Treatment with outgrowth endothelial cells protects cerebral barrier against ischemic injury

This experimental study shows that OEC-based cell therapy restores BBB integrity in an effective manner by integrating into the resident cerebral microvascular network, suppressing oxidative stress and cellular apoptosis.
Background and aims: We have previously reported that outgrowth endothelial cells (OECs) restore cerebral endothelial cell integrity through effective homing to the injury site. This study further investigates whether treatment with OECs can restore blood-brain barrier (BBB) function in settings of ischemia-reperfusion injury both in vitro and in vivo. Methods: An in vitro model of human BBB was established by co-culture of astrocytes, pericytes, and human brain microvascular endothelial cells (HBMECs) before exposure to oxygen-glucose deprivation alone or followed by reperfusion (OGD +/- R) in the absence or presence of exogenous OECs. Using a rodent model of middle cerebral artery occlusion (MCAO), we further assessed the therapeutic potential of OECs in vivo. Results: Owing to their prominent antioxidant, proliferative, and migratory properties, alongside their inherent capacity to incorporate into brain vasculature, treatments with OECs attenuated the extent of OGD +/- R injury on BBB integrity and function, as ascertained by increases in transendothelial electrical resistance and decreases in paracellular flux across the barrier. Similarly, intravenous delivery of OECs also led to better barrier protection in MCAO rats as evidenced by significant decreases in ipsilateral brain edema volumes on day 3 after treatment. Mechanistic studies subsequently showed that treatment with OECs substantially reduced oxidative stress and apoptosis in HBMECs subjected to ischemic damages. Conclusion: This experimental study shows that OEC-based cell therapy restores BBB integrity in an effective manner by integrating into resident cerebral microvascular network, suppressing oxidative stress and cellular apoptosis. Summary: Stroke is the third-leading cause of death and disability worldwide, and the limited availability of diagnostic tools, therapeutics, and biomarkers remains a major challenge. Recent research has discovered the potential of miRNAs, non-coding RNAs that regulate gene expression, as reliable blood-based markers for predicting, diagnosing, and prognosing ischemic stroke. Additionally, miRNA treatments have shown potential neuroprotective effects in animal models of ischemic stroke. However, further research is needed to determine the optimal treatment timing and route of administration.

Therapeutic hypothermia augments the restorative effects of PKC-β and Nox2 inhibition on an in vitro model of human blood-brain barrier

Summary: Therapeutic hypothermia enhances the protective effects of PKC-beta or Nox2 inhibition on BBB integrity and function in the setting of hyperglycaemia. Combining PKC-beta or Nox2 inhibition with therapeutic hypothermia is more effective in repairing physical damage, decreasing Nox activity and superoxide anion production, and suppressing actin stress fibre formation compared to using either agent alone. Additionally, the combinatory approach is more effective in maintaining normal cytoskeletal structure.

Summary: OECs show potential for repairing BBB integrity and function following ischemic damage, with greater resistance to oxidative stress compared to HBMECs. Targeting NOX2 to suppress oxidative stress may help restore BBB integrity and function, but caution is needed when using OECs as therapeutics.

Summary: This study comparatively assessed the effect of PKC-beta inhibitor LY333531 on the cerebral barrier integrity formed by OECs or HBMECs. The results showed that LY333531 protected the barrier formed by HBMECs and exacerbated the damage in OECs. Furthermore, a lower concentration of LY333531 (0.01μM) had a smaller impact on the damage.

Geospatial analysis of type 2 diabetes mellitus and hypertension in South Sulawesi, Indonesia

Summary: The spatial variation and potential linkage of type 2 diabetes and hypertension were explored in South Sulawesi Province, Indonesia. Global Moran's I and regression analysis were used to identify the characteristics involved. The analysis was based on data from 2017 and 2018 acquired from Social Health Insurance Administration in Indonesia. The findings showed that type 2 diabetes and hypertension have a random spatial distribution, and there is a significant relationship between the two conditions.

Outgrowth Endothelial Cell Conditioned Medium Negates TNF-α-Evoked Cerebral Barrier Damage: A Reverse Translational Research to Explore Mechanisms

Summary: Improved understanding of the mechanisms underlying cerebral ischemic injury is crucial for developing effective therapeutics for stroke. This study found significant elevations in TNF-alpha levels in plasma samples from ischemic stroke patients at different time points after stroke. The inflammatory cytokine was found to impair blood-brain barrier integrity and function, leading to early death. However, co-treatment with outgrowth endothelial cell-derived conditioned media (OEC-CM) mitigated the detrimental effects of TNF-alpha on the blood-brain barrier. OEC-CM showed vasculoprotective effects by suppressing anti-angiogenic factors, reducing oxidative stress and apoptosis, and improving the adhesive and tubulogenic properties of brain microvascular endothelial cells and OECs.

The secretome of endothelial progenitor cells: a potential therapeutic strategy for ischemic stroke

Summary: Ischemic stroke remains a significant cause of mortality and morbidity worldwide. While thrombolysis with recombinant tissue plasminogen activator is the primary pharmacological therapy, its narrow therapeutic window limits its application to less than 1% of stroke patients globally each year. Endothelial progenitor cells (EPCs) have potential as a therapeutic beyond the acute phase of stroke, but their limited availability and early senescence in culture conditions hinder their efficacy.
