Alcoholic chlorhexidine skin preparation or triclosan-coated sutures to reduce surgical site infection: a systematic review and meta-analysis of high-quality randomised controlled trials

This study found no benefit from either alcoholic chlorhexidine skin preparation or triclosan-coated sutures, both of which are more expensive than other readily available alternatives. Therefore, global and national guidance should be reconsidered to remove recommendations for their routine use.
Background WHO and the UK's National Institute for Health and Care Excellence recommend alcoholic chlorhexidine skin preparation and triclosan-coated sutures to prevent surgical site infections (SSIs). Existing meta-analyses that include studies at high risk of bias, combined with the recent publication of large, randomised trials, justify an updated meta-analysis of high-quality randomised controlled trials (RCTs). We aimed to test the rates of SSI according to skin preparation solutions (ie, alcoholic chlorhexidine vs aqueous povidone-iodine) and types of sutures (ie, coated vs uncoated). Methods In this systematic review and meta-analysis, we searched MEDLINE, Embase, Pubmed, and Cochrane Library databases, with no language restrictions, to identify high-quality RCTs testing either alcoholic chlorhexidine skin preparation (vs aqueous povidone-iodine) or triclosan-coated sutures (vs uncoated sutures), or both, published from database inception to Sept 1, 2021. Patients who received clean-contaminated, contaminated, or dirty surgery were included. We predefined the characteristics of a high-quality trial through an expert consensus process to develop an enhanced Cochrane risk of bias-2 tool specifically for RCTs with a primary outcome of SSI. Data were extracted from published reports. Meta-analysis was performed using a random-effects model and heterogeneity was assessed using the I-2 statistic. This systematic review and meta-analysis was prospectively registered in PROSPERO, CRD42021267220. Findings Of 942 studies identified, 933 were excluded. Four high-quality RCTs (n=7467 patients) were included that tested alcoholic chlorhexidine. No significant difference in SSI rates was noted between alcoholic chlorhexidine and aqueous povidone-iodine (17 center dot 9% [667 of 3723 patients] vs 19 center dot 8% [740 of 3744 patients]; odds ratio 0 center dot 84 [95% CI 0 center dot 65-1 center dot 06]; p=0 center dot 21, I-2=53 center dot 1%). Five high-quality RCTs were included that tested triclosan-coated sutures (n=8619 patients), with no significant difference noted between triclosan-coated and uncoated sutures (16 center dot 8% [733 of 4360 patients] vs 18 center dot 4% [784 of 4259 patients]; OR 0 center dot 90 [95% CI 0 center dot 74-1 center dot 09]; p=0 center dot 29, I-2=36 center dot 4%). Interpretation Contrary to previous meta-analyses, this study did not show a benefit from either alcoholic chlorhexidine skin preparation or triclosan-coated sutures, both of which are more expensive than other readily available alternatives. The study found that the administrative database frailty instruments performed well in predicting one-year mortality and rehospitalization, with significant improvements in classification among patients who did not experience the event.

Happy to close? The relationship between surgical experience and incisional hernia rates following abdominal wall closure in colorectal surgery

Summary: This study aimed to assess the rates of incisional hernia (IH) at 1 year following abdominal wall closure between junior and senior surgeons in patients undergoing elective colorectal surgery. The results showed that patients closed by trainees had a higher rate of IH (20%) compared to those closed by consultants (12%). Additionally, age, sex, and closure method were identified as risk factors for IH.

Summary: This study investigated the prevalence and nature of sleep disturbance after discharge following hospital admission for COVID-19 and found that sleep disturbance is associated with dyspnoea, anxiety, and muscle weakness. Targeting sleep disturbance might be beneficial in treating the post-COVID-19 condition.

Prognostic implications of the extent of downstaging after neoadjuvant therapy for oesophageal adenocarcinoma and oesophageal squamous cell carcinoma

Summary: This study aimed to evaluate the extent of downstaging in patients with oesophageal adenocarcinoma and oesophageal squamous cell carcinoma receiving neoadjuvant therapy and the difference in outcomes for a similar pathological stage. The results showed that the extent of downstaging is closely associated with patients' survival. Therefore, the optimal neoadjuvant therapy is still controversial, and identifying biomarkers associated with response to neoadjuvant regimens may allow for individualized treatment.

Systematic review of preoperative and intraoperative colorectal Anastomotic Leak Prediction Scores (ALPS)

Summary: This study systematically reviewed preoperative and intraoperative Anastomotic Leak Prediction Scores (ALPS) and validation studies to evaluate their performance and utility in surgical decision-making. It was found that there are numerous ALPS and validation studies, but poor reporting practices and methodological shortcomings limit their wider adoption. Several models appear to perform well in discriminating high-risk patients, but they all raise concerns over risk of bias and wider applicability. Large-scale, precisely reported external validation studies are required.

Summary: This international study examined the impact of global nutritional state on early postoperative outcomes in patients undergoing surgery for colorectal or gastric cancer. Severe malnutrition was found to be a risk factor for 30-day mortality and major complications following elective surgery, with a greater burden in low-income and lower-middle-income countries. Therefore, there is an urgent need for global nutritional interventions to improve early outcomes in patients with gastrointestinal cancer.

Summary: A modified Delphi process and consensus were used to identify essential topics for a locally driven surgical clerkship curriculum in rural Rwanda. The process achieved a strong consensus among experts and demonstrated high reliability. This approach can be used in other contexts to develop relevant curricula.
