Summary: This study compared the clinical feasibility and oncological outcome of different surgical techniques for inguinal lymphadenectomy (ILND) in patients with penile cancer. The results showed that laparoscopic ILND had a shorter hospital stay and lower wound healing delay rate compared to open surgery. However, laparoscopic ILND had a longer drainage time. There were no significant differences in intraoperative parameters, complications, and survival between the two groups. Saphenous vein preservation had limited value in reducing complications. Delayed lymphadenectomy may be a more reasonable option for ILND.

Summary: There is no guideline-based consensus on whether lymph node dissection (LND) should be performed at the time of radical nephroureterectomy (RNU) for upper tract urothelial carcinoma (UTUC). Most clinicians agree that there is a staging benefit to LND for UTUC, but the therapeutic benefits are not as clearly established. Prospective, randomized studies with strict inclusion criteria and defined anatomic templates are needed to definitively characterize the role of LND for UTUC.

In conclusion, LND has no impact on DFS and OS in patients with ULMS.

Summary: This narrative review evaluates the role of lymph node dissection (LND) in upper tract urothelial carcinoma (UTUC), its implications for staging and management outcomes, and future perspectives. LND remains the primary nodal staging tool for UTUC, but its therapeutic benefit is still uncertain. Advances in imaging techniques and preoperative risk assessment show promise in improving lymph node metastasis detection. Further research and multi-center studies are needed to comprehensively assess the advantages and limitations of LND in UTUC, as well as the long-term outcomes of alternative staging and treatment strategies.

Summary: This study aimed to establish signatures for different forms of endocrine hypertension (EHT) and investigate unbiased disease biomarkers. Plasma samples from 13 biobanks across seven countries were analyzed using untargeted NMR metabolomics. While signatures that could differentiate different forms of EHT were found, confounding effects related to sample center and age were also identified. Various approaches were applied to correct these confounding effects, but no biomarkers for differentiating primary hypertension from EHT could be identified.

Summary: This study retrospectively analyzed all patients who received radiotherapy for advanced adrenocortical carcinoma in five European centers since 2000. The results showed that radiotherapy is effective in adrenocortical carcinoma, and higher doses of radiotherapy are associated with longer time to progression of the treated lesion. The toxicity of radiotherapy was generally mild, and no clear predictive factors could be established.

Summary: Recent advances in omics techniques have allowed detailed genetic characterization of cortisol-producing adrenal adenoma (CPA). The current study conducted a comprehensive mass spectrometry imaging (MSI) map of CPAs in relation to clinical phenotypes and immunohistochemical profiles of steroidogenic enzymes. The results revealed novel metabolic links between endocrine function and tumorigenesis, which will further support the understanding of CPA pathophysiology.

Summary: Adrenal incidentalomas are adrenal masses detected on imaging for non-suspected adrenal disease. A revision of the first international guidelines on incidentalomas has been provided, focusing on assessing risk of malignancy, managing mild autonomous cortisol secretion, deciding surgical treatment, and determining follow-up for nonoperated patients. Key recommendations include dedicated adrenal imaging for each adrenal mass, thorough clinical and endocrine work-up to exclude hormone excess, consideration of surgical treatment for patients with mild autonomous cortisol secretion and relevant comorbidities, and appropriate surgical intervention guided by risk of malignancy and hormone excess.

Summary: This systematic review and meta-analysis found that patients with adrenal incidentalomas and mild autonomous cortisol secretion (MACS) have a higher prevalence of comorbidities such as diabetes, hypertension, and dyslipidemia, as well as increased all-cause mortality. Adrenalectomy appears to improve glucometabolic control, hypertension, and dyslipidemia in patients with MACS.

This study compared the prognostic value of DNA-based biomarkers with the S-GRAS score, and found that DNA-based biomarkers play an independent prognostic role and can enhance the predictive accuracy of the S-GRAS score in ACC.

Summary: This study investigated the occurrence of adrenal tumors in relation to sex and adrenal asymmetry. It was found that females were more prone to adrenal cortical carcinoma, aldosterone-producing adenoma, and cortisol-secreting adrenocortical adenomas, while males were more prone to pheochromocytoma. Adrenal tumors were more commonly located on the left side, except for pheochromocytoma which was more commonly on the right. Additionally, females had smaller adrenals compared to males.

It found that LAMN has a certain incidence rate in the German population, with females being more affected. Further research and the establishment of a specialized registry are necessary.
