This paper is the second in a series of two publications relating to the European Crohn's and Colitis Organisation [ECCO] evidence-based consensus on the diagnosis and management of Crohn's disease [CD] and concerns the surgical management of CD as well as special situations including management of perianal CD and extraintestinal manifestations. Diagnostic approaches and medical management of CD of this ECCO Consensus are covered in the first paper [Gomollon et al. JCC 2016].

Summary: This study aimed to compare the efficacy of anti-tumor necrosis factor agents and ustekinumab in patients with Crohn's disease. The results showed that anti-tumor necrosis factor agents as first-line biological therapy were associated with higher response rates at 3 months compared to ustekinumab.

Summary: This study conducted a prospective study on Crohn's disease patients in Hungary and found a high incidence rate, reflecting the era of biologic therapy in treatment strategies. The progression of disease behavior was low and long-term surgery rates had decreased.

Summary: This study analyzed the incidence, prevalence, disease phenotype, treatment strategy, disease course, and colectomy rates of ulcerative colitis (UC) patients in Hungary from 2007 to 2018. The results showed a high incidence of UC in Hungary, similar to high-incidence areas in Western Europe. Treatment strategies were consistent with the biological era, and the probability of disease progression and colectomy rates were lower compared to data from Western European centers.

Summary: This study aimed to develop a core outcome set for real-world studies in adult patients with inflammatory bowel disease (IBD). Candidate outcomes and outcome measures were identified through a systematic review and a modified Delphi consensus process. The international panel agreed on the final core outcome set, which will facilitate the synthesis and generation of real-world evidence.

Subjective sleep assessment compared to polysomnography in mechanically ventilated critically ill ICU patients

Summary: Sleep deprivation in the intensive care unit (ICU) is common and linked to delirium and higher mortality. Polysomnography (PSG) is the gold standard for sleep evaluation, but practical limitations lead to the use of subjective sleep assessment (SSA) in many ICUs. However, the agreement between SSA and PSG remains unclear, and SSA may overestimate total sleep time (TST). This study found that the agreement between SSA and PSG was low in mechanically ventilated ICU patients, and there was a significant overestimation of TST when using SSA.

Summary: This study investigated the long-term outcomes of autoimmune pancreatitis (AIP) and inflammatory bowel disease (IBD) in patients with coexisting AIP-IBD, as well as the predictors of complicated AIP course. The majority of patients had type 2 AIP and colonic IBD, and the AIP course was relatively benign with favorable prognosis, but a quarter of the patients developed pancreatic complications. Age, family history, and CD were identified as potential predictors of uncomplicated AIP course.

Summary: This study used the DELPHI technique to investigate the features of challenging clots in mechanical thrombectomy for acute stroke. After three rounds of DELPHI survey, it was found that white-colored clots, calcified clots, stiff clots, sticky/adherent clots, hard clots, difficult to pass clots, and clots that are resistant to pulling are considered challenging clots.

Summary: This study evaluated the impact of transition on inflammatory bowel disease (IBD) outcomes and found that transition care programs improve patient outcomes after the transfer from pediatric to adult IBD units. Active IBD at the time of transfer impairs outcomes.

Extracellular vesicles in COVID-19 convalescence can regulate T cell metabolism and function

Summary: Long-term T cell dysregulation following COVID-19 disease is associated with disease severity and long-covid symptoms. Alterations in cellular origin and protein content of extracellular vesicles (EV) during COVID-19 convalescence are linked to initial disease severity. EV derived from convalescent donors can affect the function and metabolic rewiring of CD4 and CD8 T cells, with immune-suppressive properties observed in mild cases.

Summary: This study aims to assess whether letermovir, a novel anti-CMV drug, can inhibit CMV subclinical replication in HIV patients receiving ART, and subsequently reduce CMV-associated gut damage and inflammation. The study will conduct a 14-week open-label, randomized, controlled clinical trial with 60 CMV-seropositive ART-treated HIV patients. The effects of letermovir on gut damage, microbial translocation, inflammation, and HIV reservoir size will be evaluated.
