Summary: This study aimed to revisit the current age criterion of 50 years for surgical candidacy in patients diagnosed with asymptomatic primary hyperparathyroidism (PHPT). A Markov model was constructed based on past publications to compare two potential treatment algorithms, parathyroidectomy (PTX) and observation. The results suggest that PTX is advantageous for asymptomatic PHPT patients older than 50 years, indicating a need for revisiting the current guidelines for surgical treatment in young asymptomatic PHPT patients.

Summary: This study compared the clinical value of endoscopic parathyroidectomy with autotransplantation through anterior chest approach (EACtPTx+AT) and open total parathyroidectomy with autotransplantation (OtPTx+AT) in treating secondary hyperparathyroidism (SHPT). The results showed that EACtPTx+AT had better surgical outcomes, fewer postoperative complications, and significant improvements in symptoms and levels of PTH and serum calcium.

The results showed that dialysis start time to first operation time < 3 years, postoperative PTH > 106.5 pg/mL, and postoperative phosphorus > 5.9 mg/dL were significant risk factors for recurrent SHPT.

Summary: This study evaluated the relationship between serum calcium/phosphate ratio (Ca/P) and the clinical presentation of primary hyperparathyroidism (PHPT). The results showed that Ca/P ratio is associated with the clinical and biochemical severity of the disease, but is not an additional tool to identify asymptomatic PHPT patients reaching surgical indication.

The results showed that ultrasound, computed tomography, and single-photon emission computed tomography accurately localized the pathological parathyroid gland. Intraoperative parathyroid hormone monitoring and nerve monitoring shortened operating time and improved surgical outcomes.

Summary: This study investigated the natural progression of kidney function in patients with primary hyperparathyroidism (PHPT) and whether parathyroidectomy has an effect on renal outcomes. The results showed that PHPT patients had an accelerated decline in kidney function compared to matched controls, and among patients with hypercalcemia, parathyroidectomy was associated with decreased decline in kidney function.

Summary: This study investigated the attitudes of Irish endocrinologists towards the treatment of hypothyroid and euthyroid patients. The study found that LT4 tablets are the treatment of choice for hypothyroidism, but some endocrinologists may consider combination therapy or prescribing thyroid hormones to euthyroid individuals. Psychosocial factors and comorbidities were identified as the most common reasons for persistent symptoms in treated patients.
