Summary: This study investigated the etiology and clinical characteristics of Staphylococcus aureus infective endocarditis (SA IE) after transcatheter aortic valve replacement (TAVR). It found that SA IE accounted for approximately 25% of all IE cases after TAVR and was associated with high rates of in-hospital and late mortality. Summary: By including Enterococcus faecalis as a typical endocarditis bacterium regardless of the place of acquisition or the portal of entry, the sensitivity to correctly identify definite endocarditis can be improved from 70% (modified Duke criteria) to 96% (enterococcal adjusted Duke criteria) based on data from a prospective study of 344 patients with E. faecalis bacteremia evaluated with echocardiography.

Summary: This study analyzed the risk factors affecting the treatment outcome of linezolid for vancomycin-resistant Enterococcus bloodstream infection. The study found that the linezolid minimum inhibitory concentration and concentration-time curve/MIC were not associated with mortality, while the linezolid dose per body weight, body height, and the interaction between them were independent risks for mortality. In conclusion, the clinical outcomes of linezolid treatment for vancomycin-resistant Enterococcus bloodstream infection are associated with the dose and body height.

Summary: Despite advances in diagnostic and treatment, morbidity and mortality due to infective endocarditis (IE) has not decreased. There is a discrepancy in epidemiology of IE between developed and developing countries. Over the last years, increased early detection and consequently prevalence of rheumatic heart disease (RHD) and congenital heart disease (CHD) which are considered predisposing conditions for IE, is noted. This review shows a scarcity of studies on IE in developing countries. Rheumatic heart disease and congenital heart disease are common predisposing conditions. Other risk factors are prosthetic valves, degenerative valve disease (DVD), intravenous drug use, and human immunodeficiency virus infection. While the proportion of IE cases caused by Streptococcus and Staphylococcus has increased, the number of negative blood culture (NBC) and patients getting surgery has decreased. Mortality has not changed over time. Timely diagnosis and management of patients with RHD and CHD and comprehensive management of IE are warranted.

Summary: This study investigated the association between vegetation size and embolic events (EEs) and 30-day mortality in left-sided infective endocarditis (LSIE), and identified risk predictors for these outcomes. The results showed that vegetation size >10 mm was associated with increased risk of EEs or 30-day mortality in LSIE. Other risk predictors included EE on admission, history of heart failure, elevated creatinine level, Staphylococcus aureus infection, congestive heart failure, presence of haemorrhagic stroke, alcohol abuse, presence of cardiogenic shock, and not performing left surgery. Prognosis after LSIE is influenced by multiple factors, including vegetation size.

Summary: Time to blood culture positivity (TTP) is an indirect measure of bacterial concentration in blood. A short TTP has been linked to infective endocarditis (IE) and poor prognosis in Staphylococcus aureus bacteremia. Factors associated with IE were investigated, and TTP remained associated with IE when adjusted for age, gender, comorbidity, and nosocomial acquisition, with no association between TTP and mortality.
