Summary: This study aimed to evaluate the outcomes of open lower limb fractures in patients aged 65 and over. The results showed that open lower limb fractures in the elderly have similar demographic and mortality characteristics to hip fractures. Limb salvage can be achieved in 93% of cases, with a one-stage surgical procedure being performed in 56% of cases.

Summary: Participants in cardiovascular disease randomized controlled trials often do not represent the diverse population living with the disease. This review discusses the barriers and facilitators of trial enrollment, temporal trends, and the need for representativeness. It proposes strategies to increase representativeness, such as inclusive recruitment practices, diverse trial leadership, and research capacity-building in under-represented regions. Implementing these strategies can generate better and more generalizable evidence to address healthcare inequalities.

Summary: Bone fractures are a global public health concern, but no comprehensive investigation on the use of low-dose aspirin to prevent fractures in the elderly has been conducted. This cross-sectional study found no significant association between the use of low-dose aspirin and fracture prevention in older adults. However, due to the higher incidence of fractures in the elderly, especially in women, it is important to implement more injury prevention initiatives and improve access to osteoporosis prevention and diagnosis to reduce the overall burden.

Summary: This study systematically evaluated the influence of conservatively managed osteoporotic vertebral compression fractures (OVCF) on global sagittal alignment. The results showed that conservatively managed OVCF can lead to changes in pelvic tilt, thoracic kyphosis, pelvic-lumbar angle, sagittal vertical axis, and spino-sacral angle.

Summary: The study showed that distal femur fractures in the elderly are associated with high mortality, with conservative treatment and pre-injury immobility being factors independently associated with death. Conversely, younger age and absence of certain chronic diseases were protective factors for survival. Surgical treatment had better outcomes in terms of survival, regardless of the type of fracture or implant.

The conclusions reached during the meeting were based on the consensus statements developed by working groups.

Summary: This study aimed to answer two questions: (1) What are the incidence rates of proximal femoral fractures in the elderly U.S. population? (2) What is the preferred treatment procedure for these fractures? The results showed that the incidence of proximal femoral fractures in elderly patients in the U.S. has decreased over the past decade. Arthroplasty was the main treatment for head/neck fractures, while intertrochanteric and subtrochanteric fractures were predominantly treated with intramedullary nailing.

Cemented arthroplasty reduces the risk of periprosthetic fractures compared to cementless arthroplasty.

Summary: Pertrochanteric fractures in the elderly often result in disability and reduced quality of life. This study compared the functional and radiological outcomes of patients with pertrochanteric fractures treated with either the Dynamic hip screw or intramedullary Gamma nail fixation. The results showed no significant difference in outcomes between the two treatment groups.
