Sepsis prediction, early detection, and identification using clinical text for machine learning: a systematic review

Studies have shown that utilizing both unstructured text and structured data in machine learning can improve the identification and early detection of sepsis, with fewer studies focusing on predicting patient histories beyond the current episode of care.
Objective To determine the effects of using unstructured clinical text in machine learning (ML) for prediction, early detection, and identification of sepsis. Materials and methods PubMed, Scopus, ACM DL, dblp, and IEEE Xplore databases were searched. Articles utilizing clinical text for ML or natural language processing (NLP) to detect, identify, recognize, diagnose, or predict the onset, development, progress, or prognosis of systemic inflammatory response syndrome, sepsis, severe sepsis, or septic shock were included. Sepsis definition, dataset, types of data, ML models, NLP techniques, and evaluation metrics were extracted. Results The clinical text used in models include narrative notes written by nurses, physicians, and specialists in varying situations. This is often combined with common structured data such as demographics, vital signs, laboratory data, and medications. Area under the receiver operating characteristic curve (AUC) comparison of ML methods showed that utilizing both text and structured data predicts sepsis earlier and more accurately than structured data alone. No meta-analysis was performed because of incomparable measurements among the 9 included studies. Discussion Studies focused on sepsis identification or early detection before onset; no studies used patient histories beyond the current episode of care to predict sepsis. Sepsis definition affects reporting methods, outcomes, and results. Many methods rely on continuous vital sign measurements in intensive care, making them not easily transferable to general ward units. Conclusions Approaches were heterogeneous, but studies showed that utilizing both unstructured text and structured data in ML can improve identification and early detection of sepsis.

Summary: Through in-depth genetic association analysis of COVID-19 patients and population controls, this study has identified genetic loci associated with severe COVID-19 and explored the potential effects of these loci. One association at 17q21.31 was found to be related to an inversion polymorphism creating two highly differentiated haplotypes. Additionally, a new locus at 19q13.33 was discovered, including the NAPSA gene which is primarily expressed in alveolar cells responsible for gas exchange in the lung.

Summary: Observational studies have found an association between iron status and risk of sepsis and COVID-19. Using genetically-predicted iron biomarkers, this study demonstrates a causal effect of higher iron status on the risk of sepsis hospitalization and indications of an increased risk of being hospitalized with COVID-19.

Summary: This study found that the risk of AMI in individuals with elevated albuminuria but with a low cardiovascular disease (CVD) risk profile may be amplified by anxiety and depression symptoms. The approach taken in this study makes ontology development understandable and accessible to domain experts without formal ontology training.

Trends in mortality after a sepsis hospitalization: a nationwide prospective registry study from 2008 to 2021

Summary: This study aims to examine trends in short- and long-term mortality among sepsis patients and investigate the association between clinical characteristics and mortality. Data was collected from all hospitals in Norway from 2008 to 2021, and the analysis showed a decline in overall mortality over the years. Factors such as comorbidity, site of infection, and acute organ dysfunction were found to be associated with mortality.

Summary: This study analyzed the temporal trends in incidence rate and case fatality from 2008 to 2021 and assessed the possible shifts in these trends during the COVID-19 pandemic. The study found that the incidence rate of first sepsis remained stable, but the incidence rate of recurrent sepsis increased. During the COVID-19 pandemic, the incidence rate of both first sepsis and all sepsis cases decreased. The case fatality of first sepsis decreased from 2009 to 2019 but increased during the COVID-19 pandemic.
