Studies have shown that medications targeting the TNF-alpha pathway may trigger manic episodes in patients with or without affective disorders, but further prospective research is needed to evaluate the relative risk of such side effects and identify the population susceptible to secondary mania.
A growing number of studies support a bidirectional relationship between inflammation and bipolar disorders. Tumor necrosis factor-alpha (TNF-alpha) inhibitors have recently attracted interest as potential therapeutic compounds for treating depressive symptoms, but the risk for triggering mood switches in patients with or without bipolar disorders remains controversial. Thus, we conducted a systematic review to study the anti-TNF-alpha medication-induced manic or hypomanic episodes. PubMed, Scopus, Medline, and Embase databases were screened for a comprehensive literature search from inception until November 2020, using The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Out of the initial 75 references, the screening resulted in the inclusion of four case reports (each describing one patient) and a cohort study (in which 40 patients out of 7600-0.53% - experienced elated mood episodes after infliximab administration). Of these 44 patients, 97.7% experienced a manic episode and 2.3% hypomania. 93.2% of patients had no history of psychiatric disorder or psychotropic treatment. Only 6.8% had a history of psychiatric disorders with the affective spectrum (4.6% dysthymia and 2.3% bipolar disorder). The time of onset of manic or hypomanic symptoms varied across TNF-alpha inhibitors with an early onset for Infliximab and a later onset for Adalimumab and Etanercept. These findings suggest that medications targeting the TNF-alpha pathway may trigger a manic episode in patients with or without affective disorders. However, prospective studies are needed to evaluate the relative risk of such side effects and identify the population susceptible to secondary mania.

Summary: This study conducted a long-term follow-up on 5 patients with treatment-resistant depression (TRD) who received vagus nerve stimulation (VNS) implant. The results showed that VNS had long-lasting effectiveness in improving symptoms and functioning in patients with severe and chronic depression. The study supports VNS as a viable treatment option for TRD due to its sustained efficacy and good tolerability.

Summary: This study conducted a systematic review and meta-analysis to investigate the association between attention-deficit/hyperactivity disorder (ADHD) and eye disorders as well as altered visual function. The findings suggest that ADHD is associated with an increased risk of certain vision problems and visual function impairments, but not with structural changes in the eye.

Summary: This study utilized datasets from Brazil, China, Europe, Korea, the UK, and the US to investigate the association between handgrip strength and depression in middle and older aged adults. The results showed that lower handgrip strength was associated with an increased likelihood of depression. Early assessment of handgrip strength may help identify individuals at risk for depression among this population.

Self-stigma in schizophrenia: a systematic review and meta-analysis of 37 studies from 25 high- and low-to-middle income countries

Summary: In schizophrenia, self-stigma is influenced by psychotic and depressive symptomatology, workplace stigma, and varies across countries. This meta-analysis synthesizes observational studies on self-stigma and its associated factors. The results show that self-stigma levels remain consistent over time and are influenced by factors such as living in non-urban areas, low income, unemployment, high antipsychotic dose, and low functioning. Additionally, self-stigma dimensions differ between European and other regions. The study highlights the need to explore additional factors to improve interventions targeting self-stigma.

Summary: This study conducted a systematic meta-analysis to investigate the comorbidity of mental disorders in individuals at clinical high risk for psychosis (CHR-P). The results showed that over three-quarters of CHR-P subjects have comorbid mental disorders, which significantly impact their baseline functioning and transition to psychosis. Therefore, it is crucial to conduct transdiagnostic mental health assessments in individuals at CHR-P.

Twelve years after the ARRIVE guidelines: Animal research has not yet arrived at high standards

Summary: The reproducibility crisis in animal studies threatens the credibility of their findings, which are important for human studies. The ARRIVE reporting guidelines aim to improve transparency, but compliance and awareness are still suboptimal. Alongside reporting guidelines, better training, improved research design, registration practices, and aligning rewards with rigorous animal research may be more effective in improving reproducibility.

Summary: The results of this systematic review and meta-analysis show that the risk of premature death in patients with bipolar disorder is not only related to suicide and unnatural causes, but also to physical comorbidities. Therefore, in addition to suicide prevention, promoting physical health and preventing physical conditions in individuals with bipolar disorder may reduce premature mortality risk.

Summary: This study conducted a network meta-analysis on antipsychotic-induced sialorrhea. The results showed that dopamine receptor antagonists, metoclopramide, and sulpiride were effective in treating sialorrhea, while antihistamines and atropine showed no significant effect on nocturnal sialorrhea. This study provides some guidance for the treatment of antipsychotic-related sialorrhea.
