Associations of Atrial Fibrillation Patterns With Mortality and Cardiovascular Events: Implications of the MISOAC-AF Trial

This retrospective cohort study found that in recently hospitalized patients with comorbid atrial fibrillation (AF), the presence of persistent or permanent AF was associated with a higher incidence of mortality and morbidity compared with paroxysmal and first diagnosed AF.
Aim: This retrospective cohort study aimed to evaluate the prognostic implications of the distinct atrial fibrillation (AF) temporal patterns: first diagnosed, paroxysmal, and persistent or permanent AF. Methods: In this post hoc analysis of the MISOAC-AF trial (NCT02941978), a total of 1052 patients with AF (median age 76 years), discharged from the cardiology ward between 2015 and 2018, were analyzed. Kaplan-Meier and Cox-regression analyses were performed to compare the primary outcome of all-cause mortality, the secondary outcomes of stroke, major bleeding and the composite outcome of cardiovascular (CV) mortality or hospitalization among AF patterns. Results: Of patients, 121 (11.2%) had first diagnosed, 356 (33%) paroxysmal, and 575 (53.2%) persistent or permanent AF. During a median follow-up of 31 months (interquartile range 10 to 52 months), 37.3% of patients died. Compared with paroxysmal AF, patients with persistent or permanent AF had higher mortality rates (adjusted hazard ratio (aHR), 1.37; 95% confidence interval [CI], 1.08-1.74, P = .009), but similar CV mortality or hospitalization rates (aHR, 1.09; 95% CI, 0.91-1.31, P = .35). Compared with first diagnosed AF, patients with persistent or permanent AF had similar mortality (aHR, 1.26; 95% CI, 0.87-1.82, P = .24), but higher CV mortality or hospitalization rates (aHR, 1.35; 95% CI, 1.01-1.8, P = .04). Stroke and major bleeding events did not differ across AF patterns (all P > .05). Conclusions: In conclusion, in recently hospitalized patients with comorbid AF, the presence of persistent or permanent AF was associated with a higher incidence of mortality and morbidity compared with paroxysmal and first diagnosed AF.

Summary: Congenital heart disease is the most common cardiovascular condition in individuals with Down syndrome, affecting up to 50% of patients. Other factors that contribute to cardiovascular outcomes include pulmonary hypertension, pulmonary, endocrine, and metabolic diseases, and risk factors for atherosclerotic disease. Disparities in cardiovascular care for individuals with Down syndrome, which vary across different locations and healthcare systems, are often overlooked. This review provides a comprehensive summary of the diagnosis, prevalence, and management of cardiovascular disease in patients with Down syndrome, as well as addressing disparities in care based on resource availability.

Summary: This study analyzed the changes in RV function in patients with intermediate-risk PE who were switched early to oral anticoagulation. The results showed that the majority of patients had normalized RV function within 6 days and maintained normal RV function throughout the first 6 months. However, almost one in four patients continued to have evidence of RV dysfunction over the long term.

Summary: In patients with heart failure and atrial fibrillation, the use of evidence-based target doses of RAAS inhibitors and beta-blockers is often underused in real-world conditions. A retrospective cohort study found that only 30.7% of patients received target doses of guideline-directed medical treatment (GDMT) medications at 3 months after discharge. Patients with lower eGFR levels had a lower proportion of optimally targeted GDMT achievement. Nontargeted GDMT was associated with a higher risk of mortality and cardiovascular events.

Changes in CHA2DS2-VASc score and risk of ischemic stroke among patients with atrial fibrillation

Summary: This study aimed to assess the association between changes in CHA(2)DS(2)-VASc score over time and the risk of ischemic stroke. The findings showed that an increase in CHA(2)DS(2)-VASc score was associated with a higher risk of stroke. The follow-up and Delta CHA(2)DS(2)-VASc scores were found to be more accurate predictors of stroke risk compared to the baseline score.

Summary: The proportion of very elderly patients, octogenarians and nonagenarians, is expected to increase over time. These patients are more prone to age-related diseases with higher thromboembolic and bleeding risks. Although clinical trials have under-represented the very elderly, real-world evidence suggests that OAC treatment is beneficial for this population.

Summary: This study evaluated the association of intradialytic hypertension (IDHTN) with cardiovascular events and mortality and the influence of ambulatory blood pressure (BP) and other cardiovascular risk factors on these associations. The results showed that patients with IDHTN had higher risk for mortality and cardiovascular outcomes. However, after adjusting for ambulatory BP, the association between IDHTN and these outcomes became nonsignificant.
