Background: The current study aims to determine cause-specific survival in patients with high-grade osteosarcoma while reporting risk factors for decreased survival out to 10 years. Methods: The Surveillance, Epidemiology, and End Results (SEER) Program database was used to identify all patients diagnosed with high-grade osteosarcoma from 1991 to 2010. Patient, tumor, and county-level socioeconomic measures were analyzed to determine prognostic factors for survival. Results: Cause-specific 10-year survival for patients with local/regional disease at the time of diagnosis was 65.8%, compared to 24.0% for patients with metastatic disease. Multivariate analysis revealed metastatic disease at presentation, age >= 60 years, male sex, axial location, and size >= 10 cm as independent risk factors for decreased cause-specific survival at 10 years. Patients with the lowest socioeconomic status had a disproportionate frequency of metastatic disease and large tumors at presentation as compared to more affluent patients. Conclusions: Patients with high-grade osteosarcoma have decreased cause-specific survival at 10 years when metastatic at diagnosis, patient age >= 60 years, male sex, axial tumor location, and tumors measuring >= 10 cm. Patients living in low socioeconomic counties present more frequently with metastatic disease and large tumors. Public health efforts should focus on identifying patients with osteosarcoma prior to metastasis. (C) 2015 Elsevier Ltd. All rights reserved.

Preoperative Performance of Patient-Reported Outcomes Measurement Information System in Patients with Meniscal Root Tears

Trevor R. Gulbrandsen, Zain M. Khazi, Matthew Bollier, Brian Wolf, Christopher Larson, Kyle Duchman, Qiang An, Robert W. Westermann

Summary: This study aimed to establish the preoperative validity of PROMIS PF-CT in meniscal root tears patients. The results showed a strong correlation between PROMIS PF-CT and other established PROMs, without floor or ceiling effects. PROMIS PF-CT is proven to be a valuable tool for assessing preoperative patient-reported physical function in patients with MRT.

Cartilage and Meniscus Injuries Are More Common in Patients Undergoing Delayed Multiligament Reconstruction

Alan G. Shamrock, James R. Hall, Christina J. Hajewski, Qiang An, Kyle R. Duchman

Summary: The purpose of this study was to investigate the pattern of meniscus and cartilage pathology in multiligament knee injuries (MLKIs) and determine the relationship between surgical timing and injury mechanism with degree of intra-articular injury. The study found that delayed surgical intervention was associated with increased meniscus and cartilage pathology.

Summary: This study investigated the willingness of extremity sarcoma patients from Canada, the USA, and Spain to participate in a randomized controlled trial, with the majority of patients expressing willingness. Patients' motivations included helping future patients, while barriers included the overwhelming burden of a cancer diagnosis.

Photodynamic Therapy Using Hippo Pathway Inhibitor Verteporfin: A Potential Dual Mechanistic Approach in Treatment of Soft Tissue Sarcomas

Jeffrey D. Rytlewski, Nicholas Scalora, Keith Garcia, Munir Tanas, Fatima Toor, Benjamin Miller, Bryan Allen, Mohammed Milhem, Varun Monga

Summary: Advanced sarcomas have not seen the improved outcomes observed in other cancer subtypes. Verteporfin has the potential to bring about significant changes in sarcoma treatment due to its anti-proliferative properties, including inhibition of the Hippo pathway and photodynamic therapy. Studies suggest that verteporfin could be incorporated into standard sarcoma treatment protocols, leveraging its ability to inhibit the Hippo pathway and induce necrosis through oxidative damage with photodynamic therapy.

Summary: The study validated the SORG machine-learning algorithm for predicting 90-day and 1-year survival in patients with metastatic long-bone disease, showing good discrimination, calibration, overall performance, and decision curve analysis. The developed ML algorithms were found to be effective in predicting survival and may assist clinicians in making informed decisions regarding treatment.

Summary: This randomized clinical trial compared the effectiveness of a 5-day regimen of postoperative intravenous antibiotics with a 1-day regimen in preventing surgical site infections after surgery for lower extremity bone tumors. The study found no significant difference in the rate of surgical site infections between the two regimens. However, the 5-day regimen group had significantly more antibiotic-related complications.

Web-Based Patient Educational Material on Osteosarcoma: Quantitative Assessment of Readability and Understandability

Trevor Robert Gulbrandsen, Mary Kate Skalitzky, Alan Gregory Shamrock, Burke Gao, Obada Hasan, Benjamin James Miller

Summary: This study evaluated the readability, understandability, and actionability of web-based osteosarcoma resources. The results showed that these resources scored poorly in terms of readability, understandability, and actionability, failing to meet the recommended reading level. Only a few resources were considered understandable by the general public. Authors of patient resources should improve these web-based resources to support patient understanding.

Summary: The volume of surgeries performed at hospitals for bone metastatic disease (MDB) does not significantly affect patient outcomes, while medical school affiliation is associated with lower mortality rates. However, being treated at Commission on Cancer accredited hospitals is linked to higher risk of death after surgery.

Transcutaneous Oximetry Does Not Reliably Predict Wound-healing Complications in Preoperatively Radiated Soft Tissue Sarcoma

Lukas M. Nystrom, Nathan W. Mesko, Yuxuan Jin, Chirag Shah, Andre Spiguel, Jeremy White, Benjamin J. Miller

Summary: Surgical wound-healing complications after tumor resections in preoperatively radiated tissue are a significant clinical problem, and there is currently no reliable method to predict these complications. Transcutaneous oximetry has been suggested as a potential predictor, but the evidence is inconclusive. This study found that transcutaneous oximetry cannot reliably predict wound-healing complications, and alternative approaches should be explored.

Summary: Regional lymph node metastasis (RLNM) is uncommon in patients with soft tissue sarcoma (STS), although certain STS subtypes are more prone to RLNM. The impact of node-positive disease on patient survival is still controversial. There is ongoing debate about the benefits of sentinel lymph node biopsy (SLNB) in STS patients. Further multicenter prospective studies are needed to determine the true incidence and potential benefits of SLNB in patients with clinically occult RLNM.

Adjuvant treatment of surgically treated bone metastasis patients: association with hospital characteristics and trends over time

Emma L. Herbach, Bradley D. D. McDowell, Mary Charlton, Benjamin J. J. Miller

Summary: Patients with metastatic disease of the bone (MDB) often require surgical stabilization, but there is no consensus on the subsequent adjuvant treatment. This study aimed to examine the utilization of perioperative adjuvant treatment among MDB patients. The results showed that patients treated at major medical schools or high-volume facilities had higher odds of receiving radiation and chemotherapy, while patients treated at CoC-accredited institutions had higher odds of receiving radiation and bisphosphonates. The use of chemotherapy and bisphosphonates increased over the study period, while the use of radiation declined.
