Summary: This study found that monitoring ctDNA status in plasma can detect the risk of recurrence in colorectal liver metastases patients earlier and can potentially assist in clinical decision-making in cases of indeterminate CT findings, reducing time-to-intervention.

Summary: Pancreatic ductal adenocarcinoma has a high rate of recurrence after resection, with liver metastasis being the most common distant site of recurrence. Patients with poorly differentiated tumors are more likely to recur in the liver. Liver-directed therapies should be a focus for future studies.

Summary: This study investigates the impact of adjuvant therapy (AT) on the long-term survival of patients who underwent curative resection for distal cholangiocarcinoma (DCC). The results show that AT is commonly used among patients with DCC, and it significantly improves long-term survival in patients with perineural invasion (PNI) or lymph node metastasis (LNM).

Summary: Prognostic factors for early recurrence after surgery for perihilar cholangiocarcinoma include vascular invasion, lymph node metastases, and R1 resection. Tumor size, poor tumor differentiation, and perineural invasion are also associated with an increased risk of early recurrence.

Summary: Hepatocellular carcinoma (HCC) recurrence is a major issue post liver resection, with clinical factors and loss of heterozygosity in the PTEN locus playing a role in predicting and potentially reducing the risk of recurrence. Multiple cancer gene alterations are associated with HCC progression, with specific AI mutations presenting a protective effect on recurrence risk.

Summary: The study aimed to develop a dynamic prognostic model for patients undergoing curative-intent resection for intrahepatic cholangiocarcinoma (ICC) using landmark analysis. The model incorporated selected variables and their interaction with time to predict overall survival (OS) over time. The model demonstrated good discrimination ability and accuracy in internal validation.

Summary: Both TBS and serum AFP are important predictors of prognosis among patients with resectable HCC. They have a synergistic impact on prognosis, with higher serum AFP predicting worse outcomes among patients with HCC of a certain TBS class.

Summary: This case report describes a rare case of isolated gallbladder recurrence after curative resection of hepatocellular carcinoma (HCC) and reviews the relevant literature. No similar cases have been reported before. Surgery should be the preferred option in patients with isolated gallbladder metastasis if the lesion can be resected en bloc without remnants.

Summary: This study investigated the role of LS and metabolic factors in predicting HCC recurrence for patients with early stage HCC. It was found that LS was positively correlated with fibrosis stage, and patients with LS>8.5 kPa and HOMA-IR>2.3 had the highest recurrence rate after 1-year post resection.

Summary: This study aimed to characterize the peak hazard rates and peak time of recurrence among patients who underwent resection of HCC. The results showed that the hazard of recurrence varied with time and intensity, and the BCLC stage and AFP level significantly impacted the risk of recurrence.

Summary: This study identified that high tumor grade, perineural invasion, and N0 node status of the primary tumor are associated with an increased risk of liver recurrence after curative-intent treatment of colorectal liver metastases. These factors represent a target population that may benefit the most from adjuvant liver-directed regional chemotherapy.

Summary: Several adjuvant treatments have been evaluated for preventing HCC recurrence after curative liver resection, with antiviral therapy and postoperative adjuvant transarterial chemoembolization showing promising results. However, further large-scale randomized controlled trials are needed to confirm their efficacy in different patient populations.

Summary: The study evaluated the clinical significance of surgical resection for liver recurrence in patients with curatively resected pancreatic ductal adenocarcinoma. The medical records of patients with liver recurrence after curative pancreatectomy were reviewed, and the clinical impact of liver resection was analyzed. In selected patients with solitary liver recurrence, liver resection may be a treatment option.

Summary: Liver cirrhosis is a common risk factor for hepatocellular carcinoma (HCC), but a small percentage of HCC cases occur in non-cirrhotic livers. This study found that liver resection for non-cirrhotic HCC is an acceptable treatment approach with relatively good outcomes, even for extensive tumors.

Summary: Among patients with distal cholangiocarcinoma (DCC) undergoing resection, one in four patients experienced early recurrence (ER). Factors associated with a higher ER risk included neutrophil-to-lymphocyte ratio (NLR), peak total bilirubin (T-Bil), major vascular resection (MVR), and lymphovascular invasion. An early recurrence score based on these factors may help identify patients at higher risk for ER.

Summary: Objective of this study was to evaluate the effect of intraoperative blood cell salvage and autotransfusion (IBSA) use on red blood cell (RBC) transfusion and postoperative outcomes in liver surgery. The results showed that IBSA may reduce intraoperative allogeneic RBC transfusion without compromising oncologic outcomes, but the current evidence base is limited in size and quality, and high-quality randomized controlled trials are needed.

Summary: Despite the increase in living donor liver transplants, the outcomes of retransplantation after living donor liver transplant need further investigation. This study compared the waitlist outcomes and survival post-retransplant in recipients of initial living or deceased donor grafts. The results showed similar waitlist mortality and survival rates for both types of transplants.

Summary: Many countries have national registries of liver transplant data, which are used to generate predictive models. However, the performance and transferability of these models are unclear. This study developed machine learning algorithm-based models using data from 3 national registries to predict 90-day post-transplant mortality and evaluated the predictive performance and external validity of each model.

Summary: The absolute lymphocyte count (ALC) is a surrogate marker for nutritional status and immunocompetence. This study investigates the association between ALC and post-liver transplant outcomes in deceased donor liver transplant (DDLT) patients. The results show that patients with low ALC have higher 180-day mortality, as well as increased incidence of bacteremia and cytomegaloviremia.

Summary: This study evaluated the management of combined hepatocellular-cholangiocarcinoma (cHCC-CCA) through an online survey sent to expert centres. The results showed that liver resection is considered the first-line treatment for cHCC-CCA, but there are significant differences in treatment decisions among different specialties and regions. Therefore, a multicentre prospective trial is needed to compare different treatments and optimize the therapeutic management of cHCC-CCA.

Summary: Liver transplantation offers the best chance of cure for most non-metastatic hepatocellular carcinoma patients. Downstaging and bridging therapies, such as ablation and transarterial chemoembolization, can be used to lower the patient's condition and keep them within transplant criteria. Immunotherapy shows great potential in supporting downstaging and bridging therapies.

Summary: This study investigated the impact of pre-transplant treatment for hepatocellular carcinoma (HCC) on the risk of posttransplant recurrence. The results showed that pretreatment reduced the risk of recurrence, and patients who achieved pathologically complete response (pCR) had a lower risk of recurrence. Furthermore, multiple treatments were associated with a higher risk of post-transplant recurrence.
