Carriage of beta-lactamase-producing Enterobacteriaceae among nursing home residents in north Lebanon

Background: Multidrug-resistant (MDR) Enterobacteriaceae can cause severe infections with high morbidity, mortality, and health care costs. Individuals can be fecal carriers of these resistant organisms. Data on the extent of MDR Enterobacteriaceae fecal carriage in the community setting in Lebanon are very scarce. The aim of this study was to investigate the fecal carriage of MDR Enterobacteriaceae among the elderly residents of two nursing homes located in north Lebanon. Methods: Over a period of 4 months, five fecal swab samples were collected from each of 68 elderly persons at regular intervals of 3-4 weeks. Fecal swabs were subcultured on selective media for the screening of resistant organisms. The phenotypic detection of extended-spectrum beta-lactamase (ESBL), AmpC, metallo-beta-lactamase (MBL), and Klebsiella pneumoniae carbapenemase (KPC) production was performed using the beta-lactamase inhibitors ethylenediaminetetraacetic acid, phenylboronic acid, and cloxacillin. A temocillin disk was used for OXA-48. Multiplex PCRs were used for the genotypic detection of ESBL and carbapenemase genes, and sequencing was performed to identify CTX-M-15. The medical records of each subject were reviewed on a regular basis in order to assess the risk factors associated with MDR Enterobacteriaceae fecal carriage. Results: Over the study period, 76.5% of the recruited elderly persons were at least one-time carriers. A total of 178 isolates were obtained. Phenotypic testing revealed that 91.5% of them were ESBL producers, 4% were AmpC producers, 2.8% were co-producers of ESBL and AmpC, and 1.7% were co-producers of OXA-48 and ESBL. Recent antibiotic intake was found to be the only independent risk factor associated with the fecal carriage of MDR Enterobacteriaceae. Conclusions: The high prevalence of MDR Enterobacteriaceae detected in this study and the emergence of carbapenem resistance is alarming. Efficient infection control measures and antibiotic stewardship programs are urgently needed in these settings in order to limit the spread of resistant strains. (C) 2016 The Authors. Summary: The study found that the prevalence of PCV7 serotypes significantly decreased in the PCV13 era, while PCV13-only serotypes remained stable. Non-vaccine types (NVT) increased, especially serotypes 24 and 16F. Despite a substantial increase in mortality driven by NVT, a significant decrease in antimicrobial resistance (AMR) was observed, indicating a significant impact of PCVs on IPD and AMR in vaccinated and unvaccinated populations.

Determination of MIC, MPC, and MSW of Ilex paraguariensis against non-typhoidal Salmonella with identification of the mechanisms of resistance and factors

Summary: This study investigated the antibacterial activity of Ilex paraguariensis extract against different strains of nontyphoidal Salmonella. The extract showed varying MIC values and a high MPC, resulting in a wide MSW range. Several pathogenicity genes were detected in the Salmonella isolates. The antibacterial activity of the extract was not affected by antimicrobial resistance or pathogenicity genes, suggesting the presence of other active compounds.

Increased Prevalence of EBV Infection in Nasopharyngeal Carcinoma Patients: A Six-Year Cross-Sectional Study

Summary: This study retrospectively collected demographic and clinical data of 146 nasopharyngeal carcinoma (NPC) patients and found a high prevalence (96%) of Epstein Barr Virus (EBV) infection, with genotype I being the most common. EBV infection was associated with lung metastasis but not with metastasis to other sites. The majority of patients were in advanced stages of cancer, and the overall survival mean time was 5.59 years. Further multi-center studies with larger sample sizes are needed to assess the true burden of EBV-associated NPC.

Antibacterial effect of a diet pill supplement on the human intestinal bacteria

Summary: This study aimed to determine the antibacterial effect of commercial diet pills on the intestinal microbiota. The results showed significant inhibition of the growth of intestinal microbiota by the diet pills. Further research is needed to understand the specific effects of the digested components on the intestinal microflora and human health.

Summary: This study describes the epidemiology of non-albicans Candida (NAC) infections and their resistance in Lebanese hospitals. The most common NAC species isolated were Candida glabrata (40.8%) and Candida tropicalis (23.1%). The study also found differing susceptibility to antifungal treatments among these species. Proper identification of these organisms is crucial for effective treatment.

Updates on the Status of Carbapenem-Resistant Enterobacterales in Lebanon

Summary: Carbapenem-resistant Enterobacterales (CRE) pathogens have been increasingly isolated and reported in Lebanon. However, compared to the worldwide data, research on CRE in the country is scarce and mostly limited to single center studies. This review aims to provide a comprehensive and reliable report on the current situation of CRE in Lebanon. Studies have shown a rising trend of carbapenem resistance in Enterobacterales since the first reports of CRE isolates in 2007 and 2008, with Escherichia coli and Klebsiella pneumoniae being the most frequently detected species. OXA-48 class D carbapenemases were the most prevalent among CRE isolates, but the emergence of other carbapenemases like NDM class B carbapenemase has also been observed. Strict infection control measures, including the identification of CRE carriers, are needed in Lebanese hospitals to prevent the spread of CRE. The dissemination of CRE in the community is attributed to various causes such as the refugee crisis, water contamination, and misuse of antimicrobials. In conclusion, strict infection control measures and antimicrobial stewardship programs are urgently needed in Lebanese healthcare settings.

Epidemiology of Community-Acquired Respiratory Tract Infections in Patients Admitted at the Emergency Departments

Summary: The epidemiology of CARTIs in Lebanon is important but understudied. This study presents the first Lebanese data on CARTI pathogens, revealing that viruses are the most common etiologies. Therefore, a different approach is needed for empirical management of CARTI, and rapid testing may help identify patients who require antibiotic therapy.
