Summary: This study aimed to investigate the mortality and hospitalization rates following atrial fibrillation (AF) diagnosis. The study found that cardiovascular and cerebrovascular mortality rates have declined, while non-cardio/cerebrovascular mortality rates have remained stable. The overall hospitalization rates have increased, particularly for non-cardio/cerebrovascular causes.

Summary: More than one-third of atrial fibrillation (AF) patients in this study were hospitalized at least once during a follow-up of over 3 years. Patients aged 65 years or older were mainly hospitalized for non-cardiovascular diseases rather than AF. Patients with comorbidities such as heart failure, coronary artery disease, ischemic stroke/transient ischemic attack, diabetes, chronic obstructive pulmonary disease, gastrointestinal disorder, and renal dysfunction had higher risks of hospitalization.

Summary: This study aims to assess the bidirectional association between heart failure and atrial fibrillation using real-world data. The findings suggest that atrial fibrillation is associated with an increased risk of acute heart failure hospitalization, decline of renal function, and all-cause mortality. Furthermore, atrial fibrillation patients have a higher number of hospitalizations for acute heart failure, but a reduced risk of ischemic stroke, possibly due to anticoagulant treatment.

Summary: This study developed a decision aid to guide the disposition of atrial fibrillation patients in the emergency department, and found that its implementation was associated with a reduction in hospitalization rate. However, the use of the aid also led to a slightly increased rate of return visits for atrial fibrillation within 30 days.

Summary: Among Korean dialysis patients, atrial fibrillation (AF) is a significant risk factor for various outcomes, including all-cause mortality, hospitalization, and stroke events. The use of warfarin is associated with an increased risk of hemorrhagic stroke, but not with other outcomes such as all-cause mortality, hospitalization, and ischemic stroke.

Summary: Congenital heart disease is the most common cardiovascular condition in individuals with Down syndrome, affecting up to 50% of patients. Other factors that contribute to cardiovascular outcomes include pulmonary hypertension, pulmonary, endocrine, and metabolic diseases, and risk factors for atherosclerotic disease. Disparities in cardiovascular care for individuals with Down syndrome, which vary across different locations and healthcare systems, are often overlooked. This review provides a comprehensive summary of the diagnosis, prevalence, and management of cardiovascular disease in patients with Down syndrome, as well as addressing disparities in care based on resource availability.

Summary: This study analyzed the changes in RV function in patients with intermediate-risk PE who were switched early to oral anticoagulation. The results showed that the majority of patients had normalized RV function within 6 days and maintained normal RV function throughout the first 6 months. However, almost one in four patients continued to have evidence of RV dysfunction over the long term.

Summary: In patients with heart failure and atrial fibrillation, the use of evidence-based target doses of RAAS inhibitors and beta-blockers is often underused in real-world conditions. A retrospective cohort study found that only 30.7% of patients received target doses of guideline-directed medical treatment (GDMT) medications at 3 months after discharge. Patients with lower eGFR levels had a lower proportion of optimally targeted GDMT achievement. Nontargeted GDMT was associated with a higher risk of mortality and cardiovascular events.

Summary: This study aimed to assess the association between changes in CHA(2)DS(2)-VASc score over time and the risk of ischemic stroke. The findings showed that an increase in CHA(2)DS(2)-VASc score was associated with a higher risk of stroke. The follow-up and Delta CHA(2)DS(2)-VASc scores were found to be more accurate predictors of stroke risk compared to the baseline score.

Summary: The proportion of very elderly patients, octogenarians and nonagenarians, is expected to increase over time. These patients are more prone to age-related diseases with higher thromboembolic and bleeding risks. Although clinical trials have under-represented the very elderly, real-world evidence suggests that OAC treatment is beneficial for this population.

Summary: This study evaluated the association of intradialytic hypertension (IDHTN) with cardiovascular events and mortality and the influence of ambulatory blood pressure (BP) and other cardiovascular risk factors on these associations. The results showed that patients with IDHTN had higher risk for mortality and cardiovascular outcomes. However, after adjusting for ambulatory BP, the association between IDHTN and these outcomes became nonsignificant.

Summary: With the increase in age of CHD patients, the prevalence of atrial arrhythmias is expanding. Managing comorbidities and using pharmacotherapy and ablation therapy are important in arrhythmia management. However, recommendations for the use of implantable cardioverter defibrillators for primary prevention in CHD patients, except for tetralogy of Fallot patients, are still not well established.
