Sex-Related Differences in Pharmacological Response to CNS Drugs: A Narrative Review

Studies show that there are differences in drug response between men and women, particularly in central nervous system drugs. Different genders significantly influence the pharmacokinetics and pharmacodynamics of drugs. Although many studies have conflicting results, there are certain sex-related differences in each drug class. There are differences in response to opioids, antidepressants, and antipsychotics between genders.
In the last decades, both animal and human studies have neglected female subjects with the aim of evading a theorized intricacy of feminine hormonal status. However, clinical experience proves that pharmacological response may vary between the two sexes since pathophysiological dissimilarities between men and women significantly influence the pharmacokinetics and pharmacodynamics of drugs. Sex-related differences in central nervous system (CNS) medication are particularly challenging to assess due to the complexity of disease manifestation, drugs' intricate mechanisms of action, and lack of trustworthy means of evaluating the clinical response to medication. Although many studies showed contrary results, it appears to be a general tendency towards a certain sex-related difference in each pharmacological class. Broadly, opioids seem to produce better analgesia in women especially when they are administered for a prolonged period of time. On the other hand, respiratory and gastrointestinal adverse drug reactions (ADRs) following morphine therapy are more prevalent among female patients. Regarding antidepressants, studies suggest that males might respond better to tricyclic antidepressants (TCAs), whereas females prefer selective serotonin reuptake inhibitors (SSRI), probably due to their tolerance to particular ADRs. In general, studies missed spotting any significant sex-related differences in the therapeutic effect of antiepileptic drugs (AED), but ADRs have sex variations in conjunction with sex hormones' metabolism. On the subject of antipsychotic therapy, women appear to have a superior response to this pharmacological class, although there are also studies claiming the opposite. However, it seems that reported sex-related differences regarding ADRs are steadier: women are more at risk of developing various side effects, such as metabolic dysfunctions, cardiovascular disorders, and hyperprolactinemia. Taking all of the above into account, it seems that response to CNS drugs might be occasionally influenced by sex as a biological variable. Nonetheless, although for each pharmacological class, studies generally converge to a certain pattern, opposite outcomes are standing in the way of a clear consensus. Hence, the fact that so many studies are yielding conflicting results emphasizes once again the need to address sex-related differences in pharmacological response to drugs.

Essential oils (EOs) have shown consistent antimicrobial activity, but their use in clinical settings is limited due to methodological differences and lack of in vivo data. This review explores the concept of AMR, global strategies, and the potential of EOs as alternative therapies against high priority pathogens.

Thermal Stability and Kinetics of Degradation of Moxonidine as Pure Ingredient vs. Pharmaceutical Formulation

Summary: The stability of APIs and pharmaceutical formulations is crucial in pharmaceutical research and technology. Various factors such as light, air exposure, temperature, humidity, formulation composition, and packaging materials can affect the quality and stability of APIs and finished pharmaceutical products. This study analyzed the degradation kinetics of the antihypertensive drug moxonidine in different solid mixtures using FTIR spectroscopy and thermoanalytical methods. The results showed that excipients have a stabilizing effect on moxonidine, but the decomposition mechanism is complex.

Comparison of In Vitro Antimelanoma and Antimicrobial Activity of 2,3-Indolo-betulinic Acid and Its Glycine Conjugates

Summary: Malignant melanoma is a pressing problem in the developing world, and new therapeutic agents are urgently needed. Semisynthesis is an essential method for improving the efficacy and activity of natural product precursors. Two novel semisynthetic derivatives of betulinic acid were designed and showed increased antiproliferative and cytotoxic activity against melanoma cells compared to the parent compound. Additionally, these derivatives exhibited antibacterial and antifungal activity. This study highlights the potential of these derivatives in treating melanoma and microbial infections.

Sex-Related Differences in the Pharmacological Response in SARS-CoV-2 Infection, Dyslipidemia, and Diabetes Mellitus: A Narrative Review

Summary: Pharmacological responses vary by sex in several illnesses. This review summarizes sex differences in pharmaceutical response in SARS-CoV-2 infection, dyslipidemia, and diabetes mellitus. In SARS-CoV-2 infection, men may respond better to genomic vaccinations and females to antiviral medications. In dyslipidemia, women tend to have higher HDL-C and lower LDL-C levels. Statins reduce dementia risk differently in males and females. In diabetes mellitus, females may have a higher risk of developing certain complications. Further research is needed to develop personalized treatment strategies for males and females.

Plasma miR-195-5p predicts the severity of Covid-19 in hospitalized patients

Summary: This study evaluated the early level of circulating miR-195 in Covid-19 patients and found it to be a potential biomarker of disease severity. miR-195 is associated with several clinical and paraclinical parameters and can effectively discriminate between severe and mild forms of the disease. The study also suggests a possible role of miR-195 in long Covid and chronic fatigue syndrome.

Summary: Skin tags, also known as fibroepithelial polyps (FPs) or acrochordons, are soft, pigmented excrescences that occur in 50-60% of the population, mainly in their forties. Various methods like surgical removal, cauterization, laser irradiation, and cryosurgery have been used to eliminate FPs, but over-the-counter treatments are also of interest to patients due to their non-invasive nature. This study aimed to evaluate the efficacy of a modern-pharmaceutical-formulation-type poloxamer-based binary hydrogel with Origanum vulgare L. essential oil as an active ingredient in treating FPs. The results showed positive changes in physiological skin parameters, a reduction in the number of lesions, and histological changes in the epidermis, dermis, and fibrovascular cores of FPs, suggesting the potential of the OEO-PbH treatment.
