Summary: Rates of anastomotic leak following intestinal resections in inflammatory bowel disease are significantly influenced by clinical characteristics such as low serum albumin, preoperative abscess, reoperative abdominal surgery, and steroid use. While the literature may be conflicting, common themes suggest factors like biologic therapy, immunomodulator use, and method of anastomosis do not increase anastomotic-related complications. Attention to identifying, optimizing, and reducing known risks is crucial in achieving a low rate of anastomotic leakage in colorectal surgery.

Summary: The study demonstrates that higher maintenance doses of adalimumab are more effective in prolonging time to treatment failure in CD patients who failed anti-TNF therapy, compared with standard doses. Adverse events did not differ significantly between the two dose regimens.

Summary: This study aims to investigate the outcomes of CD patients who switch to different types of biologics compared to switching to another anti-TNF after initial anti-TNF therapy failure. The results suggest that CD patients achieve more favorable outcomes when switching to biologics outside the anti-TNF class.

Summary: This study compared the closure rates of perianal fistulas in Crohn's disease patients treated with anti-tumor necrosis factor (TNF) agents or autologous adipose tissue-derived stem cell (auto-ASC) transplantation. The results showed that the closure rate was significantly faster in the auto-ASC transplantation group.

Summary: The results of this meta-analysis demonstrate that biologics are more effective than non-biological treatments in preventing postoperative recurrence in patients with Crohn's disease, with anti-TNFα agents showing superiority in preventing endoscopic recurrence. Additionally, infliximab and adalimumab have similar efficacy in preventing postoperative recurrence.

Summary: Primary hepatic lymphoma is a rare variant of non-Hodgkin's lymphoma, with an incidence of 0.016% among all non-Hodgkin lymphomas. The most common histologic subtype is large diffuse B-cell lymphoma. The pathogenesis is not clearly established, but immunosuppressive therapy has been suggested as a risk factor. We present an intriguing case study of a 23-year-old male patient with Crohn's Disease, who had been receiving a combination therapy of thiopurine and anti-TNF for 6 years and was diagnosed with primary hepatic diffuse large B-cell lymphoma.

Summary: This study analyzed the incidence, prevalence, disease phenotype, treatment strategy, disease course, and colectomy rates of ulcerative colitis (UC) patients in Hungary from 2007 to 2018. The results showed a high incidence of UC in Hungary, similar to high-incidence areas in Western Europe. Treatment strategies were consistent with the biological era, and the probability of disease progression and colectomy rates were lower compared to data from Western European centers.

Summary: This study aimed to develop a core outcome set for real-world studies in adult patients with inflammatory bowel disease (IBD). Candidate outcomes and outcome measures were identified through a systematic review and a modified Delphi consensus process. The international panel agreed on the final core outcome set, which will facilitate the synthesis and generation of real-world evidence.

Summary: Sleep deprivation in the intensive care unit (ICU) is common and linked to delirium and higher mortality. Polysomnography (PSG) is the gold standard for sleep evaluation, but practical limitations lead to the use of subjective sleep assessment (SSA) in many ICUs. However, the agreement between SSA and PSG remains unclear, and SSA may overestimate total sleep time (TST). This study found that the agreement between SSA and PSG was low in mechanically ventilated ICU patients, and there was a significant overestimation of TST when using SSA.

Summary: This study investigated the long-term outcomes of autoimmune pancreatitis (AIP) and inflammatory bowel disease (IBD) in patients with coexisting AIP-IBD, as well as the predictors of complicated AIP course. The majority of patients had type 2 AIP and colonic IBD, and the AIP course was relatively benign with favorable prognosis, but a quarter of the patients developed pancreatic complications. Age, family history, and CD were identified as potential predictors of uncomplicated AIP course.

Summary: Long-term T cell dysregulation following COVID-19 disease is associated with disease severity and long-covid symptoms. Alterations in cellular origin and protein content of extracellular vesicles (EV) during COVID-19 convalescence are linked to initial disease severity. EV derived from convalescent donors can affect the function and metabolic rewiring of CD4 and CD8 T cells, with immune-suppressive properties observed in mild cases.

Summary: This study aims to assess whether letermovir, a novel anti-CMV drug, can inhibit CMV subclinical replication in HIV patients receiving ART, and subsequently reduce CMV-associated gut damage and inflammation. The study will conduct a 14-week open-label, randomized, controlled clinical trial with 60 CMV-seropositive ART-treated HIV patients. The effects of letermovir on gut damage, microbial translocation, inflammation, and HIV reservoir size will be evaluated.
