to play important regulatory roles in the progression of GPL. Traditional Chinese Medicine and natural antioxidants show potential in treating GPL.

Summary: This article discusses the challenge that diagnosis of early gastric cancer and its precancerous lesions poses for most Western endoscopists. The subtle changes in mucosal pattern make them difficult to identify. The article reviews the usefulness of conventional and virtual chromoendoscopy and magnification endoscopy in the recognition and classification of these lesions.

Summary: This study developed an AI system for diagnosing common gastric lesions under white-light endoscopy (WLE) and found that the system achieved comparable accuracy to senior endoscopists and higher accuracy than junior endoscopists. With the assistance of the system, the diagnostic accuracy of endoscopists improved, especially for lesions other than polyps. Additionally, the diagnostic time for endoscopists was slightly reduced when using the system.

Summary: The article reviews the diagnostic and prognostic values of molecular markers in gastric cancer, focusing on the correlation between molecular alterations and prognosis in preinvasive neoplasia and early gastric cancer, as well as the (immuno)histochemical characterization of diffuse-type gastric cancer. The aim is to evaluate suitable biomarkers used in the pre-surgical phase for distinguishing patients with different prognoses and determining the best therapeutic strategy.

Summary: Microbiota from patients with intestinal metaplasia or GC can selectively colonise in the stomach of GF mice, inducing premalignant changes such as loss of parietal cells, increased inflammation, and elevated expression of tumor markers. GF mice are useful for investigating the causality of associations observed in human gastric microbiome studies.
