Summary: Long-term follow-up study of 25 patients with subarachnoid hemorrhages due to blood blister-like aneurysms (BBAs) treated with direct clipping or clip reinforcement with or without direct neck repair showed favorable clinical outcomes and indicated that direct surgical clipping or clip reinforcement with a silicone sheet appeared to be a curative surgery.

Summary: Blood blister-like aneurysms (BBAs) are rare and typically found at nonbranching sites in the supraclinoid portion of the internal carotid artery (ICA). The pathogenesis of BBAs remains uncertain due to challenges in obtaining histological specimens of the aneurysm wall and establishing experimental models. Current evidence suggests that atherosclerosis and dissection are key contributors to BBA formation, and hemodynamics may also play a role. Further research on histopathology and hemodynamics is needed to better understand BBAs.

Summary: This study presents three cases of blood blister-like aneurysms (BBAs) located at the origin of the posterior communicating artery (PCoA). The findings indicate that these lesions mainly occur in elderly females with wide-angled fetal-type PCoAs. The treatment options include direct clipping and wrap-clipping with additional techniques.

Summary: This study utilized single-cell RNA sequencing and pathological validation to investigate the transcriptomic landscape and potential pathogenesis of non-branching site blood blister-like aneurysms (BBA). The findings revealed upregulated genes related to arterial mineralization and atherosclerosis in BBA-derived vascular smooth muscle cells (VSMCs), as well as the involvement of macrophages and mural cells in the inflammation process. These results shed light on the pathophysiology of intracranial aneurysms and provide insights into aneurysm pathogenesis.

Summary: Clinical observations suggest that the shape and tortuosity of the carotid siphon play a role in the development of an aneurysm. This study uses numerical simulations to investigate this observation. Computational fluid dynamics (CFD) calculations are performed on four types of patient-specific carotid siphons, and it was found that fewer curved regions and local acute curvature result in higher velocity magnitude and giant sidewall aneurysms. On the other hand, a larger number of curved regions in the parent vessel lead to disturbed flow and smaller aneurysms at the bifurcation carina. The tortuosity of the siphon is also shown to influence the helicity and potential growth of an aneurysm.

Summary: The study revealed that blood blister-like aneurysms (BBAs) lack certain cell types in the arterial wall compared to saccular aneurysms, and are enriched with inflammatory cells, especially macrophages. Reruptured BBAs showed significantly reduced fibroblasts and adventitial collagen, potentially affecting the integrity of the aneurysm.

Summary: LVIS stent-assisted coil embolization is effective in preventing re-rupture of BBAs. However, the morphology of the aneurysm may change within a short period, so careful angiographic follow-up is needed. Appropriate preoperative antiplatelet administration and optimal timing of the treatment may reduce the risk of postoperative ischemic complication.
