Summary: Adrenocortical cancer (ACC) is a rare malignancy with a poor prognosis. The best management approach involves a multidisciplinary team and can improve survival. Even in advanced cases, multimodal treatments and skillful management can lead to significant improvements in survival. However, patients with advanced ACC often suffer from severe physical and psychological symptoms and experience a decline in quality of life. The early integration of supportive and palliative care can alleviate the burden on cancer patients, mediate aggressive treatments, and improve their quality of life.

Summary: A retrospective chart review of 53 patients initiated on continuous palliative inotropes found high rates of documented GOCC, but only slightly more than half of the patients had documented discussions regarding preferences for ICD deactivation. Patients seen by palliative care were more likely to have documented discussions about ICD deactivation, and the frequency of palliative care consultation and documented GOCC conversations increased over time.

Some key unresolved issues regarding early referral to palliative care programs need to be addressed, including patient selection criteria, roles of healthcare providers, and timing for promoting patient awareness and shared decision-making.

Summary: This study aimed to evaluate the prognostic impact of chronic obstructive pulmonary disease (COPD) on mortality in elderly patients with acute and advanced heart failure (HF). The findings show that patients with HF and COPD have more severe symptoms and a worse prognosis compared to patients without COPD. However, many patients with HF and COPD may not receive the assessment and palliative care support they need in the current healthcare setting.
